Essential Responsibilities:</b></div><br>

<ul><li>Reviews all referrals to the Complex Case Management Programs within designated timeframe per policy and procedure and evaluate for case management based on established guidelines to ensure compliance with NCQA standards. <div></div><div><div><b>GENERAL DESCRIPTION:</b></div>
<div></div><div>The C2C LMSW Case Manager is a split position with .5 FTE dedicated to providing comprehensive case management and care coordination services to individuals with chronic or complex physical and/or behavioral health needs, with a focus on those recently released from incarceration. The TCM works as part of a multi-disciplinary care team to ensure patients gain access to appropriate medical, behavioral health, substance use disorder (SUD), educational, and social services. Services include assessments, planning, linkage, advocacy, monitoring, education, and follow-up support. The remaining .5 FTE for this position will be focused on providing therapeutic services to adult patients diagnosed with Behavioral Health Disorders and Substance Use Disorders (as well as other co-occurring disorders). Responsibilities include, but are not exclusive to: psychosocial assessments, diagnoses, individual therapy, group therapy, treatment planning, psychoeducation, referrals and consultations.</div>
<div></div><div><b>SPECIFIC DUTIES AND JOB FUNCTIONS</b></div>
<div></div><div>1. Conduct comprehensive assessments of medical, behavioral, and social needs, particularly for individuals transitioning from incarceration.</div>
<div>2. Develop, implement, and monitor individualized care plans in collaboration with the patient, care team, and other providers.</div>
<div>3. Facilitate linkage to primary care, behavioral health, SUD services, and community resources.</div>
<div>4. Provide education, counseling, and ongoing support to patients and their families regarding health conditions, treatment plans, and recovery goals.</div>
<div>5. Coordinate care transitions and advocate on behalf of patients to reduce barriers to accessing care.</div>
<div>6. Participate in an integrated team approach to patient care/treatment. Treatment team will include physicians, RNs, peer recovery coaches, clinicians, etc.</div>
<div>7. Monitor patient outcomes, reassess needs, and adjust care plans as appropriate.</div>
<div>8. Maintain timely and accurate documentation in accordance with Medicaid, state, and federal requirements.</div>
<div>9. Collaborate with Medicaid health plans, corrections systems, and community partners to ensure continuity of care for justice-involved individuals.</div>
<div>10. Respond to crises and provide appropriate interventions within scope of practice.</div>
<div>11. Provide individualized behavioral health treatment to consumers: including initial psychosocial assessment, diagnoses, individualized treatment plans and on-going therapy sessions</div>
<div>12. Maintain a therapy caseload that appropriately meets the needs of patients, utilizing evidence-based practices, best care practices, industry standards, organizational guidelines, appropriate accreditation standards, and applicable federal laws, including HIPAA</div>
<div>13. Refer patients for more specialized or inpatient interventions as clinically appropriate.</div>
<div>14. Maintain quality care through participation in peer review, weekly case review meetings, policy development and quality improvement programs as requested</div>
<div>15. Complete all documentation in an appropriate, timely manner and in accordance with third party compliance standards</div>
<div>16. Maintain and update professional knowledge and proficiency through continuing education, staff meetings and other resources specific to providing integrated behavioral and mental services in a federally qualified health center</div>

The Law Enforcement/Co-Responder Program is a partnership with Newton County law enforcement agencies and View Point Health aimed at reducing the incarceration of mentally ill individuals and linking them with treatment. The programs pairs behavioral health clinicians with officers and deputies to respond together in situation of mental health crisis. The clinician performs brief assessments consulting with the officers to determine appropriate dispositions for services, arrests, diversion from arrests, diversion from unnecessary emergency department visits, or some combination of criminal justice/behavioral health treatment involvement. The clinician also facilitates training for police officers to increase competencies in intervening with a behavioral health crisis. The clinician will work in conjunction with and collaboratively as part of the Emergency Service Provider.</p><br>

<p><b>
Duties &amp; Responsibilities:</b></p>
<ul><li>
Provide rapid crisis intervention and assessment to individuals who have come to the attention the of police, on an as-needed basis.</li><li>
Provide face to face or telephonic follow-up with persons served when clinically indicated.</li><li>
Accompany police officers on patrol as allowed by officer in charge.</li><li>
Work in collaboration with police to ensure that persons with behavioral health needs are appropriately diverted from the criminal justice system and referred for appropriate behavioral health services.</li><li>
Work in collaboration with VPH outpatient clinics and other programs to ensure the individuals are linked to services.</li><li>
Provide and/or coordinate, as needed training to police personnel regarding behavioral health, substance use, trauma spectrum disorders, and other behavioral health topics as identified.</li><li>
Compile and submit monthly statistics.</li><li>
Act as primary liaison mental health community providers and Law Enforcement.</li><li>
