SUMMARY: This position works with the treatment and support team within a co-occurring residential treatment program for adult males. The Nurse's primary responsibilities are oversight of medication for the program including medication ordering and refills, checking medication documentation, and initial and ongoing training for staff who oversee the self-administration of medications.ESSENTIAL DUTIES AND RESPONSIBILITIES:<ul><li>Provides training to new personnel to be approved to oversee the self-administration of medications and documentation related to medications. This includes ensuring individuals satisfactorily demonstrate all necessary skills and competence before approving them to perform this task independently</li><li>Reviews medication documentation completed by non-medical staff to ensure medication self-administration is being consistently documented, making corrections as needed and following up with staff on any documentation omissions or improvements needed</li><li>Provides ongoing training to staff after medication errors or other medication documentation issues</li><li>Checks each resident&rsquo;s medication stock routinely calling in refills and reaching out to the pharmacy and/or their prescribers as necessary when refills are needed</li><li>Conducts medication counts ensuring documented medication amounts matches medication administration records and controlled substances logs</li><li>Maintains inventory for any medical supplies and over-the-counter stock medications and communicates items to be ordered to management</li><li>Ensures all first-aid kits remain stocked</li><li>Checks any program medications for expirations and ensures expired medications are replaced; oversees medication destruction and disposal as needed</li><li>Meets with residents reporting medical symptoms and triages them for being sent out for medical evaluation and/or care or meeting with a nurse practitioner/prescriber</li><li>Coordinates medical care for residents with outside providers as needed</li><li>Provides health education to residents and education on their medications; facilitates resident groups on health education topics, including, but not limited to nicotine cessation, HIV/AIDS, viral hepatitis, tuberculosis, sexually-transmitted diseases, overdose prevention, medications and other health-related topics</li><li>Reviews health assessments received from outside medical providers to ensure our level of care is appropriate to meet their medical needs</li><li>Coordinates and supports telehealth prescribing services provided to residents on-site</li><li>Completes any required documentation in the resident record according to agency timelines and other requirements</li><li>Responds to and manages any medical emergencies while on the premises</li><li>Maintains current license in Massachusetts as an LPN or RN</li></ul>QUALIFICATIONS ANDEXPERIENCE:<ul><li>Must be licensed as an LPN or RN in the State of Massachusetts</li><li>Must maintain current CPR certification</li><li>Experience working with people suffering from substance use and/or mental health conditions is preferable, but not required</li></ul>Job Types: Full-time, Part-timePay: $35.00 - $45.00 per hourPeople with a criminal record are encouraged to applyWork Location: In person

Responsibilities
<div>1. Follow established performance standards and performs duties according to nursing service policies and procedures.</div>

<div>3. Responsible for all as needed medications and follow up in one hour.</div>
<div>4. Follows care plan as to how medications are to be given.</div>
<div>5. Follows medication delegation guidelines as received by nurse delegator as needed.</div>
<div>6. Identify special resident problems and report them immediately to the nurse.</div>
<div>7. Provide direct resident care as assigned, completing assignments accurately and in a timely manner.</div>
<div>8. Responsible for documenting all medications and concerns for every resident under direct care.</div>
<div>9. Identify safety hazards and emergency situations and initiates corrective action immediately.</div>
<div>10. Provide nursing care to residents without violating residents&rsquo; rights.</div>
<div>11. Participate in facility education programs as assigned.</div>
<div>12. Attend all classes as assigned and complete assignments accurately and timely.</div>
<div>13. Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit.</div>
<div>14. Report suspected abuse or neglect to the appropriate state agencies and personnel.</div>
<div>15. Accurately document incidents and/or unusual problems according to established facility procedures.</div>
<div>16. Review care plan daily and performs nursing assistant tasks as outlined.</div>
<div>17. Receive all medications in PCC as needed.</div>

 Position Summary
 
 
 
 The Certified Medication Technician assists in performing routine duties to include medication administration, inventory control and storage of pharmaceutical items. Certified Medication Technicians work under the direct supervision of the Director of Nursing and/or, depending on scope of practice, receives clinical guidance from a Registered Nurse or Licensed Practical Nurse..
 
 
 
 Duties/Responsibilities

 Prepares and administers prescribed medications to patients under the supervision of a licensed Registered Nurse, and in compliance with all facility policies and procedures and Pharmacy Board rules and regulations governing medication administration and documentation. Recognizes and adheres to the six (6) rights of medication administration.

 Adheres to all facility policies and procedures and Pharmacy Board rules and regulation regarding the administration and documentation of narcotic medications.
 </li>
 <li>
 Documents administration of medication on the Medication Administration Record (MAR).
 </li>
 <li>
 Documents medications not administered on the MAR and the reason for not being administered.
 </li>
 <li>
 Adheres to proper infection control techniques during the medication administration process, including the incorporation of universal precautions when indicated.
 </li>
 <li>
 Participates in data collection, medication audits, and other performance improvement activities related to medication administration.
 </li>
 <li>
 Following policy and utilizing appropriate documents, flow sheets and forms, documents all health-related encounters for patients.
 </li>
 <li>
 Reports any adverse patient medication reactions immediately to a supervisor.
 </li>
 <li>
 Ensures the medication cart remains clean and orderly.
 </li>
 <li>
 Reorders and replaces medications and medication administration supplies, as needed, in order to perform routine job responsibilities. Checks expiration dates on medications and supplies.
 </li>
 <li>
 Ensures the confidentiality and rights of patients, the confidentiality of the health systems and department documents, and adhering to HIPAA Regulations.
 </li>
 <li>
 Assists other healthcare staff as needed in the medical unit within the scope of practice.
 </li>
 <li>
 Identifies basic signs and symptoms of deviations from normal health status. Provides this information to the nurse and/or physician.
 </li>
 <li>
 Works in conjunction with nurses to maintain daily duties required to run the medical unit in an organized fashion.
 </li>
 <li>
 Provides appropriate first aid and CPR. Reports findings to the nurse and/or physician.
 </li>

 Provides patient teaching as instructed.

 Performs clerical duties such as answering the phone, filing documents in patient record.
