The primary role of the Home Infusion Nurse is to provide clinical care and education to home-based and AIS patients receiving a wide range of parenteral medications dispensed from Optum Infusion Pharmacy (the &quot;Company&quot;). The most common route of administration is intravenous, however medications may also be given via subcutaneous infusion or injection, intramuscular injection, or intraspinal (intrathecal and epidural) infusion. Service delivery takes place in patient homes, alternate care settings and in ambulatory infusion suites (AIS), where available. All infusion services and medication delivery is provided in accordance with Optum Infusion Pharmacy policies, procedures and therapy-specific protocols, and based on comprehensive orders received from a physician or licensed practitioner. The Home Infusion Nurse fills an integral role in the clinical operations of the organization, working collaboratively with the multi-disciplinary team needed to prepare, deliver and seek reimbursement for the infusion therapy that is safely and effectively administered to each patient. 
Preferred location is along the AL/MS state line; near the Tuscaloosa area; preferably on the MS side of the state line. If candidate resides in MS, will need to have multistate license in place already. If in Alabama, needs to be willing to obtain multistate license within first 90 days of employment. 
Primary Responsibilities:<ul><li>
Establish and maintain positive clinical rapport with patients and their families, working closely with them in the provision of infusion therapy, education related to self-care and self-administration, and clinical monitoring of patient response to therapy</li><li>
Assess patient appropriateness for home infusion therapy, including safety of the home environment, availability of caregiver support, and willingness of patient and/or caregivers to become as independent in therapy administration and monitoring of response to therapy as possible</li><li>
Obtain and document patient's medical history at the start of care and ongoing, particularly as related to the diagnosis being treated with the prescribed infusion therapy</li><li>
Complete all required clinical documentation thoroughly and timely, including nursing plans of care (POC), plans of treatment (POT)/485's, supplemental physician orders, visit notes, clinical summaries, medication profiles, discharge summaries and any updates provided to members of the patient's care team, as well as obtaining patient and/or caregiver signatures on relevant consents and required forms</li><li>
Participate in case conferences as needed to comprehensively plan patient care with the multi-disciplinary care team</li><li>
Provide comprehensive skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders and the plan of treatment</li><li>
Establish, secure and maintain vascular access devices as prescribed, including (but not limited to) peripheral IV (PIV) placement, PICC and tunneled central venous catheter dressing changes, and implanted port access</li><li>

We specialize in disease management and infusion pharmacy services for chronic and acutely ill patients, especially those in underserved communities. We offer personalized home and in-suite infusion therapies, ensuring convenience and comfort. Our expert team provides a comprehensive range of treatments, including anti-infectives, biologics, IVIG, and other injectables, making each patient&rsquo;s journey as smooth as possible.
Key Responsibilities:<ul><li>
Advocate for patients, addressing their concerns and collaborating with the healthcare team to enhance the overall patient experience.</li><li>
Ensure timely and effective communication between different departments and healthcare professionals.</li><li>
Educate patients and their families about infusion therapy procedures, potential side effects, and the importance of adherence to treatment plans.</li><li>
Maintain accurate and up-to-date patient records, including documentation of assessments, interventions, and outcomes.</li><li>
Research and obtain additional data, consulting with referring offices, Nurse Practitioners, Nurse Managers, and Medical Directors when necessary.</li><li>
Adhere to established clinical protocols and guidelines to ensure the safety and effectiveness of infusion therapy.</li><li>
Serve as a resource to non-clinical staff within the Patient Services Department.</li><li>
Ensure work is completed in a timely manner to prevent delays in patient care.</li><li>
Attend departmental meetings and in-services.</li><li>
Assist with orientation and training of staff.</li><li>
Demonstrate courteous, compassionate, and professional behavior at all times.</li><li>
Perform all other assigned duties.</li></ul> 
Required Qualifications:<ul><li>
Active RN license with 1&ndash;3 years of hands-on clinical experience.</li><li>
Valid nursing license with all necessary credentials and certifications in good standing.</li><li>
Previous experience in IV therapy or infusion specialty is highly preferred.</li><li>
Strong commitment to delivering exceptional patient care with compassion and professionalism.</li><li>
Great communication and interpersonal skills, with the ability to effectively interact with patients, families, and healthcare team members.</li><li>
Ability to thrive both in a collaborative team environment and as a self-reliant individual contributor.</li><li>

 We are seeking a dedicated Nurse Practitioner to join our Rheumatology Practice and Infusion Centers, providing high-acuity outpatient care for patients receiving IV therapies for autoimmune or chronic conditions.
 
 
 <ul>
 <li>
 Conduct comprehensive assessments, order labs, manage infusion reactions, and supervise administration of IV medications.
 </li>
 <li>
 Educate patients and families on treatment plans and self-care.
 </li>
 <li>
 Supervise nursing staff, ensure safety and compliance with regulations, and maintain documentation in EHR systems.
 </li>
 <li>
 Collaborate with physicians and coordinate care, with some travel to satellite infusion sites.

 Access and maintain vascular access devices as needed.
