SUMMARY

The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. He/She provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, prescribing medications or treatments, coordinating and collaborating with patients, families and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities.

RESPONSIBILITIES
<br><br>
1. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
<br><br>
2. Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record.
<br><br>
3. Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services.
<br><br>
4. Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.

5. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
<br><br>
6. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. 7. Assures that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members.

8. Reviews and develops protocols for treatment and proposes options for interventions based on the literature in collaboration with the collaborating physician. 9. Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols

Must have and maintain a valid state driver license, have a registered and reliable vehicle that meets state law standards, and meets the insurance standards of the company. This position requires at least the following minimum requirements: Graduation from an accredited School of Nursing.

</b></div><ul><li>Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care.</li><li>
Take responsibility for patient palliative care from the initial assessment through discharge. Assessment at a minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life.</li><li>
Establish the palliative plan of care, individualized for each patient, in consultation with the patient, family and other members of the Palliative Care Team.</li><li>
Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care.</li><li>
Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.</li><li>
Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. Order and interpret laboratory and other diagnostic tests within the scope of professional practice. Order treatments and durable medical equipment as indicated.</li><li>
Communicate clinical findings to the Palliative Medical Director as needed, but no less than bi-weekly. Re-assess effectiveness of care plan on a regular basis and modify as needed.</li><li>
Maintain effective working relationships with supportive care team (e.g., Social Worker, Spiritual Care Coordinator), as applicable, and involve team when patient's care plan warrants team participation to achieve desired outcome.</li><li>
Establish and maintain effective working relationships with those contacted in the course of work.</li><li>
Communicate and document clinical findings, treatment plan and care provided in the patient's medical record in a manner consistent with acceptable standards in order to support sound medical practice and reimbursement for services provided.</li><li>

Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care.</div></li><li><div>
Perform in-depth patient assessments, including physical, emotional, and psychosocial evaluations.</div></li><li><div>
Develop and implement individualized care plans in partnership with patients and families.</div></li><li><div>
Manage and adjust treatment plans based on patient needs and progress.</div></li><li><div>
Prescribe medications (including controlled substances) and order diagnostic tests as permitted by state law.</div></li><li><div>
Maintain timely, accurate clinical documentation in compliance with professional standards.</div></li><li><div>
Communicate effectively with physicians, caregivers, and other healthcare professionals.</div></li><li><div>
Participate in evening/weekend call rotation as needed.</div></li><li><div>

About You:

Meets mandatory continuing education requirements for Gentiva and licensing boards.</div></li><li><div>
Proficient in point-of-care software and technology.</div></li></ul><div><b>
Education/Experience:</b></div><ul><li><div>
Master of Science in Nursing from an accredited school of nursing (or equivalent per state law).</div></li><li><div>
Minimum two years&rsquo; Nurse Practitioner experience.</div></li><li><div>
Specialty in Family or Adult Medicine preferred.</div></li><li><div>
Experience with Part B billing.</div></li><li><div>
Ability to work autonomously in an advanced practice role.</div></li></ul><div><b>
Licenses/Certifications/Agreements:</b></div><ul><li><div>
Current RN license and Nurse Practitioner certification/license as required by state law.</div></li><li><div>
