ABOUT THE JOB</b><br>
Alongside a team of other case managers, housing locators and community providers, the <b>Case Manager MSW</b> provides field-based, direct client outreach, linkage and retention services to people experiencing homelessness. Case management services aim to promote recovery and community integration and ultimately help clients break the cycle of homelessness. Services provided by the Case Manager include, but are not limited to, individual and group therapy, outreach and engagement, case assessment and service planning, assistance with mainstream entitlement, PHA subsidy and other county subsidy applications, housing location, advocacy and addressing health, mental health and substance use barriers. The Case Manager delivers these services by implementing evidence-based practices such as ACT (Assertive Community Treatment) and Motivational Interviewing to help clients meet their service plan goals. Position Responsibilities include:</p>

<li>Perform initial comprehensive biopsychosocial assessment; develop a plan of intervention that addresses identified needs for housing, ongoing case management services, substance abuse treatment, mental health services, and coordination of needed community services.</li>
<li>Collaborate with the client to develop an individualized treatment plan that addresses short- and long-term goals.</li>
<li>Provide ongoing case management support to assess progress and ensure treatment plan outcomes are met or adjusted.</li>
<li>Provide field-based therapeutic supportive case management services addressing mental health, functional, and substance use barriers as needed to help clients obtain and maintain housing.</li>

VASH Case Manager, MSW - (JR 5342)

<p></p><div><p>The Mental Health Coordinator position works under a judicial officer&rsquo;s supervision to provide exceptional customer service for individuals who have been civilly committed to the Court; to provide support with treatment providers, hospitals, social services, ADAMHS, and other behavioral health professionals; to coordinate with law enforcement officers and other justice partners; and to provide other case management support for the Magistrates and Judge. This position reports to an assigned Magistrate.</p></div><p></p>
<div><h2 class="jobSectionHeader"><b>Summary of Job Duties</b></h2></div>
<p></p><div><p>Monitors mental health (civil commitment) cases, in coordination and collaboration with Magistrate, case management specialist, hospitals, outpatient treatment providers, social workers, law enforcement officers, and ADAMHS, and in compliance with Ohio Revised Code, Ohio Rules of Civil Procedure, and other legal authority.</p><p>
Maintains contact with hospitals, outpatient treatment providers, family members, ADAMHS, law enforcement officers, NAMI and other agencies regarding status of individuals on Assisted Outpatient Status (AOT) status; provides detailed instructions and information regarding the AOT process as needed.</p><p>
Reviews AOT Referral Forms forwarded to the Court by the Board; handles preparation of AOT forms for review conferences, including AOT Agreement Forms, AOT Handbooks, HIPAA compliance forms, and information releases; reviews treatment plans, incident reports, and status reports and prepares the Court report for each AOT review conference; attends AOT review conferences and prepares real-time notices for individuals regarding their next conference.</p><p>
Manages fillings and workflow by utilizing the case management system and maintains accurate hard-copy records for mental health cases; tracks discharge notices and outpatient commitment notifications; monitors treatment settings for committed individuals and contacts the board regarding necessary motions for more/less restrictive settings; notifies and follows up with outpatient providers regarding the filing of intake appointment reports, treatment plans, and thirty-day status reports; receives reports from providers prior to scheduled review conferences and reviews for completeness, consistency, and appropriateness; prepares orders to submit required documentation when said documents are not received.</p><p>
Organizes and facilitates the Mental Health Collaboration and Innovation Team meetings including setting bi-monthly meetings of the stakeholders, communicating with stakeholders, finalizing agendas, reserving facilities and catering meals for meetings.</p><p>
Maintains the Court&rsquo;s mental health calendar regarding the scheduling of AOT conferences, status conferences, status hearings, and other court activities.</p><p>
Fields communications from law enforcement agencies and hospitals regarding current commitment status of individuals and coordinates with agencies and ADAMHS boards to develop an appropriate response.</p><p>
Organizes virtual review conferences with state hospitals for Montgomery County residents returning (discharged) for outpatient commitment; works with state hospitals, ADAMHS other county probate courts and outpatient treatment providers to transition individual back into the community and onto outpatient status.</p><p>
Notifies prosecutor, defense attorneys, and or criminal court regarding changes in commitment status, change to voluntary status, and/or termination of commitment for individuals committed through the process outlined in O.R.C. 2945.38.</p><p>
Receives and enters information regarding AOT into the Court&rsquo;s databases; tracks incident reports, especially hospitalizations and police interactions; notifies the Court when an individual had a disruption in their life that may lead to nonadherence; convenes with outpatient providers, Magistrate and Justice Coordinator to develop appropriate interventions for individuals who are nonadherent to treatment and/or medication.</p><p>
Coordinates with attorneys on the Court&rsquo;s list of appointees to provide counsel for indigent respondents for hearings; notifies provider agencies regarding conferences and hearings; prepares and distributes hearing schedules; coordinates, prepares and distributes notices for hearings; coordinates with court investigators when personal service is needed.</p><p>
Informs Magistrate of requests for emergency evaluation; researches and provides detailed information regarding the individual&rsquo;s status; prepares conveyance orders, orders for evaluation and other related orders; coordinates with law enforcement and hospitals regarding transport and evaluation of individuals.</p><p>
Collects significant data to assist the Court in the decision-making process when reviewing an individual&rsquo;s probate status and contributes to ongoing assessment of level of participation in services and risk of harm; works with agencies, ADAMHS, and Magistrate on a case-by-case basis to determine when cases are appropriate for termination and/or graduation from the AOT program.</p><p>
Advises agencies regarding upcoming deadlines for applications for continued commitments on individuals; prepares and sends reminder letters to the ADAMHS board one month before said applications are due.</p><p>
Ensures that the civil rights of all committed individuals are protected; accommodates requests by individuals to avoid stressors and triggers; handles requests for full hearings on continued commitments made by individuals; follows Ohio Revised Code regarding confidentiality and privacy of committed individuals.</p><p>
Provides quality assurance and reporting services for the Court regarding all mental health cases, including, but not limited to, preparation of standard and non-standard reports and ensures that all information in the Court system database is complete and accurate.</p><p>
Participates in the Assisted Outpatient Steering Committee meetings, Treatment Advisory Meetings, and other appropriate community groups and organizations; provides updates and suggestions as needed.</p><p>
Conducts an annual review of all open mental illness cases to ensure compliance with statutory requirements set forth in the Ohio Revised Code, Ohio Rules of Superintendence, and other legal authority.</p><p>
Aides colleagues in the Court when necessary due to scheduling, volume, or staffing requirements, including open mental health cases when the Case Management Specialist is unavailable.</p><p>

