The Role:
This position is critical to the LEAD Project, by providing engagement and intensive case management services to individuals suspected of low-level drug offenses and/or prostitution. The Case Manager will provide direct services to an active case load of approximately 25-30 individuals. Case managers provide outreach, long-term engagement and supportive services for participants through intensive case management activities and collaboration with LEAD partners, service providers, housing providers and other community organizations.
As a Case Manager/Peer Support Specialist much of your time will be spent in the field providing peer outreach and case management services to individuals suspected of low-level drug offenses in collaboration with LEAD partners, service providers, housing providers and other community-based organizations.
Job responsibilities include:<ul><li>
Provide Outreach and Intensive Case Management services for assigned clients.</li><li>
Engage participants on the street and at social service provider facilities to establish a working relationship and offer services.</li><li>
Assess participants for the severity of chemical dependency and housing status and determine needs for other services, e.g., medical, mental health.</li><li>
Assist participants in gaining access to a variety of funding programs (e.g., SSI, ABD, VA) &bull;</li><li>
Assist participants in finding housing and maintaining occupancy.</li><li>
Develop and maintain collaborative relationships with LEAD partners.</li><li>
Provide advocacy and support for participants within the criminal justice system including court appearances and written communication.</li><li>
Advocate for the participant with a wide variety of other service providers.</li></ul>
Additional responsibilities may include:<ul><li>
Representing Ideal Option and the LEAD Project at community meetings, attending events, conducting outreach, and building awareness.</li><li>
Develop relationships with existing and potential community partners, referring providers, potential patients, and other stakeholders with direct outreach, field marketing, attendance at community events, meetings, workshops.</li><li>
Work at various partner sites throughout the area each week to raise awareness for Ideal Option&rsquo;s services and guide staff members and patients through virtual intake and provider visits.</li><li>
Provide education and training to community-based organizations and providers about LEAD, Ideal Option, medication-assisted treatment, and substance use disorder to broaden acceptance of addiction as a chronic disease and continue to reduce stigma and misconceptions about the disease.</li></ul>

<div>Position Summary:
The Case Manager for Law Enforcement Assisted Diversion (LEAD) responds to referrals from police and community members within Chelan &amp; Douglas Counties to provide intensive case management for people whose unlawful behavior is driven by unmet needs related to substance use, mental illness, or extreme poverty. The LEAD model is based on principles of harm-reduction and non-displacement, utilizing intensive case management, peer outreach and counseling, a trauma informed care perspective, specially tailored interventions, and cultural competency to support the goals of diverting participants from the criminal justice system, reducing recidivism, and improving quality of life. LEAD case management replaces clinic-based care with community-based, low-barrier, participant-driven outreach, engagement, and care. Case Managers help participants identify their goals and interests, work to understand fears and barriers, and use motivational interviewing to support behavioral shifts that help participants secure available services and resources to advance their progress and well-being. 
Responsibilities:<ul><li>
Receive referrals from officers and dispatch immediately to referral locations within Chelan &amp; Douglas Counties to engage with the referred participant and complete an initial screening
</li><li>Engage participants in a street level outreach at their homes and in the community to develop an ongoing working relationship to offer and engage in services</li><li>
Assess participants for immediate and basic needs such as housing support, substance use treatment, physical health, mental health, health benefits/insurance, food and clothing, etc.</li><li>
Collaborate with participants to develop an individualized service plan that is specific and tailored to the individual&rsquo;s basic needs, healthcare, behavioral health treatment/services, safety, and self-change goals</li><li>
Help participants accomplish goals set in their individualized service plan such as accessing services, connecting to substance use treatment or harm reduction services, navigating local resources, completing applications and forms, accompanying to appointments, and developing skills for independent living
</li><li>Respond to and follow-up on social contact and community referrals in a timely manner and according to processes determined in collaboration with the Policy Coordinating Group (PCG)</li><li>
Participate in the on-call rotation to respond to referrals from law enforcement
</li><li>Provide transportation to participants as needed using an agency-owned vehicle</li><li>
Advocate for participants with other providers and within the criminal legal system at court appearances, consistent with the LEAD model and core principles</li><li>
Develop and maintain strong, collaborative relationships with LEAD partners including prosecutors, law enforcement, substance use and mental health providers, Chelan County Regional Jail, housing providers, and other community stakeholders
</li><li>Attend and participate in Operational Work Group (OWG) meetings, staff meetings, multi-disciplinary team meetings, individual or group supervision, and other meetings as required.</li><li>

For each case, the Case Manager shall: create and maintain the computerized case file; prepare criminal Information charging documents; determine the nature and extent of each defendant's prior criminal history; determine the appropriate sentencing guidelines ranges, enhancements and mandatory provisions to be considered when imposing sentence; prepare the sentencing guidelines form; track, record and when necessary facilitate the scheduling of each stage of prosecution from arraignment through post-sentencing appeal and probation/parole supervision; attend and assist the prosecutors as needed with Court sessions.This position prefers a bachelor's degree, preferably in criminology, sociology or the administration of justice from an accredited institution. This individual must have excellent analytical skills, must demonstrate keen attention to detail and must be proactive to assist the prosecutors and other Court personnel with scheduling matters. This individual must maintain a reputation for objectivity, integrity and accuracy in the performance of his or her duties so as to instill confidence and trust in the minds of the Judges, Prosecutors and Defense Attorneys who must rely upon the documents prepared by the Case Manager.A. Ensure that required J-NET and NCIC/CLEAN terminal inquiries are conducted properly for each case.B. Contact out-of-county and out-of-state agencies to verify the specific nature and disposition of prior offenses, translate into equivalent Pennsylvania statutes all prior convictions from other states and obtain any required certified documentation from other jurisdictions.C. Calculate the prior record score, current offense gravity score and appropriate sentencing guidelines ranges and document these calculations on a sentencing guidelines form for each case.D. Enter case sentence information into the SGS Web system.E. Remain knowledgeable as to the current law regarding sentencing guidelines and alternatives, to include mandatory sentences and sentencing enhancements, treatment programs and facilities, diversionary programs and other factors and options to be considered by Prosecutors.F. Respond to inquiries from other judicial, law enforcement, correctional or custody agencies and institutions regarding criminal histories originating in Somerset County.G. Prepare criminal Information charging documents and notify the Clerk of Courts of the acceptance, amendment or withdrawal of each count as submitted by the Magisterial District Justice.H. Monitor the progress of all prosecutions from pre-arraignment through primary disposition, review entries made to the Clerk of Courts&rsquo; dockets and ensure accurate entry of relevant data in the District Attorney&rsquo;s computer database.I. Coordinate scheduling for arraignment, pretrial conference, trial, issuance of bench warrant, appearance upon service of bench warrant and other events with the Judges, Court Administrator, Probation Department, Sheriff, Prison staff and other parties.J. Assist with file location and provision to prosecutors for daily ad hoc warrant hearings.K. Attend and assist Prosecutors with Court sessions, including the process of live docketing data entry during court proceeding.L. Assist with the training and evaluation of newly assigned Case Managers.N. Perform data mining, collection and analysis work as Executive Staff and Administration deem appropriate.O. Utilize initial &lsquo;ready&rsquo; trial lists to assign appropriate &lsquo;trial ready&rsquo; designations in the Prosecutor Management System.P. Calculate and track Rule 600.Q. Any other duties as assigned by a supervisor or member of the Executive Staff.R. Coordinate the scheduling of attorneys with the Magisterial District Courts and the Court of Common Pleas and maintain the master calendar for the office.S.
