What you'll do</div><div>
The Health Coordinator supports Pioneer Human Services&rsquo; mission by coordinating and monitoring client physical and behavioral health services within residential treatment facilities. This role assists in ensuring continuity of care by facilitating communication between clients, nursing staff (where present), medical and behavioral health providers, pharmacy services, and program staff.</div><div></div><div> 
The Health Coordinator supports the intake process, coordinates internal and external medical and behavioral health appointments (including telehealth), assists with medication monitoring and documentation accuracy in the electronic medication administration record (eMAR/MAR), and gathers and communicates relevant health information to appropriate clinical staff. At sites with licensed nurses, the Health Coordinator works under the direction of nursing staff to support care coordination and implementation of provider orders. At sites without nurses, the Health Coordinator works under the direction of Health &amp; Resource Coordination (HRCU) Manager.</div><div></div><div> 
In addition to care coordination, the Health Coordinator provides direct client support within a structured residential environment, including; engaging clients in the milieu, observing and monitoring client changes in physical and in behavioral health status, reinforcing treatment plans and health education, assisting with access to healthcare services, and responding to urgent health concerns by notifying appropriate clinical staff or emergency services as needed.</div><div></div><div> 
This role requires strong organization, attention to detail, and the ability to effectively support clients with complex medical, behavioral health, and substance use needs in a structured residential environment.</div><div></div><div> 
Primary/Essential duties and responsibilities are but not limited to:</div><div> 
</div><ul><li>Utilize program-approved screening tools and established procedures to identify and document client risks, needs, and barriers while supporting client and facility safety.</li><li>
Utilize Motivational Interviewing (MI) and other engagement strategies to support client participation and skill development.</li><li>
Interpret and explain program policies and goals to clients during orientation and throughout treatment as needed.</li><li>
Support client education by conducting health education activities and reinforcing self-care skills appropriate to the client&rsquo;s stage of recovery.</li><li>
Where applicable by site or unit, observe and document client self-administration of medications in accordance with program policies and procedures.</li><li>
Where medication responsibilities are assigned to nursing staff, promptly communicate medication-related concerns, discrepancies, or client requests to the licensed nurse.</li><li>
When applicable to the role at a specific site, review MAR documentation for completeness at shift change and escalate discrepancies according to policy.</li><li>
Maintain a safe and healthy environment which includes maintaining, cleaning, and disposing of equipment and supplies according to policy and procedures (e.g., cleaning medication rooms, disinfecting vital signs equipment, maintaining health services spaces).</li><li>
Where applicable, assist with medication storage, ordering, and returns in accordance with pharmacy services policies and site procedures.</li><li>
Assist clients in meeting health and safety needs in accordance with each individual&rsquo;s stage of recovery and program policies.</li><li>
Provide basic first aid and serve as the first point of contact for emergency response when needed, notifying licensed nursing staff, providers, or emergency services as appropriate.</li><li>
Follow provider orders and support implementation of care plans in accordance with program policy and the scope of the Health Coordinator role.</li><li>
Gather and relay client-reported health information, including health and medication history, vital signs, symptoms, diagnoses, and other relevant concerns to nursing staff or providers as appropriate.</li><li>
At sites with licensed nurses, assist Registered Nurses (RN) or Licensed Practical Nurses (LPN) with intake processes or coordination activities as directed.</li></ul><ul><li>
Support coordination of client physical and behavioral healthcare services during assigned shifts, including telehealth visits, clinic appointments, and outside provider services.</li><li>
Coordinate and document client intake and discharge processes after hours, including financial documentation and secured belongings as required.</li><li>
Coordinate internal and external services and referrals based on provider recommendations or treatment planning needs.</li><li>
Support service navigation by assisting clients in accessing health services and communicating needs to clinical staff.</li></ul><ul><li>
Maintain accurate, timely, and complete documentation of services in the electronic health record.</li><li>
Ensure required notifications are completed during assigned shifts.</li><li>
Complete eligibility assessments and follow-up assessments in designated systems.</li><li>
Ensure client information is accurate and updated from intake through discharge to support continuity of care.</li><li>
Record routine case notes and service contacts documenting client progress and services provided.</li><li>

JOB SUMMARY:
The Guest Services Coordinator at Family Motel is directly responsible for the management of the residential facility (during their shift). This program serves multiple populations in different programs: family shelter, respite shelter and a Grant Per Diem program within the facility. Service modalities include Trauma Informed Care, Harm Reduction, Motivational Interviewing, Crises De-escalation and Critical Time Intervention. Job duties include supporting appropriate housing stability; ensuring client needs are met, maintaining the safety and wellbeing of all residents, maintaining the cleanliness of the facility, coordinating activities, maintaining client files and program data entry/ maintenance. Further, the Guest Services Coordinators may develop specialty areas including housing, employment, legal services, benefits, or others as indicated by the needs of the program.
JOB DUTIES:<ul><li>
Ensures that the guest&rsquo;s voice is heard, and when appropriate the guest&rsquo;s ideas play a prominent role in programming.</li><li>
Responsible for possessing an in-depth knowledge regarding program requirements, including the referral process, support services, and field-relevant best practices.</li><li>
Demonstrates knowledge of guest-specific resources, a passion for serving the population and people experiencing homelessness, and the ability to contribute to the program-wide objectives of a diverse and dynamic team.</li><li>

** The Shelter Coordinator position provides &ldquo;rotating 30 minutes of break time coverage&rdquo; to the Shelter program
staff in the Denver Metro Area Shelters. This is a rotating position where the Shelter Coordinator will travel to
assigned sites to relieve staff for their 30-minute break. The Shelter Coordinator position is responsible for
managing the facility (in the time they are covering). The programs under Shelter purview serve multiple
populations in different programs: a Safe Haven for chronically homeless women with mental health concerns, a
family shelter, a Grant Per Diem program for Veteran Families and unaccompanied individuals, and a shelter for
older women. Service modalities for all programs under RYES include Housing First Trauma Informed Care, Harm
Reduction, Motivational Interviewing, and Crisis De-escalation. Job duties include supporting and coordinating with
other staff in the delivery of appropriate housing stability and support services for individuals and families
experiencing homelessness, including ensuring participants needs are met, maintaining the safety and wellbeing of
program participants, and maintaining the cleanliness of the facility. Further, the Shelter Coordinator will have a
basic understanding of mental health services and may have the opportunity to develop in specialty areas
including housing, employment, legal services, benefits, or others as indicated by the needs of the program(s).

Position Type and Expected Hours of Work<ul><li>This is a permanent - part time position</li><li>While Shelter Support Coordinators are on duty, they are likely single-staffed and unless they have established coverage for an approved reason, they will be expected to stay at the facility for the entirety of their shift.</li><li>All Shelter staff are considered &ldquo;essential staff,&rdquo; requiring to be on their scheduled shifts regardless of holidays or inclement weather. <div>The Harris Center for Mental Health and IDD (&ldquo;The HARRIS CENTER&rdquo;) is looking for a Service Coordinator to join our team. This person is responsible for providing assistance to individuals with intellectual disabilities in gaining access to services and supports utilizing a person directed planning process to achieve their desired outcomes. Services and supports may include waiver programs, medical, psychiatric, residential, financial, and a wide array of social services in the community. Provides service monitoring through contacts with consumers, Legally Authorized Representatives, program providers and collaterals. The ideal candidate will display compassion, education, proficiency and experience while assisting the patient. This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.
