Oversees client recordkeeping in compliance with appropriate regulatory agencies.</li><li>
Creates and implements life skills training and education opportunities for tenants.</li><li>
Collaborates with referring agencies, funders, and all related internal staff to effectively support individual client needs.</li><li>
Maintains regular contact to coordinate client benefits and address all other treatment needs.</li><li>
Reports to service providers about significant individual behavior or events that will require an immediate response.</li><li>
Acquire concrete resources to meet individual needs.</li><li>
Complete letters of request/referral forms, and other required documents of eligibility or charitable organizations and public agencies to advocate for resources that the client is entitled or eligible for.</li><li>
Makes referrals for education, employment, legal issues, and others as needed.</li><li>
Document the need for financial assistance or other resources such as wrap-around from the project to stabilize client finances.</li><li>
Utilizing various databases and/or online platforms, maintaining all documentation by the agency and appropriate regulatory standards.</li><li>
Participates in relevant Agency meetings and ongoing development of professional knowledge and skills.</li><li>
Actively supports the financial stability of the organization by meeting or exceeding established productivity and documentation standards.</li><li>
Informs supervisor of significant events/issues related to clients as appropriate and follows the program&rsquo;s defined incident reporting process as required.</li><li>
Attends regular program and departmental meetings, case reviews, and CFC quarterly meetings.</li><li>
Maintains and develops professional knowledge by completing recommended education and training, workshops, conference attendance, and readings.</li><li>
Maintains knowledge of regulations and standards that govern client population.</li><li>
Participates in appropriate interagency meetings with other treatment agencies and community groups as required. Provide updates and communication with providers including those who cannot be represented in person.</li><li>

Qualifications
Education: Bachelor&rsquo;s degree in social work, human services, criminal justice, or related field preferred. An equivalent combination of education and experience will be considered.
Credentials: Valid and clean NYS Driver&rsquo;s License. Valid PPD screening is available.
Experience: Minimum two years of experience with case management and community outreach in areas such as but not limited to persons affected by substance use disorder, prison re-entry, geriatrics, mental health, and refugees. Knowledge of HMIS, Medicaid, SNAP, Social Security, and legal systems is helpful. May require the performance of duties in various community locations and community settings that may not be handicapped accessible.

<div>St. Thomas Community Health Center is looking to hire a non medical case manager. The Non-Medical Case Management (NMCM) provides advice and assistance to empower clients to access a variety of necessary services to support medically related outcome measurements. Clients will have varying levels of needs throughout their enrollment in services. These services may include: medical, social, community, legal and financial. Clients who demonstrate a low level of need may benefit from NMCM. Enrollment in NMCM is not always permanent; as a client may move from one type of case management to the other depending on current situations. Frequent and ongoing assessments by case management (non-medical and medical) should occur to ensure clients receive the appropriate level of care.</div><div>

<div>Responsibilities</div><div> 
</div>
<ul><li>Completion of comprehensive assessment of service needs conducted by a non-medical case manager meeting the same qualifications as a medical case manager</li>
<li>Coordination of services required to support health outcomes</li>
<li>Periodic re-evaluation of client needs (housing, transportation, food, legal, emergency financial assistance, and other needed services).)</li>
<li>Encounter clients in their environment, which may include a residence, a public facility, in the streets, or in the facilities of the non-medical case manager</li>
<li>Inform the client of all Ryan White Part A funded services available, AND</li>
<li>Inform the client of all Ryan White Part A funded case management agencies in New Orleans Eligible Metropolitan Area (NO EMA), AND</li>
<li>What client can expect if s/he enrolls in case management services</li>
<li>Establish and/or verify client eligibility for services</li>
<li>Collect required state/federal client data for reporting purposes; AND</li>
<li>Completion of a full Client Eligibility Review and Certification Form (CERV)</li>
<li>Complete all Ryan White forms as required by HIPPA, Grantee, and the agency.</li>
<li>Complete and develop Ryan White Plan of Care in collaboration with the client in according to the client&rsquo;s needs and follow-up on outcomes</li>
<li>Complete referrals for Ryan White services after assessing client</li>
<li>Upload eligibility documents into electronic records system within 3-days of eligibility</li>
<li>Answer and return phone calls, emails, faxes, and makes copies in a timely manner</li>
<li>Complete all Ryan White billing forms for every client interviewed, time clock corrections, and mileage on a weekly basis and submit to the Finance Department.</li>
<li>Work in collaboration with the medical case manager and medical case manager supervisor</li>
<li>Ensure recording of health indicators (CD4, viral load, acuity)</li>
<li>Monitor service utilization (including medical appointment adherence)</li>
<li>Treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments</li>
<li>Establish whether client demonstrates a need or continued need to enroll in non-medical case management services offered by the service provider</li>
<li>Gather an on-going basis appropriate client information in order to determine client eligibility</li>
<li>Reassess at least every six month or as required to respond to a change in client status, utilizing assessment tools as mandated by OHP</li>
Adhere to standards of service as outlined in the Non-Medical Case Management by the OHP
