JOB SUMMARY:
Provides safe and appropriate pharmacy services to patients in the alternate care setting in accordance with policies and procedures, professional standards, and applicable regulatory and accrediting entities. Coordinates patient services. Assesses appropriateness of therapy, dispenses medications, supplies and medical equipment, documents patient progress toward goals, intervenes as clinically appropriate and provides education regarding appropriate medication administration and use. Additional dimensions may include working with operating budget, revenue responsibility, purchasing responsibility, etc.
Ensures company&rsquo;s goals and objectives are aligned with the organization&rsquo;s broader mission, vision, and driving strategies. Directly accountable and reports to the Pharmacy Manager or Director of Pharmacy or Operations based on branch leadership. 
MAJOR DUTIES AND RESPONSIBILITIES:
1. Demonstrates ethical pharmacy practice and ensures compliance with all company policies and procedures, state and federal pharmacy rules and regulations, professional practice standards, and accrediting entities.
2. Assesses patient's suitability for home care and appropriateness of prescribed therapy.
3. Ensures the patient database is complete and compiles the necessary information needed to design a patient-specific pharmacy plan of care with goals for therapy.
4. Recommends and writes patient-appropriate orders.
5. Assures documents that require the prescriber's or pharmacist's signature for billing are generated and obtained.
6. Assesses laboratory results and performs pharmacokinetic dosing.
7. Makes appropriate dosing recommendations and documents.
8. Initiates, creates and updates patient medication profiles.
9. Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.
10. Dispenses medications and related supplies to cost-effectively meet patient needs and assures authorization for services is obtained prior to dispensing.
11. Accurately dispenses, programs, if applicable, and troubleshoots medication administration pumps.
12. Accurately submits electronic claims for medications, as applicable.
13. Documents all communication with physicians, nurses, dietitians, patients and caregivers in the patient's medical record.
14. Documents initial patient or caregiver discussion, education and counseling regarding prescribed therapy, medication use and information, infusion method, and after hours contact information.
15. Thoroughly and objectively documents the patient's course of therapy, compliance with therapy, laboratory results, and progress towards goals of therapy with therapy-appropriate frequency.
16. Ensures all compounding, clean room and anteroom activities adhere to all state and federal regulations, USP &lt;797&gt; standards and company policies and procedures by all pharmacy staff.
17. Ensures required documentation is completed on QA/QC records and compounding documents.
18. Completes company-required orientation and annual competencies and in services.
19. Provides concise, applicable and timely responses to requests for drug information.
20. Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates
21. Obtains and processes patient clinical information to allow for processing refills and supplies at least 24 hours in advance of patient's need.
22. Plans work to ensure cost-effective delivery methods are utilized.
23. Oversees the work of pharmacy support personnel and ensures their job duties are completed in a timely manner.
24. Delegates non-clinical tasks to assigned support personnel.
25. Prepares for and actively participates in patient care conferences including Nutrition Support Rounds.
26. Participates in the on-call process and documents on call activities in the patient medical record, if appropriate, and communicates with relevant personnel.
27. Assists in the training and supervision of pharmacy support personnel, pharmacy interns and pharmacy-profession students.
28. May assume role in of the Pharmacist in Charge or general branch manager during temporary absences and/or vacancies as appropriate.
29. Performs other related duties as directed by supervisor.

Determines the suitability of individual patients for home care. Collects and organizes all patient-specific information needed to determine the suitability of patients for home care. Assesses patients' suitability for home care in accordance with Option Care policies. Medical condition and prescribed medication therapy suitable for home care services and prognosis with clearly defined outcome goals.</li><li>
Designs, recommends, monitors and evaluates patient-specific pharmacy care plans for the pharmacotherapy of home care patients. When appropriate, makes suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters for a home care patient. Collects and organizes patient-specific information needed to prevent, detect, and resolve medication-related problems and to make appropriate medication therapy recommendations. Appropriate monitoring parameters are identified for each patient. Completes nutritional assessment of patients and makes appropriate recommendation.</li><li>
Modifies the pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient-specific information. Competent in use of pharmacokinetic principles and formulas. Competent in assessing patient laboratory values.</li><li>
Provides medication-use education to Option Care staff, other professionals, home care patients and/or their caregivers. Uses effective patient education techniques to provide counseling to home care patients and/or their caregivers.</li><li>
Ensures continuity of pharmaceutical care to and from the home and other patient-care settings. Uses a systematic procedure to communicate pertinent patient information to and from the home and other patient-care settings. Prepares and dispenses medications using appropriate techniques and following Option Care policies and procedures. Accurately calibrates equipment. Prepares medications so they are appropriately concentrated, without incompatibilities, stable, and appropriately stored. Adheres to appropriate safety and quality assurance practices. Prepares labels that conform to the organization's policies and procedures. Medication contains all necessary and/or appropriate ancillary labels. Inspects the final medication before dispensing. Assures presence of authorization to dispense. Complies with state/federal laws and USP standards, including Drug Enforcement Administration (DEA) controlled substance regulations set forth by the state and DEA. Complies with policies regarding the perpetual inventory systems for schedule II narcotics, controlled substances and other medications subject to diversion.</li><li>
Manages the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment used in the management of home care patients. Accurately programs, maintains and gives advice to others on the use and maintenance of all medication administration and medication-related equipment used by Option Care. Participates in the process for assessing, managing, and reporting medication-related incidents and equipment error and/or failures. Accurately completes QAR documents according to the Option Care policy. Monitors and reports discrepancies in perpetual inventory systems for schedule II narcotics and other medications subject to diversion.</li><li>
Provides concise, applicable, and timely response to requests for drug information from health care providers and home care patients. Formulates responses to drug information requests based on analysis of the literature. Attends educational seminars or participates in other CEU opportunities in order to promote professional competency and complies with state mandated CEU requirements.</li><li>
Manages the use of investigational drug products according to established regulations and Option Care policies and procedures. Utilizes pharmacy support personnel effectively. Prioritizes the workload and organizes the workflow, taking into account available resources. Needed work is accomplished in the time available. <div>Department Overview:

