<p><b>About the department: </b>Supporting hospital and emergency room patients of all ages, the Social Services Department is available 24/7 to offer assistance with discharge planning, resource referrals, and consultative services. Working as part of an interdisciplinary team, Social Workers and Designees provide clear communications to patients, family members, and other care team members to ensure smooth transitions and continuum of care.</p>
<p><b>A typical day will include: </b>Conducting safety risk assessments to make appropriate recommendations and referrals for safety planning with patients and/or caregivers.</p>
