One (1) year of full-time, paid experience performing social services delivery activities such as assessing client needs, determining appropriate actions and establishing case plans, monitoring client progress, maintaining case records, or making referrals to other resources, agencies, or services.</p><div><b>
AND</b></div>
A Master&rsquo;s Degree or higher from an accredited college or university in social work, behavioral science, sociology, psychology, social psychology, counseling, education counseling, gerontology, marriage and family therapy, nursing, vocational rehabilitation, criminal justice, public administration or other field closely related to the intent of the class.<p><b><br>

General Qualifications</b><br>

Criminal History and Background Checks: The County may access criminal history information on candidates who have accepted a conditional offer of appointment for this class consistent with the provisions of Board of Supervisors Resolution No. 82-602, Personnel Policies, and applicable federal and state laws. The County shall not consider for employment any candidate who has been convicted of a felony or misdemeanor that relates to or impacts the candidate&rsquo;s ability to perform the job duties of this class unless it is determined that mitigating circumstances exist. For purposes of accessing criminal history information, the candidate will be fingerprinted. A subsequent arrest notification may be obtained.<br>

The County may also conduct a background check on the candidate prior to appointment to a position within this class. The background check may include personal and professional reference checks, credit history checks where authorized by Labor Code section 1024.5, Social Security Number verification, professional license/registration verification, military service information and driving history. Information obtained in the course of this background check will be considered by the appointing authority in the selection process. <li>Manage and supervise case management team, including performance evaluations, scheduling, orientation, and training as well as making recommendations on decisions for employee hires, transfers, promotions, salary changes, discipline, terminations, and similar actions. Resolve employee problems within position responsibilities.</li>
<li>Determine caseloads and assign cases to teams; review treatment plans to ensure completeness and accuracy; track all necessary data related to case management performance targets and timeliness of reports; maintain internal and external file compliance.</li>
<li>Interview individuals served to obtain medical history data and other pertinent information; conduct intake and administer diagnostic tests to identify behavioral and personality traits; document results.</li>
<li>Assist in the development and implementation of the Individual Service Plans (ISP) including annual, ongoing meetings.</li>
<li>Consult with a treatment team, including a medical doctor and other specialists, in decisions related to changes in a treatment plan, amend the plan as directed, and arrange additional support as necessary.</li>
<li>Plan, arrange, and/or administer therapeutic treatment such as behavior modification, social skills, anger management, and stress management therapy based on a professional license or clinical competency.</li>
<li>Refer individuals to supportive services to supplement treatment and counseling as required.</li>
<li>Provide on-call coverage for caseload and participate in rotational emergency coverage and crisis intervention support, as needed.</li>
<li>Conduct supervisory home visits and counsel individuals and groups as needed.</li>
<li>Ensure that initial and ongoing RBWO waivers are completed and submitted promptly, and that SCORE is updated and accurate at all times if required by state.</li>
