The Palliative Care LPN provides remote support to seriously ill patients and their families by monitoring symptoms, managing pain, and offering emotional support over the phone. <li>Electronic Medical Record (EMR) activities to include referral entry, name changes, demographic changes, NTUCs and processing deaths.</li>
</ul>
Scheduling:
<ul><li>Coordinates field staff schedules utilizing EMR scheduling tool. Ensure patients are scheduled for home visits according to departmental standards and processes.</li>
<li>Assist nurses with arranging unscheduled visits daily as needed.</li>
<li>Schedule and/or assist nurses with scheduling evaluation and admission visits for hospice enrollments in all settings as needed.</li>
