The Department of Emergency Medicine at the University of Washington invites applications for three (3) teaching faculty positions at the rank of Teaching Associates, with an anticipated start date of September 1, 2026, or thereafter. Visits are on-demand, low-barrier and synchronous telehealth visits using video or audio-only technology. Interpreter services are available for patients who do not speak English as their primary language. This position will work closely with our Linkage to Care Coordinators (LTCC&rsquo;s) that help link patients to longitudinal care in their area. This role centers on ensuring that every patient encounter receives efficient, accessible, high quality, respectful and responsive health care for OUD.

Program Overview

The Advanced Care Partner Program creates a team-care structure to collaborate with PCPs who are managing complex patient panels.

The ACP &quot;care team&quot; model pairs a highly clinically experienced provider (ACP) with a PCP to collaborate closely by sharing the care and continuity management of the patient panel. The PCP's task queue provides the focus of this collaborative platform whereby the Advanced Care Partner devotes their work to keeping all categories within SLAs, provides real time responses to patient messages, follows up with chronic care monitoring and management, and intimately understands the patient panel &quot;as their own&quot;, such that complex patients are known well, medical issues are managed efficiently, highest quality conversations engage patients' trust, and continuity of care between the patient and two providers is seamless and exemplary. Further, the ACP care-team model promotes career-sustainability, mitigates the potential for decision fatigue, prevents practice isolation, and addresses burn-out risk factors. The program prioritizes well-being and belonging for both patients and PCPs.

Advanced Care Partner Role:

The Advanced Care Partner is a full-time, remote clinician who works business hours live online with a team of PCP peers, a clinical supervisor, and the Clinical Case Management team of Care Navigators and nurses, as needed, for highly complex cases. The ACP will partner with approximately 5 PCPs and their patient panels, owning these shared panels with longitudinal accountability over time.

The ACP is responsible for maintaining task category SLAs, including but not limited to:
<ul>
<li>Answering clinical questions from patients and engaging in clinical dialogue to complete these conversations.</li>
<li>Clinical decision-making resulting from information coming from messages, lab results, and consults.</li>
<li>Owning medication changes, titrations, and adjustments for chronic conditions as needed.</li>
<li>Providing video visits and phone follow-up care as needed.</li>
<li>Completing clinical paperwork such as disability, FMLA, home health sign-offs.</li>
<li>Attention to closing patient care gaps, creating orders and follow up as needed</li>
<li>Processing Consults and providing related follow up management/care as needed.</li>
<li>Referrals to and collaboration with Clinical Case Management.</li>
<li>Providing urgent visit patient scheduling as needed.</li>
<li>Providing continuity of care longitudinally.</li>
<li>Participating as an active member of the care team including real-time Slack room responses, attending huddles, presenting cases, providing support to CCM team members.</li>

<div>This is a unique part-time opportunity to join the Providence Swedish Cancer Institute (PSCI) Colorectal Oncology team at our First Hill and Edmonds campus. This role is ideal for an Advanced Practice Clinician seeking meaningful, specialized cancer care in a highly collaborative academic-style environment, while maintaining part-time flexibility.
Providence Swedish Cancer Institute is seeking a dedicated Advanced Practice Clinician (PA/ARNP) to support our growing Colorectal Oncology program. This position plays an integral role in delivering high-quality, patient-centered care to a complex oncology population, working in close partnership with internationally recognized colorectal surgeons and a multidisciplinary cancer care team.
About the Role:
In this role, the APC will support both inpatient and outpatient aspects of colorectal surgical oncology care. Responsibilities may include inpatient rounding, perioperative management, post-operative care, clinic follow-ups, care coordination, and patient education. The position offers strong physician collaboration, mentorship, and the opportunity to develop deep expertise in colorectal cancer care within one of the region&rsquo;s leading cancer programs.
This part-time position is based at Providence Swedish&rsquo;s First Hill campus in Seattle and is structured to support work-life balance while remaining clinically impactful.

Responsibilities:

The Advanced Practice Provider (APP) provides care for blood and marrow transplant patients and immunotherapy patients in both the inpatient and outpatient settings under the supervision of the attending physician. The APP is responsible for the primary care of the patient to include assessment, triage and management of the transplant patient as part of a collaborative transplant team. Responsibilities include obtain medical history, perform physical examinations, review and order diagnostic studies, radiographs and prescribe treatments. APPs perform bone marrow aspirations, lumbar punctures and are first assists with Bone Marrow Harvests and provide follow-up care of bone marrow donors under the supervision of attending physician.</div>
Qualifications:
<div>
Required:</div><ul><li>
Must be a graduate of a Physician Assistant training program or appropriate Nurse Practitioner program and be able to practice in the state of Washington (licensed).</li><li>
Have National Certification as appropriate for profession</li><li>
Must be licensed for schedule II drug authority at time of credentialing</li><li>
Strong oral and written communication skills required.</li><li>
Basic computer skills are essential.</li><li>
Must be able to work as part of a multi-disciplinary team.</li><li>
Must enjoy long-term relationships with patients undergoing intense therapy.</li></ul><div>
Preferred:</div><ul><li>
Oncology or BMT experience preferred.</li></ul><div>

The annual base salary range for this position is from $126,984 to $200,678, and pay offered will be based on experience and qualifications.
