WHAT YOU WILL DO:

Provides direct case management services to individuals and families who are homeless or at risk of becoming homeless, focusing on helping them achieve housing stability. Uses Family Development and Housing First models of case management that is client driven, solution focused and facilitates change and movement toward self-sufficiency. Specific duties may include, but are not limited to home visits, intakes, the tracking of services, and assisting in program development. 


WORK SCHEDULE:

Monday through Friday: 9 hour shifts Mon-Thu, 4 hours on Friday. 


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Provides homeless housing related case management services using the Family Development Model to families participating in homeless housing programs.</li><li>
Conducts intakes and assessments.</li><li>
Assists in program development work and advocacy and education to community partners.</li><li>
Maintains confidential case notes on each individual or family client in a timely manner.</li><li>
Develops and maintains cooperative working relationship with other agencies and service providers.</li><li>
Serves as link between client and resources.</li><li>
Provides reporting and tracking information for data management systems.</li><li>
Determines client eligibility for homeless housing services.</li><li>
Ensures that all federal, state, agency and/or funder guidelines are met.</li></ul> 


OUR IDEAL CANDIDATE WILL HAVE:
<ul><li>
Minimum four-year degree and six months relevant experience; or year for year relevant experience in place of degree.</li><li>
Case management experience in social service employment setting, and a working knowledge of issues and resources in relation to housing services for low-income families preferred.</li><li>
Experience working with persons living with mental illness, experiencing chronic homelessness and/or experiencing Domestic Violence and its long term affects</li><li>
Knowledge of the disease of Chemical Dependency and its signs and symptoms.</li><li>
Experience with crisis prevention and intervention and crisis management; including resolution and recovery.</li><li>
Valid Washington State driver&rsquo;s license and five-year driving history within Opportunity</li><li>
If hiring for a bilingual position. Must be able to speak, read, and write with proficiency and fluency in the required language.</li></ul>

OVERVIEW OF PRIMARY RESPONSIBILITES: 
The Supported Housing Case Management program provides housing for chronically homeless single adults and families who are eligible for DSS benefits, SSI or SSD, with Behavioral health and Chemical dependency issues. It also provides housing for individuals who reside in OMH psychiatric centers, Article 28 inpatient hospitals, residential treatment programs and adult homes as well as housing for individuals who are and have been residents of either OMH Psychiatric Centers or OMH and OCFS operated residential programs for one year or longer.
 
PRIMARY FUNCTIONS: 
<ul>
<li>Engage the recipients and create a shared Housing Health and Recovery Plan that will reflect the principles of person-centered, strength-based recovery, with the ultimate goal of self-sufficiency.</li>
<li>Provide outreach and assistance securing apartments in the affordable housing complex.</li>
<li>Provide services that will touch all of the life domain areas including employment, finance, permanent relationships, medical needs, health, legal concerns, family, parenting, education, transportation, safety, culture, spirituality, and social.</li>
<li>Partner with recipients as they integrate into the community of their choice and support them to make informed choices of the available supports, services, and recreational opportunities.</li>
<li>Assist with a plan for discharge when the recipient is ready to transition out of Supported Housing. Anticipate supports and services needed to highly support the transition and ensure implementation of all linkages to facilitate success.</li>
</ul>
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:

<li>Serve as a model, guide to other staff on the SHCM team.</li>

Position Description:
Under the direction and supervision of the Portfolio Operations Manager, the District Manager (DM) is responsible for the overall day-to-day operations of their community and team as well as providing leadership to additional community teams. The DM mentors and provides their teams with a clear understanding of TCB's standard operating procedures, objectives, mission and each property&rsquo;s economic and physical requirements. The DM will ensure that their teams are equipped to perform in achieving occupancy, operating and capital expense budgets, and customer service objectives. The DM is accountable for mentoring, developing and directing their site teams to ensure consistent adherence to company standard operating procedures, provide career progression and optimal performance.
Essential Functions:
<ul><li>Seek to build employee teams and provide training, motivation and direction for superior performance, supervision and performance management of all property management staff.</li>
<li>Identify, attract, hire, and retain top talent in the industry. Be visible and participatory in local agency and industry organizations and events to build network and brand identity for TCB.</li>

<ul><li>Bachelor&rsquo;s degree or 4 years or more of progressive responsibility in residential property management, hospitality or retail industry required.</li>
<li>3+ years of multi-family experience or a comparable role in hotel/resort/retail management required.</li>
<li>Prior experience in and knowledge of affordable housing and applicable compliance required.</li>
<li>Extensive experience working with District of Columbia affordable housing programs and regulatory agencies is strongly preferred, including the District of Columbia Housing Authority (DCHA), Department of Housing and Community Development (DHCD), and Department of Buildings (DOB).</li>
<li>Demonstrated knowledge of District of Columbia affordable housing regulations, compliance requirements, inspections, enforcement actions, and agency reporting requirements.</li>
<li>Proven experience identifying, addressing, and resolving regulatory infractions, agency findings, building violations, and compliance deficiencies while maintaining positive relationships with regulatory partners.</li>
<li>Strong understanding of affordable housing operations, including Housing Choice Voucher (HCV), Project-Based Voucher (PBV), RAD, LIHTC, and other federal, state, and local housing programs.</li>
<li>Experience managing multiple sites within a portfolio while balancing operational performance, compliance obligations, financial goals, resident satisfaction, and team development.</li>
<li>Ability to effectively interact with government agencies, investors, legal counsel, residents, and community stakeholders to achieve property and portfolio objectives.</li>
<li>Industry certification such as Accredited Residential Manager or C3P required.</li>
<li>Proficiency in Yardi or other industry software and excellence in Microsoft Office.</li>
<li>Real Estate License or other licensing if required by state law.</li>
