Title: Service Coordinator (Mental Health Case Manager)

Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Responsibilities:

The Case Manager is responsible for developing, coordinating, and implementing services for adult consumers as identified in the goal-oriented service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life.

The Recovery Coach will:
<ul><li>Link consumers with a mental health or behavioral health diagnosis to resources and encourage, teach, and guide them to advocate for themselves.</li>
<li>Case Manager will complete comprehensive assessments, service plans, and environmental matrixes to determine level of care and frequency of contact with consumers on their caseload.</li>
<li>Complete other paperwork requirements from OMHSAS, Magellan Behavioral Health, Montgomery County, and Merakey and will be monitored by Case Manager and completed timely.</li>
<li>Have appointments with consumers in their homes, community, and hospitals and should be familiar with Montgomery County, including being able to access resources to meet client's needs.</li>
<li>Carry a caseload between 25-30 clients and Case Manager will meet with clients according to matrix score, either every 14 or 30 days while maintaining Merakey's productivity standard.</li>
<li>Attend supervision meeting with Manager monthly and team meetings weekly.</li>
<li>Work typically Monday-Friday: 9:00am - 5:00pm but shift may vary at times based on client's needs. This role includes optional on-call.</li>

This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
Earn $22.29 per hour.

About the Role

The Case Manager will have the prime responsibility for organizing comprehensive service planning and assessment activity and assuring service plan implementation through direct case management or co-provider services. The Case Manager will also have either primary or shared responsibility, depending upon the specialty/discipline, in providing the residents with specific linkages/advocacy services with co-providers of housing, mental health, substance abuse, and healthcare services etc. as well as with parole boards. Together with the residents, the Case Manager will formulate residents' comprehensive service plan in conjunction with specialty program staff. For some residents the Case Manager will be the major provider of a particular service and, for others, a secondary support agent.

Primary Responsibilities
<ul><li>Participate in resident orientation.</li><li>Complete assessments and maintain service plans using integrated healthcare approaches.</li><li>Provide counseling, daily living instruction, and crisis intervention.</li><li>Transport residents to appointments and support income optimization.</li><li>Facilitate recreational and vocational linkages.</li><li>Conduct inspections, document progress, and complete incident reports.</li><li>Participate in quality improvement and maintain accurate records.</li><li>Attend required meetings, trainings, and supervisions.</li><li>Execute emergency plans including CPR/First Aid.</li><li>Perform additional job-related duties as assigned.</li></ul>
