JOB PURPOSE/SUMMARY

Summary of role of team:
<ul><li>Works within the Children's CSU at East Tennessee Children's Hospital (ETCH), as well as the larger Children's Crisis Continuum.
</li><li>This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.</li><li>
This position requires utilizing a personal dependable vehicle to conduct Center business.
</li><li>Maintaining a dependable vehicle and certified driver status is a condition of employment.
</li><li>CPR and First Aid Certification required.
</li><li>Mandatory to remain awake and alert during shift.</li></ul>

Summary of position:
<ul><li>Serves as a clinical member of the multi-disciplinary team at the Children's CSU housed at East Tennessee Children's Hospital (ETCH), providing treatment, rehabilitation, and support for client population.
</li><li>The CSU RN must be adept at multi-tasking.
</li><li>Provides direct medical care to clients in a CSU setting including clients in varying levels of crisis, frequently working with SPMI and clients with co-occurring issues.
</li><li>Assists with crisis interventions and medical emergencies, obtains vital signs each shift or more frequently as indicated. Administers intramuscular injections, communicate with PCPs during recovery partnership and transition phases, and liaison with other agencies as indicated.
</li><li>RN is responsible for triaging referrals, completing RN intake/assessments on all clients admitted during their shift, obtaining urine drug screens, BAL and pregnancy tests on admissions, obtaining admission orders, dispensing routine and PRN medications, taking orders from prescriber and updating MAR, completing electronic and paper charting in a timely manner and participating in treatment team meetings.
</li><li>Additionally, RN may take calls from recently discharged clients requesting bridge prescriptions, follow-up calls on weekends, obtain prior authorizations and coordinate with pharmacies.
</li><li>RN collaborates with Mobile Crisis and Family Walk in Center assisting with clients being referred to higher level of care, completing S-BARs.
</li><li>Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
</li><li>Schedule may require 12 hours shifts and rotating weekends.
</li><li>Regular attendance is an essential job function.

TYPICAL WORKING CONDITIONS/ENVIRONMENT
<ul><li>
This position will be housed at East Tennessee Children's Hospital (ETCH) where the Children's CSU will be operating.
</li><li>Staff will be expected to work in and create a therapeutic environment while meeting the medical and facility requirements for operating a CSU.</li></ul>

1. Daily Staff Duties
<ul><li>
Performs the initial admission medical assessment and completes required admission paperwork including nursing assessment, nursing note and nursing admission orders.</li><li>
Update medication tabs.</li><li>
Update allergy lists.</li><li>
Complete paper MAR.</li><li>
Obtain urine drug screen, BAL and pregnancy tests upon admission.</li><li>
Follows all established protocols and guidelines for admission prior to admitting the client to the unit.</li><li>
Makes the determination of the appropriate admission protocols needed for each client (ex.-agitation protocol, diabetic protocol, HTN protocol).</li><li>
Obtains admission orders from the on-call prescriber in a timely manner.</li><li>
Completes charting for each client during every shift prior to the end of shift.</li><li>
Completes transition paperwork and discusses medication and aftercare instructions with client and/or family. Provides copy of paperwork for clients.</li><li>
Counts narcotics and documents at every shift change. Notify supervisor immediately of any discrepancies.</li><li>
Obtains and charts vital signs on every shift.</li><li>
Monitor for significant changes in client's condition/behavior every shift and make referrals to obtain new orders as needed.</li><li>
Administer scheduled, routine, and PRN medication in a timely manner.</li><li>
Obtains blood sugars as ordered.</li><li>
Triages potential referrals with Mobile Crisis and Walk in Center.</li><li>
Coordinates arrival times for potential clients with Mobile Crisis, Walk in Center and Transporters</li><li>
Complete C-SSRS on each client</li><li>
Attend to guest and/or client needs during medical emergencies (ie: seizures, falls, etc)</li><li>
Coordinate with Emergency Departments and EMS</li><li>
Obtain Prior Authorizations</li><li>
Answer phone calls from recently transitioned guests regarding medication issues</li><li>
Weekly medication counts of routine medications</li><li>
Update Admission/Discharge log each time a client is admitted or transitioned from unit.</li><li>
Complete follow-up calls on weekends in accordance to program requirements.</li></ul>

2. Team Building
<ul><li>
Must be punctual and maintain a good attendance record.</li><li>
Participates in daily treatment team meetings.</li><li>
Participate in shift reports.</li><li>
Coordinate transitions with other staff.</li><li>
Maintains a positive and professional working relationship with all staff members and supervisors.</li><li>
Communicates and approaches conflict in a problem solving manner with staff and supervisors.</li><li>
Accepts additional assignments and/or changes in assignments to meet the needs of Crisis Services.</li><li>

Starting salary for this position is approximately $31.51 /hr based on relevant experience and education.

Schedule:
<ul><li>The Children's Crisis Stabilization Unit (CSU) will operate 24/7, 365 days a year.
</li><li>The holder of this position will be expected to work shifts that meet the needs of the program operation, which may include day, night, weekend, and holiday hours.
