Primary Responsibilities:<ul><li>
Provides case management, individual and group skills training, and development, mentoring and crisis intervention as directed by a QMHP and documented in the Individual Service and Support Plan (ISSP).</li><li>
Actively promotes the harmonious delivery of services through coordination of interventions in collaboration with treatment team members and foster providers achieved by ongoing communication and active problem solving around outcomes, barriers, and case observations.</li><li>
Maintains and completes required records, reports, documentation as required by departmental policy and state and federal law.</li><li>
Transports individuals, family members and other supports to/from services.</li><li>

 ABOUT THE OFFICE: Pursuant to Article 18-B of the County Law, the Assigned Counsel Plan (ACP), in consultation with the Presiding Justices of the 1st and 2nd Appellate Divisions, has been providing quality legal services to indigent persons since 1966. All services provided by the Assigned Counsel Plan are funded by the New York State Office of Indigent Legal Services and the City of New York through the Office of Criminal Justice. The Court Liaison assists and advocates for adults living with co-occurring mental health and substance use disorder who are involved with the criminal justice system. They increase coordination and collaboration between the criminal justice and mental health systems, improve access to mental health and substance use services, and enhance continuity of care. Additionally, the court liaison assists individuals with court-mandated/recommended treatment, acting as a bridge between the courts, treatment providers, and the client. Key responsibilities include assessing clients' needs, monitoring their compliance with court orders, providing case management, and ensuring communication between all parties involved, such as judges, lawyers, probation and treatment providers. They simplify the process for the client by explaining requirements, assisting with paperwork, and preparing them for court appearances. 
PRINCIPAL DUTIES AND RESPONSIBILITIES:

<li>Maintain a caseload of clients offering case management services and assistance with any legal or CPS concerns.</li>

The Role:
This position is critical to the LEAD Project, by providing engagement and intensive case management services to individuals suspected of low-level drug offenses and/or prostitution. The Case Manager will provide direct services to an active case load of approximately 25-30 individuals. Case managers provide outreach, long-term engagement and supportive services for participants through intensive case management activities and collaboration with LEAD partners, service providers, housing providers and other community organizations.

Develop and maintain collaborative relationships with LEAD partners.</li><li>
Provide advocacy and support for participants within the criminal justice system including court appearances and written communication.</li><li>
Advocate for the participant with a wide variety of other service providers.</li></ul>
Additional responsibilities may include:<ul><li>
Representing Ideal Option and the LEAD Project at community meetings, attending events, conducting outreach, and building awareness.</li><li>
Develop relationships with existing and potential community partners, referring providers, potential patients, and other stakeholders with direct outreach, field marketing, attendance at community events, meetings, workshops.</li><li>
Work at various partner sites throughout the area each week to raise awareness for Ideal Option&rsquo;s services and guide staff members and patients through virtual intake and provider visits.</li><li>
Provide education and training to community-based organizations and providers about LEAD, Ideal Option, medication-assisted treatment, and substance use disorder to broaden acceptance of addiction as a chronic disease and continue to reduce stigma and misconceptions about the disease.</li></ul>
Our ideal candidate has:<ul><li>
Experience serving populations impacted by extreme poverty, behavioral health conditions, and mental illness from a harm reduction model of care.</li><li>
Experience supporting and advocating for participants within the criminal justice system including court appearances and written communication.</li><li>
Street outreach experience providing services and resources to the populations we serve.</li><li>
Ability to advocate and effectively communicate and problem-solve under pressure in high-stress situations.</li><li>
Must live in the greater area and be familiar with resources.</li><li>
The ability to respectfully engage and develop a working alliance with the people we are serving.</li></ul>
Requirements:

Minimum Requirements:<ul><li>
An undergraduate degree related to social services or behavioral health.</li><li>
Experience working with communities involved in street-based sex work.</li><li>
1-2 years working with people in recovery providing guidance and support</li><li>
Experience working with a CRM system</li><li>
Experience with an EHR system and HIPAA compliance</li><li>
Certified Recovery Mentor Certificate or Peer Support Specialist Certification</li></ul></div>
