OVERVIEW OF PRIMARY RESPONSIBILITES: 
The Supported Housing Case Management program provides housing for chronically homeless single adults and families who are eligible for DSS benefits, SSI or SSD, with Behavioral health and Chemical dependency issues. It also provides housing for individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes as well as housing for individuals who are and have been residents of either OMH Psychiatric Centers or OMH and OCFS operated residential programs for one year or longer.
 
PRIMARY FUNCTIONS: 
<ul>
<li>Engage the recipients and create a shared Housing Health and Recovery Plan that will reflect the principles of person-centered, strength-based recovery, with the ultimate goal of self-sufficiency.</li>
<li>Provide outreach and assistance securing apartments in the affordable housing complex.</li>
<li>Provide services that will touch all of the life domain areas including employment, finance, permanent relationships, medical needs, health, legal concerns, family, parenting, education, transportation, safety, culture, spirituality, and social.</li>
<li>Partner with recipients as they integrate into the community of their choice and support them to make informed choices of the available supports, services, and recreational opportunities.</li>
<li>Assist with a plan for discharge when the recipient is ready to transition out of Supported Housing. Anticipate supports and services needed to highly support the transition and ensure implementation of all linkages to facilitate success.</li>
</ul>
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT: 
<ul>
<li>Serve as a model, guide to other staff on the SHCM team.</li>
<li>Respond and Intervene in crisis situations as needed</li>
<li>Complete required documentation of services provided and assist in orientation of other staff as needed</li>
<li>Comply with regulations set by employer, as well as NYS OMH</li>
<li>Work closely with community providers on facilitating a smooth transition for all referrals into the Supported Housing Program.</li>
<li>To work a flexible schedule evenings and weekends as needed to ensure the needs of all recipients are met.</li>
<li>Off-hours on-call and emergency situation response</li>

QUALIFICATIONS AND ATTRIBUTES:

WHAT YOU WILL DO:

Provides direct case management services to individuals and families who are homeless or at risk of becoming homeless, focusing on helping them achieve housing stability. Uses Family Development and Housing First models of case management that is client driven, solution focused and facilitates change and movement toward self-sufficiency. Specific duties may include, but are not limited to home visits, intakes, the tracking of services, and assisting in program development. 


WORK SCHEDULE:

Monday through Friday: 9 hour shifts Mon-Thu, 4 hours on Friday. 


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Provides homeless housing related case management services using the Family Development Model to families participating in homeless housing programs.</li><li>
Conducts intakes and assessments.</li><li>
Assists in program development work and advocacy and education to community partners.</li><li>
Maintains confidential case notes on each individual or family client in a timely manner.</li><li>
Develops and maintains cooperative working relationship with other agencies and service providers.</li><li>
Serves as link between client and resources.</li><li>
Provides reporting and tracking information for data management systems.</li><li>
Determines client eligibility for homeless housing services.</li><li>
Ensures that all federal, state, agency and/or funder guidelines are met.</li></ul> 


OUR IDEAL CANDIDATE WILL HAVE:
<ul><li>
Minimum four-year degree and six months relevant experience; or year for year relevant experience in place of degree.</li><li>
Case management experience in social service employment setting, and a working knowledge of issues and resources in relation to housing services for low-income families preferred.</li><li>
Experience working with persons living with mental illness, experiencing chronic homelessness and/or experiencing Domestic Violence and its long term affects</li><li>
Knowledge of the disease of Chemical Dependency and its signs and symptoms.</li><li>
Experience with crisis prevention and intervention and crisis management; including resolution and recovery.</li><li>
Valid Washington State driver&rsquo;s license and five-year driving history within Opportunity</li><li>

