The Case Manager will provide case management and service coordination at a 105-unit Permanent Supportive Housing program. The Case Manager will be required to have frequent contact, not limited to, weekly or monthly face-to-face meetings, telephone conversations, e-mail and other electronic exchange with colleagues is expected.

Responsibilities: 
<ul><li>Maintain a case load of high risk/high intensity tenants.</li></ul>
Provide enhanced case management services which will be informed by a Housing First approach in the office and at the resident&rsquo;s home using evidence-based practices such as Motivational Interviewing and Harm Reduction techniques. 
<ul><li>Conduct assessments and program intakes all assigned residents.</li><li>Ensure data is accurate and complete and that all required accompanying documentation is collected.</li><li>Develop and update person centered service plans (every 6 month);</li><li>Utilize trauma informed interventions and culturally responsive engagement techniques to assist residents with overcoming barriers to housing stability and assist them in their community integration.</li><li>Identify each resident&rsquo;s strengths, needs, abilities, and preferences.</li><li>Act as liaison for the psychiatrist at the program site and manage the appointment times of residents receiving services from the doctor.</li><li>Provide referrals, advocacy, and linkages to public benefits including but not limited to healthcare, SNAP, SSl/SSDI, and public assistance.</li><li>Maintain and refer to a network of resources and contacts that will further their community integration.</li><li>Implement groups with a focus on life skills, education and support, relapse prevention and mental health counseling,</li><li>Coordinate with Workforce Opportunities department to link residents to employment and educational assistance;</li><li>Coordinate aftercare services and follow-up contacts for post-discharge updates.</li><li>Conduct outreach and engagement including apartment visits and inspections.</li><li>Participate in community development efforts such as Community Meetings and supporting the residence in establishing partnerships with other providers in the community.</li><li>Ensure compliance with organizational and funder regulations and documentation. Complete a monthly Audit Tool and ensure that all citations are cured.</li><li>Work with Property Management staff to assist resident to meet terms of their lease including timely payment of rent and avoidance of any nuisance behaviors.</li><li>Provide all services in a manner that respects cultural differences amongst residents.</li><li>Provide administrative support to the Assistant Director and Program Director as needed</li><li>Other duties as assigned by a supervisor.</li></ul></div>

Cocoon House&rsquo;s Snohomish Young Adult Housing (SYAH) program will assist program participants by offering five units of transitional housing as an immediate solution for young adults experiencing homelessness. While in transitional housing, program participants will receive &ldquo;Rapid Rehousing&rdquo; assistance, receiving support and financial assistance to find their own, independent housing. Throughout the program, program staff and participants will work together on short- and long-term employment and education goals.
The Rapid Rehousing Program Manager, internally referred to as the SYAH Program Manager will oversee all elements of the above programming: the transitional housing program, Rapid Rehousing services, and education and employment assistance. This position will consist of managing budget and staffing for the SYAH program, supporting case management staff and developing and refining transitional housing and rapid rehousing procedures. The SYAH Program Manager will also work to ensure that the SYAH program works in concert with other Cocoon House programs, providing a continuum of services that contribute to Cocoon House&rsquo;s mission of breaking the cycle of homelessness.

Alongside a team of other case managers, housing locators and community providers, the Case Manager MSW provides field-based, direct client outreach, linkage and retention services to people experiencing homelessness. Case management services aim to promote recovery and community integration and ultimately help clients break the cycle of homelessness. Services provided by the Case Manager include, but are not limited to, individual and group therapy, outreach and engagement, case assessment and service planning, assistance with mainstream entitlement, PHA subsidy and other county subsidy applications, housing location, advocacy and addressing health, mental health and substance use barriers. The Case Manager delivers these services by implementing evidence-based practices such as ACT (Assertive Community Treatment) and Motivational Interviewing to help clients meet their service plan goals. The Case Manager provides individualized support to Deaf, DeafBlind, Hard of Hearing, and DeafDisabled adults through the Access to Services (ATS) Program and Medicare Assistance Program (MAP), focusing on case management, advocacy, and long-term service retention.
This role ensures that clients have sustained access to healthcare, Medicare benefits, housing, employment, and financial assistance by addressing barriers and fostering self-advocacy skills.
This position delivers one-on-one assistance, education, and advocacy to beneficiaries navigating Medicare benefits, ensuring they understand their coverage options, rights, and responsibilities while supporting them in making informed healthcare decisions. The Case Manager also provides comprehensive support for accessing employment services, social programs, and community resources.
Reporting to the CSD Works NV Program Manager, the Case Manager works collaboratively with Program Coordinators, call center representatives, service navigators, community agencies, and service providers to deliver culturally responsive, person-centered care. The position requires strong communication, problem-solving, and organizational skills, as well as the ability to navigate complex service systems with empathy and cultural humility.

