POSITION RESPOSIBILITIES:<ul><li>
Design, develop and execute Medical Awareness &amp; Educational programs needed for HCPs.</li><li>
Obtain, analyze and share patients and HCPs insights related to Patient Programs across the region.</li><li>

Clients will have varying levels of needs throughout their enrollment in services. These services may include: medical, social, community, legal and financial. Clients who demonstrate a low level of need may benefit from NMCM. Enrollment in NMCM is not always permanent; as a client may move from one type of case management to the other depending on current situations. Frequent and ongoing assessments by case management (non-medical and medical) should occur to ensure clients receive the appropriate level of care.</div><div>
