POSITION PURPOSE:
Following the guidelines of the Illinois Nursing Act and under the direction of the office physician or office Registered Nurse, provides professional nursing care for office patients. Working as a member of the medical office health care team, promotes wellness and quality patient care.
ESSENTIAL FUNCTIONS:<ul><li>
Gathers data by physical assessment of persons with a predictable outcome for the physician; documents in chart or EMR if applicable.</li><li>
Reports problems based on assessment to the physician.</li><li>
Suggests nursing actions that can prevent, reduce, or eliminate health problems to the physician.</li><li>
Performs basic nursing care under the direction of the physician or registered nurse, and documents in chart or EMR if applicable.</li><li>
Shares observations of the progress of the patient reaching established goals with the physician.</li><li>
Instructs patient and family regarding medication and treatment directions for the physician.</li><li>
May chart on behalf of the provider when in the exam room. Chart note to include name, title, date and time of the entry. Chart must then be signed off on by the provider.</li><li>
Assigns ICD-10 codes on charge sheets or in EMR for billing office and monitors CPT coding to assure proper documentation exists for the physician.</li><li>
Performs other duties as assigned by supervisor.</li></ul>
SPECIALIZED KNOW-HOW &amp; REQUIREMENTS:<ul><li>
Current Illinois Practical Nursing Licensure required.</li><li>
Previous Medical Office experience preferred.</li><li>
BLS required within 30 days of hire and must be maintained throughout employment at CGH.</li><li>
Thorough knowledge of medical equipment and instruments is strongly preferred.</li><li>
Prior knowledge in drawing blood preferred.</li><li>
Basic computer knowledge preferred.</li><li>
Demonstrates ability to document patient needs and assessments.</li><li>
Demonstrates knowledge necessary to provide care appropriate to any age-related needs of the patients served in the physician office. Demonstrates knowledge of the principles of growth and development appropriate to the patient population served. Able to assess and interpret data about the patient&rsquo;s status in order to identify each patient&rsquo;s needs and provide the appropriate care, including age-related care, needed by the pediatric, adolescent and/or geriatric patient group.</li></ul>

STAFF RN-ACUTE CARE-SPLIT SHIFT

<div>The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include, but are not limited to the following:

<li>Provide skilled nursing care under the direct supervision of the Clinical Supervisor</li>
<li>Complete evaluation tasks, including vital signs and medication reviews</li>
<li>Coordinate with physicians, therapists, and other individuals in the client&rsquo;s care plan</li>
<li>Assess and chart observations of the client&rsquo;s condition at each visit</li>
<li>Maintain complete and accurate documentation of nursing care</li>
<li>Work collaboratively with all members of the healthcare team</li>
<li>Provide education to clients and families on proper home health care procedures and strategies</li>
<li>Record patients' medical information and vital signs.</li>
<li>Monitor, record, and report symptoms or changes in patients' conditions.</li>
<li>Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans</li>
<li>Modify patient treatment plans as indicated by patients' responses and conditions</li>
<li>Monitor all aspects of patient care, including diet and physical activity</li>
<li>Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems</li>
<li>Perform other duties as assigned</li>

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
<ul>
<li>Associate degree required; Graduate of an accredited state-approved, Bachelor&rsquo;s Degree from the school of nursing preferred</li>
<li>Current and valid Connecticut Licensed Practical Nurse license</li>
<li>One year of LPN experience, Current CPR certification (required), Driver&rsquo;s License, Car Insurance, Car Registration</li>
<li>Strong interpersonal skills that build personal rapport and establish trust with the client</li>
<li>Computer literate</li>
<li>Strong organizational skills</li>
<li>Excellent written and communication skills</li>
<li>Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)</li>
<li>Home health care experience</li>
<li>Bilingual (Spanish) preferred</li>
