</li></ul><ul><li>You will have a high school diploma, or equivalent.</li><li>
Professional in appearance and conduct.</li><li>
Mature interpersonal skills to work effectively with co-workers.</li><li>
You can read, write, understand, and communicate in English with our Residents!</li><li>
You will have a positive and energetic attitude who will LOVE our Residents!</li><li>
You will possess computer skills to be able to print documents and enter information about our Residents.</li><li>
You must be active as this role requires standing, walking, bending, kneeling, and stooping all day.</li><li>
You must have the ability to frequently lift and/or move items up to 50 pounds.</li><li>
You must be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.</li><li>
You must be criminally cleared.</li><li>
Be in good health and physically and mentally capable of performing assigned tasks. Good physical health shall be verified by a health screening performed by a physician not more than six (6) months prior to or seven (7) days following employment. Must test free from pulmonary tuberculosis at time of health screening and annually thereafter.</li></ul>

Position- Medication Partner
Position Type: Part-Time

Our starting wage for Medication Partners is: $18.00-$23.00 per hour!
 
 
Shift Schedule- Wednesday 7am-7pm and Thursday 7pm-11pm

What are we looking for?<ul><li>
You must be at least eighteen (18) years of age.</li><li>
You must be appropriately certified per state guidelines and certification is active and in good standing.
</li></ul><ul><li>You will have a high school diploma, or equivalent.</li><li>
Professional in appearance and conduct.</li><li>
Mature interpersonal skills to work effectively with co-workers.</li><li>
You can read, write, understand, and communicate in English with our Residents!</li><li>
You will have a positive and energetic attitude who will LOVE our Residents!</li><li>
You will possess computer skills to be able to print documents and enter information about our Residents.</li><li>
You must be active as this role requires standing, walking, bending, kneeling, and stooping all day.</li><li>
You must have the ability to frequently lift and/or move items up to 50 pounds.</li><li>
You must be able to assist residents with sitting, standing, and walking, as well as assisting persons after a fall.</li><li>
You must be criminally cleared.</li><li>
Be in good health and physically and mentally capable of performing assigned tasks. Good physical health shall be verified by a health screening performed by a physician not more than six (6) months prior to or seven (7) days following employment. Must test free from pulmonary tuberculosis at time of health screening and annually thereafter.</li></ul>

Shift Details
Med Tech / Certified Medication Aide
PART-TIME Mon- Fri; Every other weekend 11P to 7A Shift
The Med Tech is responsible for monitoring the health and well-being of our residents and provides leadership support and assistance with resident care and services. Responsibilities include but are not limited to medication care assistance/administration, communication with resident support services and families and maintaining clinical quality assurance while leading, demonstrating and supporting all the elements of wellness which are core components of Phoenix programs and services.
PRINCIPLE DUTIES AND RESPONSIBILITIES
Med Tech / Certified Medication Aide
Resident Care<ul><li>
Identifies on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) process</li><li>
Ensures proper follow through and documentation for residents with a change in clinical care needs</li><li>
Supports the Wellness Nurse with Completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident on a monthly basis</li><li>
Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out</li><li>
Assist with obtaining weights and vital signs monthly for each resident prior to completion of monthly wellness visit</li><li>
Assist with resident care as needed</li></ul>
Requirements:

EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION
Med Tech / Certified Medication Aide<ul><li>
Must be certified Medication Aide and/or meet the state requirements for medication aide</li><li>
A minimum of one year of experience working with elders or disabled individuals, preferred</li><li>
Must have demonstrated Leadership capabilities</li><li>
Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred</li><li>
Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times</li></ul></div>