FCS Housing &amp; Employment Case Manager / Sober Living House ManagerPosition SummaryThe FCS Housing &amp; Employment Case Manager / Sober Living House Manager is responsible for supporting individuals with behavioral health, substance use, housing instability, and employment barriers as they work toward long-term recovery, self-sufficiency, and community integration.This dual-role position provides intensive Housing and Employment Services under Washington State's Foundational Community Supports (FCS) program while also overseeing the daily operations, resident accountability, safety, and recovery-focused environment of one or more sober living homes.The Case Manager serves as an advocate, mentor, coach, and recovery support professional who helps individuals obtain and maintain housing, secure competitive employment, build independent living skills, and successfully sustain long-term recovery in a structured, supportive living environment.The ideal candidate is compassionate, highly organized, recovery-oriented, and capable of maintaining professional boundaries while supporting residents through challenging life transitions.Essential Duties and ResponsibilitiesFCS Housing Services<ul><li>Conduct comprehensive assessments of housing barriers, strengths, needs, and goals.</li><li>Develop and maintain individualized Housing Support Plans.</li><li>Assist participants in locating, obtaining, and maintaining safe and stable housing.</li><li>Support participants with housing applications, lease agreements, landlord communication, and housing retention strategies.</li><li>Coordinate housing resources, rental assistance programs, and community-based services.</li><li>Assist participants with independent living skills, budgeting, transportation, and community integration.</li><li>Conduct regular home visits and community-based meetings.</li><li>Maintain compliance with all FCS documentation, billing, and service requirements.</li></ul>FCS Supported Employment Services<ul><li>Conduct employment assessments to identify vocational interests, skills, strengths, and barriers.</li><li>Develop individualized Employment Plans and measurable employment goals.</li><li>Assist participants with job searches, resume development, interview preparation, and application completion.</li><li>Provide job coaching, employment retention support, and workplace problem-solving assistance.</li><li>Build relationships with local employers and community partners.</li><li>Assist participants in obtaining vocational training, certifications, and educational opportunities.</li><li>Support participants in maintaining competitive employment and career advancement.</li></ul>Sober Living House Management<ul><li>Oversee daily operations of assigned recovery residences or sober living homes.</li><li>Ensure residents understand and comply with house rules, expectations, curfews, and recovery standards.</li><li>Conduct resident orientations and intake meetings.</li><li>Facilitate resident accountability, conflict resolution, and positive community living.</li><li>Coordinate resident move-ins, room assignments, and discharge planning.</li><li>Monitor resident progress toward recovery, housing, employment, and personal goals.</li><li>Collaborate with house operators, clinical providers, treatment teams, probation officers, and community partners.</li><li>Ensure homes remain safe, supportive, clean, and recovery-focused environments.</li><li>Coordinate drug screenings, breathalyzer testing, room inspections, and other accountability measures as required by program standards.</li><li>Address resident concerns, disputes, and behavioral issues promptly and professionally.</li></ul>Recovery Support &amp; Advocacy<ul><li>Provide recovery-oriented coaching and encouragement.</li><li>Assist residents in developing healthy coping skills, relapse prevention strategies, and recovery support networks.</li><li>Connect residents to outpatient treatment, behavioral health services, medication management, peer support, and recovery meetings.</li><li>Support participants in navigating legal obligations, probation requirements, court mandates, and community supervision expectations.</li><li>Encourage personal responsibility, accountability, and independent decision-making.</li></ul>Crisis Intervention<ul><li>Respond to resident crises, conflicts, relapse events, and behavioral health concerns.</li><li>Utilize de-escalation techniques and trauma-informed approaches.</li><li>Coordinate emergency services and crisis response resources when necessary.</li><li>Document incidents accurately and timely.</li><li>Facilitate safe transitions, transfers, or discharges when appropriate.</li></ul>Documentation &amp; Compliance<ul><li>Maintain accurate, timely, and professional case records.</li><li>Complete all required FCS documentation, assessments, service plans, progress notes, and billing requirements.</li><li>Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, and company policies.</li><li>Participate in audits, quality assurance reviews, and compliance activities.</li><li>Submit reports, documentation, and productivity requirements within established timelines.</li></ul>QualificationsEducationPreferred:<ul><li>Bachelor's Degree in Social Work, Psychology, Human Services, Behavioral Health, Counseling, Sociology, Criminal Justice, or a related field.</li></ul>Equivalent education and experience may be considered.ExperiencePreferred:<ul><li>Two (2) years of experience in behavioral health, substance use treatment, housing services, employment services, case management, peer support, recovery residences, or community-based social services.</li><li>Experience working with individuals experiencing substance use disorders, mental health challenges, homelessness, justice system involvement, or employment barriers.</li></ul>Lived ExperienceIndividuals with lived experience in addiction recovery are strongly encouraged to apply.Preferred:<ul><li>Minimum of five (5) years of continuous recovery.</li></ul>Preferred Certifications<ul><li>Certified Peer Counselor (CPC)</li><li>Certified Peer Recovery Specialist (CPRS)</li><li>Certified Chemical Dependency Professional (CDP)</li><li>Substance Use Disorder Professional (SUDP)</li><li>Other behavioral health or recovery-support certifications</li></ul>Knowledge, Skills &amp;
