Job Summary</b><b>:</b><br>
Under direction, provides social work and case management services to support the health and safety of individuals served. Provides person-centered planning and support to clients in making informed choices regarding services. Maintain caseload and provide ongoing case management including crisis support.</li><li>
Effectively interview and engage clients to assess and evaluate needs.

Make field visits to client&rsquo;s homes, other facilities, and community-based settings.</li><li>
Explain policies, regulations, and programs to clients and the public.</li><li>
Advise clients of rights and responsibilities under programs or services.</li></ul>
</li><li>Through direct observation, assess and identify individual and family strengths, needs, and risks. Develop and complete assessments and documentation, including social histories, home assessments, and individual or family service plans to address identified service needs and goals. Develop and implement service plans and/or treatment plans to support safety and wellbeing.</li><li>
Record all client contact in statewide social services information system (SSIS). Maintain accurate client records in Electronic Document Management Systems. Enter necessary data to access funding streams; improve computer skills as required by position.</li><li>
Complete all paperwork in a timely manner in accordance with agency policy and mandated timelines. Prepare and maintain case records which meet federal, state, and agency guidelines.</li><li>
Generate case plans and assessments to outline needs, current functioning levels and expectations of clients and other involved parties. Prepare and maintain progress notes, narrative summaries, reports, and other required case documentation in the file and in Statewide and agency databases.</li><li>
Facilitate access to and refer clients to appropriate community, regional and statewide services.</li><li>
Follow through on treatment and service plans.</li><li>
Monitor progress toward goals and objectives.</li><li>
Prepare court petitions, write, and file court reports, summarize current evaluations, and prepare updated documents for court situations as required. Provide testimony during court hearings. Provides testimony or written recommendations in county or district court or in administrative appeal hearings regarding client cases.</li><li>
Perform intake as assigned.</li><li>
Prepares necessary correspondence, required narratives, eligibility reviews and other incoming/outgoing communication about the client.</li><li>

Title: </b>Service Coordinator (Mental Health Case Manager)</p>
<p><b>
Job Duties:</b> The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.</p>
<p><b>
Schedule:</b> Full-Time</p>
<p><b>
Qualifications:</b></p>
<ul>
<li>Two (2) years of full-time experience or part-time equivalent in a mental health setting.</li>
<li>A bachelor's degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.</li>
<li>Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.</li>
<li>Familiarity with regulatory standards.</li>

<p><b>About the Campus: </b>Located in the beautiful central California coastal city of Santa Barbara, clients are welcomed into our safe, calming, and homelike environment, where they will receive compassionate recovery support from our diverse team of clinical staff who are trained in Trauma-Informed Approaches of care. Our clients receive comprehensive mental health services and support for up to 23 hours. After this, they are referred to mental health services in the community that will best meet their needs so that they may continue their recovery journey.</p><div><p><b>
