The salary range for this role is:
$21.62 - $30.47

The selected candidate provides tracking, release planning, and follow up services to pre-trial, sentenced, and civilly committed DMH eligible clients incarcerated in county, state, federal, and out-of-state correctional facilities. The Forensic Transition Team Coordinator meets with DMH clients in correctional settings and provides linkage to community-based services. The incumbent also gathers statistical information and tracks all DMH incarcerated clients via data collection systems.
Applicants will need access to reliable transportation for considerable work-related travel from the Area Offices to correctional facilities and places of detention, not only in the assigned areas but also relevant facilities and locations throughout the Commonwealth as required.

** The Shelter Coordinator position provides &ldquo;rotating 30 minutes of break time coverage&rdquo; to the Shelter program
staff in the Denver Metro Area Shelters. This is a rotating position where the Shelter Coordinator will travel to
assigned sites to relieve staff for their 30-minute break. The Shelter Coordinator position is responsible for
managing the facility (in the time they are covering). The programs under Shelter purview serve multiple
populations in different programs: a Safe Haven for chronically homeless women with mental health concerns, a
family shelter, a Grant Per Diem program for Veteran Families and unaccompanied individuals, and a shelter for
older women. Service modalities for all programs under RYES include Housing First Trauma Informed Care, Harm
Reduction, Motivational Interviewing, and Crisis De-escalation. Job duties include supporting and coordinating with
other staff in the delivery of appropriate housing stability and support services for individuals and families
experiencing homelessness, including ensuring participants needs are met, maintaining the safety and wellbeing of
program participants, and maintaining the cleanliness of the facility. Further, the Shelter Coordinator will have a
basic understanding of mental health services and may have the opportunity to develop in specialty areas
including housing, employment, legal services, benefits, or others as indicated by the needs of the program(s).
Essential Duties and Responsibilities<ul><li>
Ensures that the program participants&rsquo; voices are heard, and when appropriate the participants ideas play a prominent role in programming.</li><li>
Responsible for possessing an in-depth knowledge regarding program requirements, including the referral process, support services, and field-relevant best practices.</li><li>
Demonstrates knowledge of participant-specific resources, a passion for serving the population and people experiencing homelessness, and the ability to contribute to the program-wide objectives of a diverse and dynamic team.</li><li>
Clearly documents all participant interactions along with required eligibility and demographic information. Can develop specialized knowledge related to service delivery database systems within the organization including Service Point and Homeless Management Information System (HMIS).</li><li>
Performs professional duties by maintaining confidentiality of all information and participating effectively within and across teams.</li><li>
Maintains physical and emotional safety of all program participants in the facility. Manages the milieu and addresses any program participant conflicts immediately to de-escalate. Working Conditions and Physical Requirements<ul><li>Ability to be single staffed while in the facility while having the responsibility for the safety of program participants.</li><li>Ability to quickly and safely go up and down stairs regularly throughout the shift to do perimeter checks, and in cases of an emergency.</li><li>Some lifting is required but should rarely exceed 50lbs.</li></ul>
Position Type and Expected Hours of Work<ul><li>This is a permanent - part time position</li><li>While Shelter Support Coordinators are on duty, they are likely single-staffed and unless they have established coverage for an approved reason, they will be expected to stay at the facility for the entirety of their shift.</li><li>All Shelter staff are considered &ldquo;essential staff,&rdquo; requiring to be on their scheduled shifts regardless of holidays or inclement weather. This position is an advanced practitioner of peer recovery support services in a Permanent Supportive Housing program model, offering both Housing First and Sober Living options. Summary
 
The Family Service Coordinator is responsible for monitoring ERSEA (Eligibility, Recruitment, Selection, Enrollment, Attendance) practices and coordinating family services across sites including the promotion of family wellness, building community partnerships and operating as a liaison between programs, classrooms and home settings in the areas of family engagement, child development and mental health services.
 
Essential Functions: 
<ul>
<li>Perform the functions of this position with a strong understanding of the Head Start Program Performance Standards in addition to local, state and other federal guidelines;</li>
<li>Supervise and provide support to family service staff at sites;</li>
<li>Monitor compliance using data management systems including PROMIS, WES, Pre-Kids;</li>
<li>Monitor the development and progress of Family Engagement Goal Setting;</li>
<li>Develop system of creating and submitting reports to management team that provide information related to ERSEA, family goal setting, services provided and any other information that may be requested;</li>
<li>Assist with coordination and facilitation of parent and staff workshops;</li>
<li>Advocate and collaborate with community agencies to support child, family and agency outcomes</li>
<li>Coordinate and facilitate Health Advisory Committee meetings in accordance with Head Start guidelines</li>

Minimum Qualifications:

<li>Bachelor's Degree in Human Services or a related field plus 5 years' experience working in Human Services, Psychology or related field that includes course work in principles of social work, child development, counseling and/or psychology;</li>
