</b></div><ul><li>Provide comprehensive adult primary care, including health maintenance, disease prevention, diagnosis, and treatment of acute and chronic medical conditions.</li>
<li>Conduct comprehensive medical assessments and develop individualized treatment plans based on patient needs.</li>
<li>Manage complex chronic diseases, including diabetes, hypertension, COPD, CHF, and other multi-system conditions.</li>
<li>Perform routine preventive screenings, physical exams, and immunizations in alignment with USPSTF and HRSA guidelines.</li>
<li>Provide post-hospital and post-ER follow-up care to ensure safe transitions and continuity of care.</li>
<li>Maintain accurate and timely documentation in the electronic health record (EHR) in compliance with clinical and regulatory standards.</li>
<li>Educate patients and families on disease management, treatment options, and preventive health behaviors.</li>
<li>Collaborate with internal team members and external specialists to coordinate patient care.</li>
<li>Participate in value-based care programs, performance improvement initiatives, and HRSA quality measures to improve population health outcomes.</li>
<li>Contribute to care coordination and referral management for specialized services.</li>
<li>Participate in call coverage rotation as assigned (typically one week/month).</li>
