  What You Will Do

   Lead and manage the Medication Assisted Treatment (MAT) program and clinical provider scheduling
  </li>
  <li>
   Conduct patient care activities including chronic care clinics and infirmary rounds
  </li>
  <li>
   Ensure proper medical documentation and adherence to formulary and treatment guidelines
  </li>
  <li>
   Review clinical practices and participate in committees and best practice projects
  </li>
  <li>
   Implement and update clinical policies, collaborating with medical and administrative teams

Job Responsibilities:</b></p><ul><li>
Under the direction of a case management supervisor, the position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives; obtains in-depth social histories, develops and assures community and/or agency resources to meet the consumer's needs; serves as QIDP and provides monthly monitoring of the program.</li><li>
This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.</li></ul><p>