This position provides technical support to clinic personnel and offsite pharmacists who are responsible for the
safe and proper distribution and use of medications by patients in outpatient infusion clinics that are not staffed with a licensed pharmacist. Patients may range in age from neonates, infants &lt;1 year old, children and adolescents ages 1-18, adults ages 19-65, and the elderly age &gt;65. The technician is responsible for
conducting the day-to-day operations of the outpatient infusion preparation area. Responsibilities include:
sterile product and hazardous admixture/preparation, automated inventory maintenance of controlled and non- controlled medications, narcotic discrepancy reconciliation, pharmacy computer system order entry, clinic
audits, supply ordering, medication delivery, regulatory compliance recordkeeping, utilization of pharmacy-
related technology and performing other duties as assigned. 
Relief employees must be available to work at least four (4) days per pay period as determined according to the department needs
Function/Duties of Position:

Positions in this function require various nurse licensure and certification based on role and grade level. Licensure includes RN or LPN/LVN, depending on grade level, with current unrestricted licensure in applicable state. Positions this function include RN and LPN/LVN roles responsible for overseeing and participating in day to day clinical care of patients. Employees are involved in direct patient care (in a clinic, in-home care, etc.). 
The primary role of the Home Infusion Nurse is to provide clinical care and education to home-based and AIS patients receiving a wide range of parenteral medications dispensed from Optum Infusion Pharmacy (the &quot;Company&quot;). The most common route of administration is intravenous, however medications may also be given via subcutaneous infusion or injection, intramuscular injection, or intraspinal (intrathecal and epidural) infusion. Service delivery takes place in patient homes, alternate care settings and in ambulatory infusion suites (AIS), where available. All infusion services and medication delivery is provided in accordance with Optum Infusion Pharmacy policies, procedures and therapy-specific protocols, and based on comprehensive orders received from a physician or licensed practitioner. The Home Infusion Nurse fills an integral role in the clinical operations of the organization, working collaboratively with the multi-disciplinary team needed to prepare, deliver and seek reimbursement for the infusion therapy that is safely and effectively administered to each patient. 
Preferred location is along the AL/MS state line; near the Tuscaloosa area; preferably on the MS side of the state line. If candidate resides in MS, will need to have multistate license in place already. If in Alabama, needs to be willing to obtain multistate license within first 90 days of employment. 
Primary Responsibilities:<ul><li>
Establish and maintain positive clinical rapport with patients and their families, working closely with them in the provision of infusion therapy, education related to self-care and self-administration, and clinical monitoring of patient response to therapy</li><li>
Assess patient appropriateness for home infusion therapy, including safety of the home environment, availability of caregiver support, and willingness of patient and/or caregivers to become as independent in therapy administration and monitoring of response to therapy as possible</li><li>
Obtain and document patient's medical history at the start of care and ongoing, particularly as related to the diagnosis being treated with the prescribed infusion therapy</li><li>
Complete all required clinical documentation thoroughly and timely, including nursing plans of care (POC), plans of treatment (POT)/485's, supplemental physician orders, visit notes, clinical summaries, medication profiles, discharge summaries and any updates provided to members of the patient's care team, as well as obtaining patient and/or caregiver signatures on relevant consents and required forms</li><li>
Participate in case conferences as needed to comprehensively plan patient care with the multi-disciplinary care team</li><li>
Provide comprehensive skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders and the plan of treatment</li><li>
Establish, secure and maintain vascular access devices as prescribed, including (but not limited to) peripheral IV (PIV) placement, PICC and tunneled central venous catheter dressing changes, and implanted port access</li><li>
Administer medications as prescribed, performing clinical assessments in accordance with the Company's policies, procedures and protocols, which may include frequent vital sign monitoring, management of acute infusion reactions, monitoring of side effects, and procurement of laboratory specimens as well as reporting of patient status as it relates to other concomitant medical conditions</li><li>
Communicate observations of patient response to therapy and overall physical assessment/condition with members of the care team in a timely manner, which may include the nursing supervisor, dispensing pharmacist, registered dietitian, and licensed prescriber, among others</li><li>
Establish collaborative relationships with home health agencies (HHAs) delivering services on behalf of/or in conjunction with the Company, providing agency education and support as requested by the nursing supervisor</li><li>
Maintain a comprehensive knowledge base of infusion therapies administered in the home and ambulatory infusion center, utilizing Company policies, procedures and protocols in addition to all available training resources to ensure understanding of each therapy before initiating treatment</li><li>
Maintain current nursing licensure in all states in which patients are serviced, as required by the respective State Boards of Nursing and/or Department of Health</li><li>
Comply with all State and Federal regulations and laws, including employee health and safety protections mandated by the Occupational Safety and Health Administration (OSHA), patient privacy and data security required by the Health Insurance Portability and Accountability Act (HIPAA), as well as all home infusion standards of practice</li><li>
Complete all training as required upon hire and annually per policy, including cardiopulmonary resuscitation (CPR) certification</li><li>
Serve as a professional representative of the Company when meeting with and/or providing education to external customers, payers and referral sources</li><li>
Assist in staff orientation and training, serving as a clinical educator and resource as requested</li><li>
Drive personal/rental vehicle (per travel policy) in the completion of job duties, including (but not limited to) travel to the Company office, patient homes, area hospitals, physician offices, home health agency offices, and the Company's Ambulatory Infusion Suite (AIS)</li><li>
