What Will You Do: 
Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.
 Utilizes interview, observation and evaluation in assessing clients and applies nursing judgment, consistent with practice standards, in formulating nursing interventions and making recommendations to the physician, client/family and IDT/IDG.
 Report changes in client condition as appropriate and in a timely manner, to the client’s physician and/or Case Manager/designee and obtain orders for changes in the plan of treatment to respond to the client’s condition.
 Re-evaluates and updates patients’ plan of care based on patient goals and progress towards outcomes.
 Assess patient and family learning styles and needs for teaching regarding disease process, self-care, end of life care, and dealing with ethical concerns as well as patient goals as part of plan of care.
 Respond appropriately to changes in patients’ physical, psychological, or spiritual conditions.

Graduate of an approved nursing education program
 Active RN license in the State of Ohio
 1+ years of clinical nursing experience (Hospice preferred)

Job Summary:
Coordinates, facilitates and performs interdisciplinary plan of care through assessment and evaluation of patient condition and effectiveness of care under the direction of the attending physician by traveling to patient homes, Community-Based Residential Facilities (CBRF), assisted living locations, hospitals or other to meet patient and family/caregivers, assess patient care needs, acuity in relation to policies, procedures and regulations of home care and/or hospice services.

PRIMARY RESPONSIBILITIES<ul><li>
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.</li><li>
May perform point of care testing according to policies and procedures.</li><li>
Collaborates, consults and communicates with physician regarding the plan of care and changes to patient status. Provides physician with regular updates on progress and recommends revision of plan care, documents status and communication in adherence with regulations.</li><li>
Coordinates and communicates with multi-disciplinary team members and outside agencies as appropriate to achieve expected outcomes for patient care and education through end of life or discharge, ensuring accurate and complete documentation and coordination of unique care plan.</li><li>
Completes admission paperwork and verifies insurance information. Completes a comprehensive medical assessment by entering data into the clinical computer program. Ensures that patient assessment is completed in a timely manner.</li><li>
Provides skilled nursing care within the established plan of care, including performance of tasks such as taking vitals, performing wound care, IV care, ostomy care, blood draws, injections, and the physical assessment of the overall patient&rsquo;s medical condition.</li><li>
Documents visit notes, treatments, and care. Follows visit pattern according to physicians orders and ensures required visits are completed in a timely manner. Ensures all documentation is timely, accurate and complete in adherence to regulations.</li><li>
Collaborates with the patient, patient&rsquo;s family and healthcare team to develop a comprehensive plan of care based on the unique needs of the patient. Responds to calls from patients, family members and others, completes follow-up to address needs, issues, or concerns.</li><li>
Works in a constant state of alertness and safe manner.</li><li>

Position Summary
The Hospice and Palliative Care RN plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.
Admissions/visits/assignments made are directed and guided by the designated team member coordinating the shift. Hospice and Palliative Care RNs will perform consultation and admission visits to assess patient&rsquo;s needs, complete the admission assessment, contractual agreements, and all necessary documentation. In addition, they will conduct routine and emergency visits to Hospice and Palliative Care patients. Hospice and Palliative Care RNs will travel to the patient&rsquo;s place of residence to conduct the nursing visit.
Hours for this position are 10:30am to 7pm when scheduled Monday to Friday. Weekend and holiday hours when working in these rotations are 8am to 4:30pm.

Follows Hospice Alliance policies, standards of practice and operational guidelines.</li><li>
Participate in hospice and community health programs as requested to promote the growth and understanding of the hospice concept. This includes but is not limited to participation in the Hospice Alliance observation program that provides other health care professionals with the opportunity to observe and learn about hospice care, college partnerships, and other Hospice Alliance sponsored education events.</li><li>
Establish a CNA plan of care as well as indirectly and directly supervise the plan of care per regulations.</li><li>
Participation in Hospice Alliance town hall meetings, clinical team meetings, trainings, memorials, and other events.</li><li>
