Under the direction of the Director of Pharmacy, this position fills orders for parenteral and enteral medications, monitors patient drug therapies and provides drug information. Supervises and directs support personnel. Responsible for the procurement storage, dispensing, supervision and management of all legend and non-legend drugs for the hospital and shall maintain complete and accurate records of all drug transactions by the pharmacy.</p>
<p><b>
Job Responsibilities:</b></p>
<ul>
<li>Prepares and dispenses medication orders per physician request according to established policies, procedures and protocols: interprets medication orders (verbal and written and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles; compounds and dispenses pharmaceuticals accurately according to physician orders; issues controlled substances to patient care areas and maintains records as required by law.</li>
<li> Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols: monitors drug therapy regiments for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose; assists with pharmacokinetics consult service and drug dosing per organizational protocol; reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed; reads, extracts, and interprets information in patient charts accurately; provides drug information to patients and their families; documents all clinical activities and interventions accurately and completely; participates in quality improvement and medication use review activities of the department. Maintains logs, records, and other documentation as assigned; participates in the development and presentation of orientation, education and training programs to the pharmacy, medical, nursing, and other staffs.</li>
<li>Contributes to the quality and effective operation of the department. Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel; works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner; completes and documents inspections of all assigned medication storage areas at least monthly. Identifies and replaces out dated and unusable drugs; keeps pharmacy areas and equipment clean, neat, and well organized; sustains formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection.</li>
<li>Maintains competence required for current job title/position: maintains current pharmacist license; attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy; completes all competence/skills assessment requirements.</li>
<li>Interprets department policies and procedures to personnel, patients/family members, etc., as necessary; assists in cost containment development of the department within budgeted parameters.</li>
<li>Identifies potential risks to patients: identifies risks to patients in order to reduce incidence of injury; identifies actions to eliminate, minimize or report risks; identifies procedures to follow in the event of an incident; reports processes for common problems, failures and user errors.</li>
<li>Adheres to all Hospital Policies and Procedures, specifically Guidelines for Appropriate Conduct, Corporate Compliance, Diversity, and Confidentiality.</li>

Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes. Also responsible for providing patient care according to the laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies. May be responsible for prescribing medications under specific collaborative drug therapy management agreements (CDTM).<br>

<ul><li>Directs and coordinates the activities and delivery of pharmacy services.</li><li>Supervises and verifies the accuracy of all work completed by support personnel during an assigned shift.
</li><li>Operates within the limits of pharmacy policy and procedures.
</li><li>Ensures compliance with target drug programs, formulary management, medication management initiatives, and adverse drug event monitoring programs.
</li><li>Documents and reports clinical interventions, medication errors, adverse drug events.</li><li>Participates in department quality improvement efforts.

No Licenses and Credentials Pharmacist [State License] - Generic - HR Only required Massachusetts Controlled Substances Registration [MCSR - Massachusetts] - Massachusetts Department of Public Health preferred Experience Experience working as a pharmacist 2-3 years preferred Knowledge, Skills and Abilities - Extensive knowledge and ability in all areas of the department including: IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution. - Attention to detail. - Demonstrate appropriate knowledge of principles of growth and development over the life span of the neonate, adolescent, adult, and geriatric patient Possess ability to assess patient data relative to age specific needs Provide care as described in the department's policies/procedures. - Works relatively independently. - Participates in department quality improvement efforts. - Participates in the training of medical, nursing and allied health professionals. - Must complete 20 continuing education credits per year to maintain license. - Familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices.<p></p><p><b><br>
