The firefighter is the initial classification within the Fire Department. Individuals in this classification normally work under the supervision of a Fire Lieutenant or Fire Captain. The primary responsibilities of a Firefighter are to respond to emergencies, to combat and extinguish fires, provide medical treatment to victims, perform rescues, and do other duties that may be required. Individuals in this classification must be proficient in driving the apparatus to which they are assigned and knowledgeable of the location and operating procedures of all equipment assigned to that apparatus. Work is reviewed through direct observation by the immediate supervisor. Any Firefighter may be assigned to another division within the Fire Department at the discretion of the Fire Chief. 
 
 
<ul><li>Responds to emergency and non-emergency calls for service, including those involving fire suppression, vehicle extrication, hazardous materials response, medical and traumatic emergencies, and other calls for service as required.</li>
<li>Performs firefighting and rescue tasks, including fire suppression, ventilation activities, and building searches; technical rescue tasks, including vehicle extrication, water, and rope rescue; provides emergency medical services; and responds to other calls for service as required.</li>
<li>Performs daily operational readiness evaluations of assigned fire apparatus, equipment, and tools; completes related documentation.</li>
<li>Performs maintenance and minor repair work to ensure the upkeep of department property and assists in the maintenance of station buildings and grounds.</li>
<li>Conducts fire station tours and performs other safety education activities such as school presentations, health fairs, and other public events related to fire safety, prevention, and community risk reduction.</li>
<li>Participates in wellness activities to maintain fitness for duty and perform essential job functions.</li>
<li>Participates in required training programs and maintains all required job-related certifications.</li>
<li>Performs pre-fire planning and building inspection duties.</li>
<li>May work out-of-class as a Driver/Operator, as staffing needs dictate.</li>
<li>Participates in the instruction of volunteer firefighters on a variety of topics.</li>

Minimum Qualifications

CHRISTUS Santa Rosa Hospital - Westover Hills (CSRH-WH) is a 150-bed hospital serving the fastest growing area of San Antonio. Specialized care includes orthopedic and surgical services, ICU, women&rsquo;s services, a newborn nursery, comprehensive cardiovascular care from diagnostics to open heart surgery, vascular lab, sleep center, emergency services, the CHRISTUS Weight Loss Institute, wound care, rehabilitation, and more. Summary:
The Paramedic is a certified and/or licensed paramedic and is part of the hospital&rsquo;s Emergency Department patient care team. Reporting to the ED Nursing Director or Nursing Manager, the Paramedic collaboratively works in a complementary and assistive role with nursing. Under the direct clinical supervision of the licensed physician present in the same area or adjacent area, the paramedic provides assistance and performs procedures, and is authorized to provide advanced life support, administer medications and perform a patient evaluation of signs and symptoms based on their field skill sets. Responsibilities:
<ul><li>Completely operates patient monitoring equipment, records, rhythm strips, runs 12 lead EKGs, biomarkers, and glucometer readings under the direction of the licensed physician.</li><li>
Performs phlebotomy, collects laboratory specimens as ordered and inserts saline lock, Foley catheter and nasal gastric tubes under the direction of the licensed physician.</li><li>

Superior Air-Ground Ambulance Service

History of the Company: Superior Ambulance Service started in 1959 with one ambulance and today is the largest independent, locally owned, and operated emergency medical services provider in the Midwest. Superior employs more than 3,500 licensed EMTs, Paramedics and Nurses, operating a fleet of more than 800 ambulances throughout Illinois, Indiana, Ohio, Michigan, and Wisconsin. Superior also provides Critical Care, helicopter, and fixed wing emergency medical transportation.</div><div></div><div> 
Superior Ambulance Service is participating in a grant-based program amongst 80+ Emergency Departments Nationwide. The Community Paramedic-Led Transitions Intervention (CPTI) team will dedicate half of their role by identify dementia patients to participate in a hospital to home transitions model of care and the other half support transport calls on the ambulance. CPTI is a program designed to support patients discharged from the Emergency Department. The Paramedic coach will provide a post-hospital phone call(s) soon after discharge, one in-person home visit and up to 3 follow-up telephone calls over 30 days. During these encounters, the Paramedic coach will utilize learned motivational techniques, behavior modeling, skill transfer, and role play to improve the patient's self-management. Areas of focus include effective medication management, encouraging primary care follow-up, red flag awareness and maintenance of personal health goals. Paramedic coaches do not deliver traditional EMS associated services (e.g. they do not provide medical care or medical evaluation). The Paramedic coach will travel to the participant in a marked vehicle and in uniform. When the Paramedic coach isn't supporting their CPTI duties they will provide traditional EMS on the rig with their partner.
</div>Responsibilities:
<div>
The primary duties and responsibilities of the Paramedic consist of, but are not limited to the following:</div><ul><li>
Perform the post-hospital phone call to develop the coaching relationship, explain the CPTI model, engage the person living with dementia (PLWD) and their care partner, and schedule the home visit.</li><li>
Perform the home visit within ~5 days following discharge. Work with the individual or care partner (together= &ldquo;dyad&rdquo;) to identify their biggest health concern and their immediate goals for the next 30 days. Coach the individual through the four pillars of care.</li><li>
Utilize active listening, motivation interviewing and teach back methods to develop individuals&rsquo; selfmanagement skills.</li><li>
Assess the patients baseline activation level for self-care through use of a standardized assessment tool.</li><li>
Complete three follow up phone calls to the individual on or about the 2nd, 3rd and 4th weeks following discharge.</li><li>
Coaches will be responsible for completing accurate and timely charting of patient encounters in accordance with learned processes and be accountable for completion of the intervention in the appropriate time frame. Complete data entry after each encounter on CTI+ platform.</li><li>
Participate in regular meetings with the program coordinator, CPTI national team, other CPTI coaches and various hospital partners for ongoing program management.</li><li>
Establish effective communications and relationships with ED/hospital staff and CPTI partners.</li><li>
Participate with quality assurance activities.</li><li>
Follow program procedures for documenting and tracking.</li><li>
Demonstrate strong organizational and time management skills.</li><li>
Participation in all required didactic, clinical and lab education programs and maintenance of all required certifications and licenses.</li><li>
Complete and maintain currency in all areas of safety education as required by Superior Ambulance, contracted air operators, and other healthcare partners.</li><li>
Demonstrate effective communication skills with all external and internal customers, including.</li><li>
patients and their families, hospital staff, the community, other Superior Ambulance staff and Critical.</li><li>
Care administration. Represents the study and Superior Ambulance in a professional manner.</li></ul>

Training Expectations<ul><li>

Coaches must attend mandatory CPTI training (appx 4 weeks) and all applicable follow-up trainings prior to launch.</li></ul><div> 
Physical Requirments</div><ul><li>
Must successfully complete pre-employment testing including a physical and lifting agility examination.</li><li>
Must be able to occasionally work outdoors in inclement weather.</li><li>

Job Requirements

Educated and currently certified/licensed in the state of employment and according to state requirements as a Licensed Practical Nurse, Licensed Vocation Nurse, Paramedic, or EMT. Current CPR certification required.
