In this role, you will:
<ul>
<li>Conduct comprehensive initial screening process to determine eligibility for program services, providing assessment, intake and enrollment where appropriate.</li>
<li>Manage a caseload of specific clients including developing Service Goal Plan, budgeting, and monitoring of client&rsquo;s compliance with program rules.</li>
<li>Conduct and document client meetings to monitor progress.</li>
<li>Conduct phone follow-up after completion of the program.</li>

<li>Process re-certifications as per funder&rsquo;s requirements.</li>
<li>Outreach to landlords for mediation, negotiation, and tenant relations.</li>
<li>Monitor direct assistance that can be used for financial support, rent, utility, and security deposit payments.</li>
<li>Compile and prepare appropriate tenant/landlord contracts and landlord check request payments to ensure timely delivery.</li>
<li>Participate in regular supervision with supervisor by providing regular updates and preparing case presentations.</li>
<li>Maintain client files in accordance with program requirements and NVFS guidelines.</li>
<li>Maintain current level of knowledge on program software and other information on housing assistance programs.</li>
<li>Enter data for record keeping and reporting to meet relevant guidelines.</li>
<li>Comply with the Standards and Ethics for Housing Counseling</li>
<li>Responsible for maintaining certification CEUs if certified.</li>

Your Background: We understand that you may obtain skills through a variety of professional, personal, educational, and volunteer experiences.

<li>Prefer one of the following:</li>

Veteran Housing and Benefits Specialist

Summary: Under direct supervision of the Program Manager provide housing placement, support and case management services for eligible homeless veteran families through outreach, assessment and engagement of potential members. Assist veterans and their families in finding and accessing requested community linkages and resources through the application of intensive case management services based on housing first and psychosocial rehabilitation model of treatment and services.

Street to Home, Brooklyn
 
Breaking Ground’s Street to Home program makes regular contact with more than 2,000 street homeless individuals and connects more than 500 individuals to housing, medical and mental health services, substance abuse counseling, and other essential supports each year. The caring, individually tailored attention clients receive at each stage of their journey from street to home ensures that more than 95% of people who Breaking Ground places remain stably housed.
 
The Housing Advocate works with street homeless conducting street outreach with the objective of engaging individuals to determine their eligibility for Street to Home services and to help secure appropriate housing solutions for all people who are street homeless. If chronic, the Housing Advocate will build a relationship with clients utilizing motivational interviewing to encourage the client to work towards permanent housing. If not chronic, Housing Advocate will utilize available resources to secure other housing placements. The Housing Advocate will always work with a partner and will spend most of the shift in an automobile conducting street outreach. The incumbent will respond to 311 calls and community concerns; participate in Code Blue and Code Red alerts; identify new hotspots/encampments and work to eradicate existing hot spots/encampments; conduct a bi-annual vulnerability index on all clients in their catchment area; and enter all required data into outreach database(s).
 This position has a 35-hour work week.

 program. MLK BHC offers residential Drug Medi-Cal (DMC) services for a maximum of 99 adult individuals,
 comprising 33 beds each for judicially involved men, men, and women. MLK BHC collaborates closely with
 other Behavioral Health Center programs to ensure a comprehensive range of services for low-income
 and Medi-Cal-eligible individuals. Situated on the Martin Luther King Jr. Hospital campus in the
 Willowbrook area of South Los Angeles, the program serves residents of all Los Angeles County.
 
The SUD Counselor III is part of an interdisciplinary team that supports our clients through assessment of
 their needs and strengths in support of their health and recovery. This in-person role involves providing
 counseling in both individual and group settings, delivering curricula in accordance with program-specific
 requirements, and offering case management. The SUD Counselor III also assists clients in navigating
 systems of care while fostering a supportive treatment environment.

Responsibilities/Duties: 
•
 Counsel with emphasis on prevention and/or behavior modification with individuals under supervision in the criminal justice system on probation or parole.
 
•
 Act as an advocate to coordinate identified services addressing employment, housing, basic adult education, cognitive restructuring, mental health, life skills, and substance use.
 
•
 Act as an advocate to resolve crises
 
•
 Collect information about individuals under supervision through assessments, interviews, observations, and random drug screening.
 
•
 Counsel individuals under supervision in individual and group sessions to help develop coping skills and recover from dependencies.
 
•
 Develop and implement treatment plans
 
•
 Maintain confidentiality of records relating to the client's treatment
 
•
 Monitor, evaluate, and record individual progress according to measurable goals described in the treatment plan.
 
•
 Prepare, document, and maintain all case notes, treatment records, and reports.
 
•
 Facilitate substance use and/or cognitive programming, based on the needs of each circuit. Non-traditional hours may be required to ensure program delivery. Travel may also be required for training, court, as well as to facilitate groups in other assigned locations.
 
Minimum Qualifications: Bachelor's degree in a related field from an accredited college or university OR certification as an addiction counselor from the Georgia Addiction Counselors Association (GACA) or equivalent certification.
 
Preferred Qualifications: 
Bachelor's degree in a related field from an accredited college or university. One year of experience providing individual and/or group counseling services to individuals under supervision in the criminal justice system on probation or parole identified to have substance abuse and/or cognitive needs; Credentialing (i.e., Licensed Professional Counselor (LPC), Certified Substance Abuse Counselor (CADC/CAC), Licensed Clinical Social Worker (LCSW), or comparable credential).

 Part-Time Non-Credentialed Counselor: $28.37/hour

 Part-Time Credentialed Counselor: $31.37/hour
