Responsibilities:
<ul><li>Receive rapid intake of clinical consult support requests via phone, voice over internet</li><li>
protocol, custom web form, or other request modality deemed necessary by the Government and route promptly and appropriately.</li><li>
Facilitate the rapid transfer of relevant clinical and operational information from the requester to the specialty provider.</li><li>
Understand the originating site operational limitations and military operational environmental factors.</li><li>
Execute tasks expediently despite limited or ambiguous information.

Identify problems and shortages of specialty care to the supervisor.

Collect and integrate data from various types of software, to include CHCS, AHLTA, MHS Genesis, and MS Office (Excel, Word, Outlook, and PowerPoint).</li><li>

<div>The Home Health Patient Care Manager is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.

<li>Receives referrals and ensures appropriate clinician and/or therapist(s) assignments for timely patient evaluation by signing off after authorization and plotting start of care (SOC) visits.</li>
<li>Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.</li>
<li>Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance to physician orders.</li>
<li>Oversees and assures development, implementation, and updates to the individualized patient plan of care, as appropriate.</li>
<li>Manages and documents phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians. Uses coordination notes to document, as needed and appropriate.</li>
<li>Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.</li>
<li>Coordinates all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.</li>
<li>Follows-up on lab and other clinical diagnostic test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow-up, and needed plan of care modifications and communicates such to clinicians.</li>
<li>Schedules, prepares for, facilitates, and documents case conference/SOC reports and facilitates effective exchange of information across disciplines especially with adverse findings, changes in patient condition, daily and urgent updates, as necessary.</li>
<li>Assists clinicians in coordinating the transfer and discharge of patients from agency services as indicated by the physician.</li>
<li>Receives report from field clinicians prior to scheduled days off on patient status and ongoing needs.</li>
<li>Processes new orders and updates the visit frequency, as appropriate, when the oncall RN takes supplemental verbal orders which alter frequency going forward.</li>
<li>Writes and processes orders when taking verbal orders directly from the physician and communicates such to field clinicians.</li>
<li>Assures payer change documentation is completed properly and timely, as required.</li>
<li>Reviews clinician visit notes weekly to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff. Takes necessary action to correct adverse findings and communicates trending to clinical director.</li>
<li>Reviews, evaluates, and supervises service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as random patient visits, medical record reviews and case conferences.</li>
<li>Enters infections and incidents/occurrences into the online Risk Management Incident Reporting System, as specified by policy.</li>
<li>Assists in the orientation of new agency personnel.</li>
<li>Provides direction and leadership to clinical team members in collaboration with the clinical director.</li>
<li>Provides direct patient care, as necessary, in accordance to scope of practice and physician orders.</li>
<li>Participates in QAPI program.</li>

Position Summary:

In this role, the successful candidate Coordinates and procures services necessary to meet the post-acute care needs of patients identified as requiring such services. Ensures a timely and smooth transition to the most appropriate post-acute care setting. Works collaboratively with interdisciplinary patient care staff both internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care. Ensures maximization of provider reimbursement of patient days through timely and accurate insurance reviews.

Job Responsibilities:

<li>Performs other duties as assigned</li>
<li>Performs an initial utilization review of patients for appropriateness of setting using industry standard utilization clinical criteria including, but not limited to InterQual, Milliman Care Guidelines (MCG), etc. Collaborates with the Attending of record and the Physician Advisor as indicated for review of any patient not meeting criteria for level of care ordered. Performs continued stay and extended stay reviews when there is a change to a higher level of acuity, authorized days expire, required by the insurance payer contractual agreement, stay is denied or reduced to a lower level of care.</li>
<li>Provides patient clinical reviews to payer in a timely and efficient manner to obtain certification of the patient days and secure reimbursement. Confers with Attending Physician, Physician Advisor, and multidisciplinary team as needed to mediate concurrent denials.</li>
<li>Screens all inpatients utilizing case management departmental screening criteria for potential post acute care needs. Perform an assessment for discharge needs for all patients that meet screening criteria, are referred by care team or are requested by patient/designee. Develops a discharge plan in collaboration with the patient/designee and care team incorporating patient preferences. Implement the discharge plan with referrals, including necessary medical information, to appropriate post acute care services. Reassessment of this process will be done with ongoing review of all readmissions to ensure that the plan was responsive to the discharge needs.</li>
<li>Completes all necessary documentation as per departmental policy and procedure utilizing Allscripts. Completes Patient Review Instrument (PRI) accurately and in a timely manner.</li>
<li>Identifies and reports risk and quality issues to appropriate department. Tracks avoidable days in Allscripts to identify opportunities for improvement.</li>
<li>Provides patients with all necessary notices in accordance with requirements for CMS conditions of participation and insurance payer contracts. Issue Important Message (IM), Discharge Notice, Advanced Beneficary Notice (ABN), Detailed Notice of non-coverage, Preadmission/Admission Notice of non-coverage and Hospital Issuance of non-coverage (HINN) as appropriate and in accordance with hospital policy and procedure. Process Medicare Code 44 as appropriate in accordance with policy. Ensure patientdesignee is provided with eplanation of billing status when a Code 44 or observation is ordered.</li>
<li>Participates in patient care unit interdisciplinary meetings to assess patients' current and projected clinical status, plan of care, discharge plan and expected discharge date. Facilitates communication among the interdisciplinary health care team members. Coordinates patient care conferences among appropriate team members and patient/designee to discuss identify barriers, concerns, resolve conflicts and communicate choices in the plan of care. Participates in in-service activities and continuing education programs as scheduled.</li></ul> 


Minimum Qualifications: 
To qualify you must have a Bachelor's degree in Nursing required for employees hired after 9/1/2020 (those who were hired prior to 9/1/2020 are grandfathered). Current and valid New York State Registered Nurse License. BLS certification (Issued by the American Heart Association-AHA). 3 - 5 years Clinical acute care.
