Windom Area Health is seeking a dedicated Clinical Nurse Coordinator to join our team. The Clinical Nurse Coordinator will be responsible for coordinating our OB/GYN clinic, ensuring optimal patient care coordination, and prior authorization submissions. This role also involves providing direct patient care and physician support, including rooming patients, assisting with procedures for in-person visits, and assisting with telemedicine visits. Scheduling would cover weekday shifts as needed, with scheduled days varying based on staffing availability.
Qualifications:<ul><li>
Current RN license in the State of Minnesota with a minimum of 3 years of clinical nursing experience.</li><li>
Clinic experience is preferred.</li><li>
Experience in managing or coordinating clinic operations is a plus.</li><li>
Basic Life Support (BLS) certification.</li></ul>

Job Summary and Qualifications</h5>
The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization&rsquo;s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance. 
As the Clinical Nurse Coordinator(Charge RN) for Medical Oncology unit, you will be collaborating with the director to lead a team of amazing medical professionals. This is a great opportunity to take your career to the next level! 
What you should expect to do in this role: 
<ul><li>
You will collaborate with the Director to provide leadership to our team and play a role in fiscal, human resource, and operational functions</li><li>
Your role will actively participate in department and hospital strategic planning and will effectively implement new programs and services</li><li>
You will be maintaining an effective competency and staff development program</li><li>
You will work on day-to-day operations such as: processing payroll, managing department expenses, participating on hospital committees, processing employee evaluations, managing personnel issues</li><li>
You will be assisting in maintaining a departmental staffing plan that meets the needs of the customer and is responsive to the changing census</li><li>
You will assure an appropriate skill mix and staffing levels in accordance with volume demands</li><li>
You will create/foster a positive environment and will lead by example and by holding staff accountable for their behaviors and attitudes (compassionate, professional, communicative)</li></ul> 
What qualifications you will need for this role: 
<ul><li>
 Current Colorado Registered Nurse License or valid compact license</li><li>
Current BLS, American Heart Association or American Red Cross, required</li><li>
Bachelor Science Nursing or actively pursuing with completion within 3 years of hire required</li><li>
Graduate of an accredited school of nursing required</li><li>
Minimum of 2 years clinical RN experience required</li><li>
Two years progressive supervisory or charge nurse experience preferred</li></ul>

Job Summary:

The MDS / Care Plan Coordinator is responsible for assuring the timely completion of accurate resident assessments and interdisciplinary care plans that meet Federal and State guidelines. This includes identifying resident acuity and needs, helping to determine specific care needs, and communicating needs and expectations to families and responsible parties. 


Essential Duties:
<ul><li> Responsible for integrating nursing, dietary, social recreation, restorative, rehabilitation, and physician services to ensure appropriate assessment and reimbursement.</li></ul>
<ul><li> Responsible for completion of all Medicare/Medicaid case-mix documents to assure appropriate reimbursement for care and services provided within the Facility.</li></ul>
<ul><li> Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category.</li></ul>
<ul><li> Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission.</li></ul>
