POSITION SUMMARY:
The Memory Care Director position champions Charter Senior Living&rsquo;s Flourish Memory Care philosophy and lives the core values and mission of Charter Senior Living; Enhancing the Human Spirit. This position is responsible for the day-to-day operations and leadership of the Memory Care Neighborhood and is responsible for training, development, and ongoing compliance of the program standards for the Flourish Memory Care Programs, community policies and procedures and applicable state regulations as they apply to memory care. This position also works collaboratively with the management team to ensure the planning and implementation of all therapeutic programming, entertainment, service planning and achievement of occupancy development goals. The Memory Care Director is the process owner to implement and oversee the points of distinction established in the areas of Flourish Restore, Flourish Enrich, Flourish Connect, Flourish Create, Flourish Engage, and Flourish Contribute. This role must possess compassion, and a genuine concern for the well-being and interest of seniors, along with experience in dementia care and life enrichment programming.
Knowledge, Skills, Abilities, and Experience:
Area of study: Recreation Therapy, Social Work, Human Development, Psychology, or related field. 
Years of Experience: Mid-Level. Minimum of three years-experience working with Seniors with at least one year being with persons with Dementia.
 
Type of Experience: Three to five years of experience working with individuals with Dementia including leading programs, handling complex behavioral issues, and educating staff on families on Alzheimer&rsquo;s or Dementia related topics.
 
Special Certifications: Activities Director Certification, Certified Therapeutic Recreation Specialist, Certified Dementia Practitioner, or other related Certifications. 
Level of Formal Education: Associates degree or equivalent experience required. Bachelor&rsquo;s degree or equivalent experience preferred.

 Responsible for environmental rounds to check for safety of exit doors, resident room safety and unit safety, and adherence to brand standards

 Coordinates and facilitates the RCP Training program for primary staff on unit. Responsible for tracking completion and scheduling of RCP classes.
 Suggests residents for restorative potential and assures program is consistently carried out by staff to maintain resident's maximum practicable level of function.
 Cooperate with other resident services for complete evaluation of care.
 Assist in establishing linkages with and arranging for services from public and private sources for income, health, mental health and social services.
 Assist the resident in making application for, and maintaining, income entitlements and public benefits.
 Coordinate care planning meets per state regulations.
 Coordinates, implements, and monitors all resident service plans with the Wellness Director.
 Utilizes Brandywine standards and standards of professional organizations in conducting resident care.
 Demonstrates knowledge and skills necessary to safely provide care appropriate to age of residents managed on unit.

 Assist with Admits, transfers, and discharge residents as required.

 Requisitions and arranges for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures.
 Assists the physician on rounds and ensures that pertinent information is communicated to and from him/her.
 Assists in resident rehabilitation activities. Where required, assists in ambulation, and provides emotional support. Monitors the results of physical therapy activities.
 Gives direct physical and psychological care in emergencies, as well as in the presence of illness or disability in order to maintain life, provide comfort, reduce stress, and enhance the resident's ability to cope. Reports all observations and changes in condition to the RN.
 Prepares and administers medications as ordered by the physician and utilizing all standards/policies of medication administration.
 Ensures that adequate supplies and equipment is on hand to meet the nursing needs of the residents.
 Complies with Controlled Substances policy and procedures in maintaining accurate inventories and controlled logs. Follows appropriate guidelines for investigation of discrepancies.
 Reviews medication administration records for completeness of information, accuracy in the ECP of the physician's order, and adherence to stop order policies. Reports any discrepancy to the RN.
 Disposes of drug and controlled substances as required, and in accordance with established procedures.
 Monitors residents ,reports to the RN and take appropriate action as instructed by the RN and MD for side effects and adverse reactions to medications.
 Adheres at all infection control policy and procedures.
 Administers and documents all treatments as ordered and in accordance with policies and procedures.
 Maintains accurate resident care records and document pertinent data reflecting the use of the nursing process, the care provided and resident response to care. Reports all changes in resident care or condition to the RN.
 Assists in the development of resident care assignment sheets.
 Reviews all resident care manager assignments for completion
 Works in partnership with the INVEST Coordinator to ensure a therapeutic and socially engaging environment

Qualifications 
Education/Training:
Licensed Practical Nurse Proof of successful graduation from accredited Nursing Program and possession of an unencumbered LPN license. Training in dementia care desired.
Experience:
Previous experience with Dementia population preferred. Previous experience in a hospital, assisted living, skilled nursing care or other related medical community required. Working Conditions: Moves intermittently during working hours and subject to frequent interruptions. May be subject to hostile and emotionally upset residents, family members, personnel and visitors. Must be able to cope with mental and emotional stress of the position.
