Job description: We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support. Your expertise in managed care, patient assessment, and medical terminology will be essential in ensuring the well-being of our patients. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care
