This position must be filled by an individual who has a passion for social justice work, is a team player, and is committed to working alongside people as they transition out of homelessness into permanent and stable housing. This position could include a mixed caseload of clients from two programs, Shelter Plus Care and the DC Department of Human Services Permanent Supportive Housing Program.Principal Duties:<ul><li>Provide supportive services and develop trusting relationships with households being served by Everyone Home DC&rsquo;s Permanent Supportive Housing and Shelter Plus Care Program.</li><li>Provide face-to-face engagements to clients both in their homes and in the community.</li><li>Case Managers are expected to meet or exceed monthly contact requirements for each of their clients.</li><li>Engage clients to collaboratively develop goals and individualized service plans to support their housing stability, mental health, substance abuse, health, education, income, employment, and other priority areas.</li><li>Assist clients who desire to relocate with housing navigation services, including helping with obtaining identifying documents, completing housing applications, and identifying and viewing housing units.</li><li>Support clients to maintain housing stability through individually tailored services and by facilitating effective connections to community services and resources.</li><li>Examples of support services include successful tenancy problem-solving, independent living skill-building, connection to public beneﬁts and employment assistance, money management support, and connection to medical, mental health, and substance use disorder services.</li><li>Act as a liaison with landlords and other community stakeholders to promote the success of clients and the program.</li><li>Collaborate with other service providers in the community to ensure that clients are receiving high-quality, effective services to meet their needs.</li><li>Maintain timely and accurate written and computerized records, compile reports, and complete other program documentation (e.g. case notes, incident reports, home visit reports, monthly and quarterly reports) and maintain appropriate conﬁdentiality of residents&rsquo; information and records.</li><li>Maintain client data in the Homeless Management Information System (HMIS), Housing the Homeless (HTH), and Insync and report regularly on client progress and goal attainment.</li><li>Participate in program meetings and case review meetings</li><li>Carries out all duties consistent with grant requirements and internal policies and procedures.</li><li>Will complete SSI/SSDI, Outreach, Access and Recovery(SOAR) training within the first 90 days of onboarding.</li></ul>Secondary Duties:<ul><li>Participate in all individual and group supervision meetings as scheduled.</li><li>Attend all program and all-staff meetings as well as Everyone Home DC events.</li><li>Participate in and attend ongoing training and professional development opportunities.</li><li>Work on special projects and committees as needed.</li><li>All other relevant duties as assigned.</li></ul>POSITION SPECIFICATIONS:Essential Training/Certifications:<ul><li>Minimum of an Associate&rsquo;s degree or two years of college in a relevant humanities, social science, or related field.</li><li>Bachelor's Degree preferred.</li><li>Minimum of two years of experience with homeless services programs or provision of social services. Essential Duties and Responsibilities: The Case Manager plays a crucial role in the Rapid Re-housing program, dedicated to assisting individuals experiencing homelessness in Northern Santa Barbara County. Rapid Re-housing aims to provide temporary financial assistance and support to facilitate a return to permanent housing. As a Case Manager, the responsibilities include delivering comprehensive case management, housing navigation, and housing retention services to eligible clients qualified for rapid re-housing subsidies. This multifaceted role encompasses outreach and engagement, conducting assessments and service planning, assisting with mainstream entitlement applications, linking clients to employment services, locating suitable housing options, addressing stabilization needs, and fostering clients' recovery and integration into the community. Located in East New York, Brooklyn, Phase 1A includes 376 income based rental residences, with 94 units dedicated to families who have experienced homelessness due to domestic violence and who qualify for ESSHI services.  The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. POSITION SUMMARY: The B.R.I.D.G.E To Home (B2H) Program serves individuals returning from Illinois prisons who face significant and often compounded barriers to successful reentry. Frequently excluded from traditional housing and support systems, these individuals are at increased risk of homelessness and recidivism. The program&rsquo;s primary goal is to provide permanent supportive housing for individuals traditionally considered hard to place, creating a foundation for long-term stability and successful reintegration into the community. Guided by low-barrier, housing placement principles, the B2H Program eliminates preconditions such as abstinence or mandatory treatment participation, offering immediate access to housing.
ESSENTIAL DUTIES AND RESPONSIBILITIES
<ul><li>Collaborate with the Network Housing Developer to identify safe, affordable housing options appropriate for reentry clients, including those with complex barriers or limited income history.</li>
<li>Provide specialized case management services, including interviews and intake assessments, housing and rental eligibility screenings, and the development of individualized housing stabilization plans that reflect clients' reentry needs, goals, and risks.</li>

Across 24 shelters in New York City, URI provides safe housing for nearly 4,000 individuals and their pets every night and serves more than 40,000 people annually.

URI&rsquo;s work is grounded in survivor-centered, trauma-informed, and culturally responsive care. In addition to emergency and transitional housing, URI delivers a robust continuum of Justice Initiatives&mdash;including economic empowerment, legal education and advocacy, prevention and youth programming, and abusive partner intervention&mdash;designed to break cycles of violence, instability, and poverty. URI also develops permanent affordable and supportive housing, advancing long-term pathways to safety, healing, and stability for families and communities.

Innovative Urban Village, opening in Summer 2026, is URI&rsquo;s first supportive housing program, developed in partnership with Christian Cultural Center. Located in East New York, Brooklyn, Phase 1A includes 376 income based rental residences, with 94 units dedicated to families who have experienced homelessness due to domestic violence and who qualify for ESSHI services. As URI&rsquo;s inaugural supportive housing initiative, Innovative Urban Village requires the development of a comprehensive program model, policies, procedures, and service framework from inception through full implementation.

POSITION OVERVIEW

The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence.

This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices. Case Managers collaborate closely with Housing Specialists, the Economic Empowerment Specialist, and the Director of Clinical Services to ensure integrated, high quality service delivery aligned with ESSHI and OTDA standards.

MAJOR DUTIES AND RESPONSIBILITIES

Direct Service &amp; Case Management