JOB DEFINITION:</b></p><p>
In conjunction with the HOST Project Manager and Senior Program Manager, the HOST Mental Health Clinical Supervisor specifically oversees the acceptance and assignment/facilitation of referrals to the HOST program. In conjunction with the HOST Project Manager, this position oversees all aspects of outreach and engagement, short-term case management services, as well as contractual obligations. HOST Supervisors collaborate internally and externally to promote continuity of care for our clients. The supervisors are expected to coordinate with internal outpatient teams, such as SAGE, and other PHP providers to assure successful transition of consumers to on-going care. The HOST Clinical Supervisors will have education and experience as a Mental Health Professional.</p><p><b>
MAJOR DUTIES AND RESPONSIBILITIES:</b></p><ul><li>
Assist HOST Project Manager in planning, implementation and coordination of all HOST services and other relevant services at the agency.</li><li>
Assist in Screening/assessing referrals to HOST Program for eligibility and provide coordination of other HOST staff involved in screening and initial engagement effort.</li><li>
Coordinate and participate in psychiatric consultations, clinical supervision meetings, program meetings, and in-service training of mental health program staff; participate in clinical reviews and case conferences.</li><li>
Assess staff members supervised for strengths and training needs. Provide regular supervision, clinical consultation, performance feedback, support and assurance of needed training for staff.</li><li>
Facilitate team meetings in a manner, which promotes team members contributing in their areas of strength and learning as needed. Develop team cohesion, which effectively serves the clientele and provides the needed support for team members to effectively perform their job.</li><li>
Coordinate and schedule for all aspects of HOST services, including scheduling of individual team members and coverage of offices as needed; review and authorize staff time sheets; submit on time to payroll department.</li><li>
Guide and participate in appropriate delivery of screening/assessment activities to assure resources are used for the target population.</li><li>
Review team clinical documentation to assure that team members maintain current and complete clinical records, and that records comply with the agency, state, and county clinical accountability policies and procedures.</li><li>
Provide individual supervision/consultation for assigned team members.</li><li>
In cooperation with agency management and HOST staff, establish and maintain cooperative relationships with a broad spectrum of human service organizations that provide resources needed by mental health consumers.</li><li>
Provide clinical outreach &amp; case management services to a partial caseload.</li><li>
Become and stay aware of related contractual and working agreements; assure supervised staff awareness of same.</li><li>
Collaborate with non-mental health DESC staff to ensure continuity of care for consumers using other DESC services.</li><li>
Assist Clinical Programs Entry Services Manager in coordination of services and activities, which span the mental health department and other DESC programs.</li><li>
Assure the gathering of accurate service and other program data; assist with report writing in compliance with HOST/PATH/City of Seattle contracts.</li></ul><p><b>

Eligible for a Licensed AAC credential or any other superseding credential that meets RCW 71.05.020 requirements to act as a Mental Health Professional whose scope of practice includes independently conducting mental health assessments and making mental health diagnoses.</li><li>
Master's degree in a social work, psychology or other relevant behavioral science or Bachelor of Nursing degree with specialty in mental health.</li><li>
One or more years' experience in community based behavioral health services.</li><li>

Experience working with adults experiencing homelessness, mental illness and/or substance use disorders.</li><li>

THE OPPORTUNITY</b></p><p>
Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant&rsquo;s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling.</p><p>
BJI seeks a Case Manager for the borough&rsquo;s Supervised Release Program (SRP) Brownsville/East New York office. Reporting to Supervising Case Manager, Case Manager is the front line of service delivery and key to ensuring that the program meets its mandate. Specifically, the Case Manager will supervise SRP participants according to their assigned tier and level; work with participants to identify and address barriers to compliance and court attendance; conduct outreach to participants, court stakeholders, community-based service providers, and collateral contacts; connect participants to appropriate services in the community to support their stabilization, and ensure accurate and timely court reporting on participants&rsquo; compliance with program requirements. Accurate and timely data entry is a core responsibility for this position. Additionally, the Case Manager will co-facilitate pro-social and psycho-educational groups. Please note that this role is not eligible for clinical hours.</p><p></p><p><b>

Qualifications:</b></p><ul><li>

Must be detail-oriented with strong organizational and time management skills;</li><li>
