ESSENTIAL JOB DUTIES/RESPONSIBILITIES:<ul><li>
The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.</li><li>
The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.</li><li>
It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).</li><li>
The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.</li><li>
The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.</li><li>
For patients that are unable to come to the office&mdash;in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.</li><li>
PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.</li><li>
Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.</li><li>

As the hospitalist team, facilitate the transition of care from the ED to inpatient cardiology services. Responsible for providing comprehensive and evidence-based healthcare for patients, including, but not limited to, problem identification, inquiry of medical history, identification of psychosocial concerns and determinants of health, relevant physical exam, medical assessment, and appropriate management/treatment of acute and/or chronic medical conditions, as well as recommended screening tests/health maintenance ensuring optimal health, decreasing complication, and expediting recovery. 
 
Essential Functions 
 
1. Teaching &amp; Educational Leadership 
<ul><li>
Supervises Internal Medicine residents in the clinic, providing experiential, real-time teaching in primary care, population health, value-based, and equity-focused practice.</li><li>
Teaches residents to integrate social determinants of health, and community-engaged approaches into clinical decision-making.</li><li>
Participates in structured educational activities including journal club, lecture series, case conferences, and resident workshops.</li><li>
Mentors residents in quality improvement, health-equity research, and scholarly projects.</li><li>
Collaborates with program leadership to develop and refine curriculum aligned with ACGME competencies and Parkland priorities.</li><li>
Provides formative and summative feedback to residents, supporting their professional growth.</li></ul> 

2. Direct Patient Care in Value-Based Care Clinic
 
<ul><li>Provides comprehensive Internal Medicine care using evidence-based guidelines and value-based care principles.</li><li>
Helps identify and establish value-based standard practices for the clinic</li><li>
Uses population-health tools, risk stratification, and care-coordination strategies to improve outcomes and reduce disparities.</li><li>
Identifies and addresses non-medical drivers of health impacting patient care.</li><li>
Works collaboratively with care managers, pharmacists, social workers, behavioral health, and community partners to deliver coordinated, team-based care.</li><li>
Partners in a positive and collegial manner with rest of GME related personnel both at Parkland and UT Southwestern</li></ul>
