JOB SUMMARY: The Highland Rivers Behavioral Health Intensive Case Management (ICM) Team is a medium intensity Community-Based program for adults with severe and persistent mental illnesses such as Bi-Polar Disorders, Schizoaffective Disorders, and other Sub-Categories of thought disorders.

The Case Manager works with the individual and other supports and service providers to develop an Individual Recovery Plan (IRP) that includes the medical, behavioral, social, educational, vocational, housing, financial and other needs of the eligible individual. Services help individuals develop strategies and support to avoid out-of- home placement and reduce the need for more intensive services, and coordinate access to other needed community services and resources. 


Duties and Responsibilities include:

Provide transportation for the individuals.</li><li>
Must visit individual's homes.</li><li>
Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the individual&rsquo;s Individual Recovery Plan (IRP).</li><li>
Partner with the individual and CORE services provider in the development of the IRP.</li><li>
Provide individual interventions to include identifying service needs; developing strategies and supportive interventions to avoid out of home placement or the need for more intensive services; assisting individuals to increase social support networks to ameliorate life stresses resulting from the individual's condition; and coordinating rehabilitation services as specified in the individual's IRP Partner with the individual and CORE service provider in the development of the individual recovery plan.</li><li>
Link and serve as advocate with other agencies to garner needed resources for individuals.</li><li>
Provides assistance and support in crisis situations.</li><li>
Provides assessment, monitoring, and assistance to individuals.</li><li>
Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community.</li><li>
Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and individual charts are closed according to policy.</li><li>
Sees a caseload at the frequency defined in the service plan; documents attempts to contact individuals and ISP reviews; completes monthly caseload reports; track appointments and case management.</li><li>
Meet weekly contact and productivity standards</li><li>
Attend treatment team meetings as directed by Program Supervisor.</li><li>
Assist with service-related compliance.</li><li>
Maintain confidentiality for all indirect/direct service in accordance with agency policies and HIPPA policies.</li><li>
Other responsibilities and tasks as may be assigned.</li></ul>

ESSENTIAL DUTIES AND RESPONSIBILITIES
<ul><li>Responsible to assist assigned individuals to obtain needed mental health services as well as adequate food, housing, medical care, employment, education and recreation.</li>

<li>Responsible to assist each individual in identifying activities, which would increase his marketable skills or enhance his vocational or educational status.</li>
<li>Responsible to maintain face-to-face and telephone contact with assigned consumers and collaterals, preferable weekly intervals between consumer contacts must not exceed two weeks.</li>
<li>Responsible to spend 70% of working hours in contact with assigned persons and collaterals, including travel time.</li>
<li>Responsible to spend at least 50% of working hours outside of the office.

Case Manager &ndash; Virtual Intensive Outpatient Program (IOP)
Position Summary
The Virtual IOP Case Manager provides care management services to adults with substance use disorders, serious emotional disturbances, and severe and disabling mental illnesses. Under the supervision of the Program Supervisor, the Case Manager partners with consumers to assess needs, develop individualized treatment plans, coordinate care across service systems, and support recovery-focused goals. This role requires meeting caseload and billing expectations on a monthly, quarterly, and annual basis.
Essential Job Functions<ul><li>
Assess and evaluate adults to determine appropriateness and need for care management services</li><li>
Assist consumers with entry into services and monitor progress throughout the continuum of care</li><li>
Develop, coordinate, and monitor Individual Treatment Plans (ITPs) in alignment with agency policy and procedure</li><li>
Ensure treatment plans are completed within 5 encounters or 21 days, whichever is sooner, and that clinical assessments are updated annually</li><li>
Coordinate treatment team meetings and ensure treatment plans are outcome-based and recovery-focused</li><li>

Virtual Service Delivery
Services are delivered primarily via telehealth through the agency's virtual IOP platform. All telehealth documentation must reflect clinical appropriateness for remote delivery. Case Managers are expected to maintain flexible scheduling to accommodate consumer availability, including evening hours as needed.
Community Based Psychiatric Rehabilitation Services (CBPRS)
Case Managers may also provide CBPRS, documented separately with an individual superbill. CBPRS consists of intensive behavioral management and stabilization services designed to reduce disability, restore functioning, and support social integration. Specific expectations include:<ul><li>
Skills training that includes clear description, modeling, practice, and performance feedback</li><li>
Consultation with family members or key support persons as clinically indicated</li><li>
Individual CBPRS up to 2 hours and group CBPRS up to 2 hours per 24-hour period; exceptions require department approval</li><li>
Group CBPRS limited to 8 adults per staff member</li><li>
Face-to-face delivery is preferred; telehealth is permitted when clinically indicated or when access barriers exist, with documentation of rationale and attempts to identify local supports</li></ul>
Attendance and Scheduling
This position requires flexible scheduling to meet consumer needs. Schedule changes will be communicated in writing. Attendance at staff meetings and required trainings is expected and may occasionally require schedule adjustments.

Annual Salary: $49,079.50

Qualifications/Requirements:
<ul>
<li>Bachelor's degree is required.</li>
<li>Experience working with at-risk youth and families</li>
<li>Proficient in Microsoft Office Suite. Experience using an electronic health record system is a plus.</li>
<li>Strong communication and interpersonal skills</li>
<li>Position requires reliable transportation, valid driver's license, and auto insurance coverage</li>
<li>Bilingual (Spanish Speaking) is a plus.</li>