Case Management &amp; Client Support<ul><li>
Conduct client intakes and needs assessments to identify goals, barriers, Medicare eligibility, coverage gaps, and healthcare access needs.</li><li>
Provide individualized case management through regular check-ins and follow-up support.</li><li>
Deliver tailored Medicare counseling, including enrollment support, plan comparison, and coverage optimization.</li><li>
Advocate for client eligibility and participation in healthcare, housing, financial assistance, and social service programs.</li><li>
Assist clients in resolving issues with Medicare benefits, claims, billing, denials, or appeals.</li><li>

THE OPPORTUNITY
Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court and community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in so doing, uses an arrest as a window of opportunity to change the direction of a participant&rsquo;s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling.
SIJC seeks a Housing Specialist for the borough&rsquo;s Supervised Release Program (SRP). Reporting to the Community Engagement Coordinator, the Housing Specialist will leverage a deep and comprehensive knowledge of housing opportunities and challenges in the NYC area to provide in-house assistance to participants experiencing housing instability. Responsibilities will include assisting participants in obtaining both short-term and permanent housing opportunities. The Housing Specialist will work closely with the Clinical Director to build and maintain relationships with and have knowledge of local housing providers to complete housing referrals and help ensure case management staff are knowledgeable about housing providers and opportunities by facilitating in-house information sessions.
Responsibilities include but are not limited to:<ul><li>
Work with participants and their case managers to understand participants&rsquo; housing needs, including any relevant factors such as family size, safety needs, mental health, conviction history, and other special circumstances that may impact housing opportunities and needs;</li><li>
Assist participants with the completion of housing applications including supportive housing, housing subsidies or vouchers, rental assistance, and other similar programs and opportunities;</li><li>
Assist participants in accessing emergency housing at shelter locations and drop-in centers;</li><li>
Assist participants in obtaining identification and vital documents needed to obtain housing;</li><li>
Work with the Clinical Director to develop and maintain relationships with and knowledge of local providers of housing to understand housing-specific services, eligibility, intake requirements, and opportunities and challenges towards the goal of optimizing participants&rsquo; access to housing;</li><li>
Provide referrals and warm handoffs to service providers that are outside the scope of the Housing Specialists&rsquo; work, such as legal services providers;</li><li>
Explore new and additional housing service providers to assess quality and accessibility of services offered;</li><li>
Develop and facilitate presentations on housing service providers to the direct practice teams to share information and guidance to optimize successful, quality referrals;</li><li>
Work collaboratively with the case management team to effectively maintain accurate and timely data entry and case documentation aligned with the program model, court reporting obligations, and best practices;<ul><li>
Consistently case conference with participants&rsquo; Case Managers;</li><li>
Ensure that meetings with participants also fulfill the obligations of check-ins and are recorded in the program database;</li></ul></li><li>
Responsible for consistent and effective cross-team communication and collaboration to proactively identify successes, challenges, and opportunities to strengthen programming, create opportunities for meaningful collaboration, maximize internal and external resources and expertise, and substantive and comprehensive services to participants;</li><li>

Participate in the Housing Justice Working Group, including monthly meetings, retreats, and other gatherings;</li><li>

Qualifications:<ul><li>

Comprehensive expertise related to housing opportunities and challenges in NYC required;</li><li>
Comprehensive knowledge of and/or experience with the criminal legal, child welfare, and/or mental health systems preferred;</li><li>
Must be detail-oriented with strong organizational and time management skills;</li><li>
Must be able to work effectively and independently and as part of a highly interdependent, multi-disciplinary team within a fast-paced and dynamic work environment;</li><li>
Ability to work collaboratively and effectively with a variety of program stakeholders, including judges, prosecutors, defense attorneys, court staff, and site partner agencies; and</li><li>

Compensation: The compensation range for this position is $54,000 - $65,400 and is commensurate with experience.
