Position Summary: We are seeking a passionate and dedicated Activity Coordinator to join our senior living community's Memory Care Department. The Activity Coordinator will play a crucial role in enhancing the quality of life for our residents with memory-related challenges, such as dementia or Alzheimer's disease. As the Activity Coordinator, you will be responsible for planning, organizing, and implementing engaging and stimulating activities tailored to the unique needs and abilities of our Memory Care residents. Your primary goal will be to create a supportive and enjoyable environment that promotes socialization, cognitive stimulation, and emotional well-being. Essential Functions:
<ul><li>Assesses each patient/resident for interests, talents, and previous patterns of time use, and develops activity approaches based on that assessment.</li><li>
Establishes and maintains necessary records regarding activities' attendance and activity care plan approaches.</li><li>
Develops activity goals and objectives for each patient/resident and integrates the activity approaches within the Patient/Resident Care Plan.</li><li>
Identifies facility and community resources that can contribute to the recreation program. Plans and schedules activities and develops a monthly calendar of activities.</li><li>
Secures entertainment, program leaders, volunteers, materials, equipment, and transportation well in advance of scheduled dates.</li><li>
Assures all patients/residents are involved in activities: group, individual (bedside) and/or independent on a weekly basis.</li><li>
Ensures 6 activities are held daily and at least 2 evening activities are held per week.</li><li>
Recruits and trains a volunteer activities staff and maintains volunteer programs that utilize community involvement.</li><li>
Evaluates effectiveness of programs in terms of patient/resident outcomes.</li><li>
Develops and maintains a budget for activities programs.</li><li>
Coordinate Facility newsletter or other promotions of activities programs through local media.</li><li>
Serves on various committees as requested by Administrator.</li><li>
Ability to function as a Team Leader/Role Model.</li><li>
Has reviewed Cantex Continuing Care Networks Clinical Policies and Procedures for Abuse Prevention and knows the employee's responsibility to enforce it.</li><li>
Completes an initial recreational assessment and Section F of the MDS within 5 days of admission and/or change of condition.</li><li>
Coordinates with the NSD regarding the patients/residents Meal of the Month and other food-related programs.</li><li>
Participates in Discharge Planning and recapitulation of patients/residents stay.</li><li>
Complies with protocols outlined in the PCMS #10, Activities.</li><li>
Attends weekly/monthly in-services, understands, and complies with patient/resident Care Management Systems.</li><li>
Responsible for assuring patient/resident safety as appropriate.</li><li>
Performs other tasks as may be assigned.</li></ul>
