Responsibilities and Purpose
<ul>
<li>Obtain and administer prescribed medications and treatment in accordance with approved nursing techniques.</li>
<li>Observe residents and record significant conditions and reactions. Notify appropriate parties of residents&rsquo; change in condition and reaction to drugs, treatments and significant incidents.</li>
<li>Take vital signs to detect deviations from normal and assess condition of residents.</li>
<li>Keep residents&rsquo; family and physician informed of any changes in resident behavior, medications or conditions of health.</li>
<li>Modify residents&rsquo; care plan, care directives, Activities of Daily Living (ADL) and Nurse Aide flow sheets in conjunction with residents&rsquo; changes in condition.</li>
<li>Provide bedside care.</li>
<li>Prepare and administer injections and/or enemas.</li>
<li>Collect samples and monitor catheters.</li>
<li>Provide wound care.</li>
</ul>
Required Experience and Education
<ul>
<li>Current and unencumbered Oklahoma Licensed Practical Nursing (LPN) License OR Oklahoma Registered Nurse (RN) License.</li>
<li>1+ years of experience in a Skilled Nursing or Long Term Care environment.</li>

Position Summary:

<li><ul><li> Provides nursing care in accordance with Resident Care Policies and Procedures and ensures the safety and well-being of the residents is maintained.</li></ul></li>
<li><ul><li> Demonstrates expertise in dispensing medications and treatments by all routes as ordered by the Physician within established time frames. Ensures no medications are left unattended.</li></ul></li>
<li><ul><li> Assesses effectiveness and observes for interaction and reaction of medication and treatment. Documents medication and treatment administration in a timely and accurate manner according to Resident Care Policies and Procedures.</li></ul></li>

</li><li>Supervises and assists in bowel and bladder training program for residents, as needed.</li>

<li>Receives medications and medical supplies daily and checks accuracy of orders received; logs narcotics appropriately; notifies RN supervisor of any discrepancies as soon as possible.
</li><li>Communicates with direct care staff with all resident care concerns at the beginning and throughout the shift.
</li><li>Answers families&rsquo; and visitor&rsquo;s questions as necessary.</li>
<li>Is responsible for emergency equipment and general nursing supplies; restocking medication/treatment carts and any other supplies used.
</li><li>Participates in care plan meetings when asked.</li>
<li>Makes rounds with physicians.</li>
<li>Reports all accidents and incidents to RN supervisor and documents on resident chart and on incident reports; informs doctor and family.</li>
<li>Transcribes physician&rsquo;s orders as per p/p; ensures all orders are written correctly and have appropriate dx.
</li><li>Monitors hydration of residents, including accurate I &amp; O. Notifies dietician and MD if not meeting fluid needs or exceeding needs and documents same in residents&rsquo; chart.
</li><li>Makes rounds during mealtime to see that each resident has received help as needed, assists in hand feeding, assures that sufficient fluids, special feedings and nourishments are administered as needed.</li>
<li>Completes assignments timely, i.e.: quarterly evaluations</li>
<li>Coordinates activities with ancillary departments to ensure that treatments plans are followed and/or revised as needed. (i.e.: therapy)
</li><li>Utilizes equipment properly and ensures direct care staff does same; communicates to RN supervisor if education is needed.
</li><li>Monitors weight of all residents, utilizing protocol if there is a significant weight loss.</li>
<li>Ensure plan of care is being followed.</li>
<li>Cooperates with all staff; is respectful to coworkers.
</li><li>Practices safe working habits.</li>
<li>Answers call l lights promptly.
