LifeMoves | Haven Family House (HFF) provides interim supportive housing in Menlo Park to 23 families, 9 of which are veteran families. During their stay, clients participate in holistic services tailored to each household&rsquo;s unique needs and goals. Clients receive support to build self-sufficiency and return to housing stability, including but not limited to accommodations, meals, laundry, case management, and therapy. The team supports clients with housing, employment, benefits, physical and behavioral health through case management, workshops, and connection to specialized resources.
</div></div><div><div>The Haven Family House Interim Supportive Housing Case Manager reports to the site Program Director and provides case management, supports daily site operations, maintains a safe and welcoming environment, monitors the site, responds to client needs, and supports client check-ins, group activities, and overall site coordination. The position plays a key role in supporting people moving toward safe, stable housing and greater self-sufficiency. They provides case management services, life skills support, and resource connections and work in partnership with clients to set goals, celebrate progress, and overcome challenges. Using a trauma-informed and strengths-based approach, this position builds trust and respect, making sure clients feel heard, valued, and supported throughout their journey.</div></div><div><div>

Eagle Star Housing is a non-profit organization that has been helping Veterans and individuals experiencing housing instability become self-sufficient in Upstate New York since 2012. We pride ourselves on providing the most comfortable and supportive environments possible to ensure Veterans and other vulnerable individuals can successfully reach their individualized goals.</p><p>
Eagle Star Housing is looking for qualified candidates to provide supportive services to residents in a Permanent Supportive Housing program located at one site in Rochester, NY. These candidates should be passionate about access to affordable housing and working with those that have served in the United States Armed Forces and individuals with severe mental illness (SMI). The primary goal is to assist residents in achieving increased stability and self-sufficiency using a strengths-based, Housing First approach.</p><p><b>
Position Summary:</b></p><p>
Under the direction of the Permanent Supportive Housing Director, the candidate will assist in ensuring full capacity of permanent supportive housing units under the guidelines of the Empire State Supportive Housing Initiative (ESSHI) for those that have served in the United States Armed Forces and individuals with severe mental illness (SMI). Within these guidelines, and at the direction of the Housing Director, residents will require case management, individualized service plans, coordination with outside providers and weekly structured activities to include life skills. Additional functions performed include goal planning, connecting clients to community resources, home visits, and rental payment monitoring. It also includes assisting residents in housing searches if they no longer require the supportive housing program. Work is accomplished by advising and motivating program participants to accomplish their goals.</p><p>
Candidate will coordinate and work collaboratively with the Housing Director to identify individualized resident needs are met. Candidate will assist residents in getting settled into their apartment and will ensure regular check-ins with those tenants to ensure successful transition. Candidate will collaborate with community partners, facilitate group activities, and maintain notes on each resident in an electronic medical record. POSITION SUMMARY
<br>The primary role of the Residential Manager is to ensure that all of the needs of the people supported in the program are met while working in partnership with them. These needs include, but are not limited to ensuring their rights are both promoted and respected, supporting their health and safety and working with them to identify and realize their personal outcomes. This position is critical to the LEAD Project, by providing engagement and intensive case management services to individuals suspected of low-level drug offenses and/or prostitution. The Case Manager will provide direct services to an active case load of approximately 25-30 individuals. Case managers provide outreach, long-term engagement and supportive services for participants through intensive case management activities and collaboration with LEAD partners, service providers, housing providers and other community organizations.</p><p>
As a<b> Case Manager/Peer Support Specialist </b>much of your time will be spent in the field providing peer outreach and case management services to individuals suspected of low-level drug offenses in collaboration with LEAD partners, service providers, housing providers and other community-based organizations.</p><p><b>

Engage participants on the street and at social service provider facilities to establish a working relationship and offer services.</li><li>
