<div><b>Description</b><p>
The staff Occupational Therapist is responsible for evaluating and treating patients and facilitating rehabilitation and independence. Develops and designs treatment plans for physically, mentally and/or emotionally disabled patients. Occupational Therapists enable, enhance and improve patients&rsquo; ability to gain independent, productive daily living and working environments.</p><b><br>
Responsibilities</b><p>
1. Examines and assesses patients' physical, cognitive and learning abilities to determine realistic attainable rehabilitation goals and plan of care.</p>
<p>
2. Treatment interventions are based on findings of the evaluation and the plan of care.</p>
<p>
3. Selects educational, recreational and social activities that will help individuals learn work and life-management skills within limits of their cognitive and physical capabilities.</p>
<p>
4. Reassesses patients' progress and prepares reports that details progress.</p>
<p>
5. Documents therapy activities and maintains necessary records in patient files.</p>
<p>
6. Provides training and education to caregivers in preparation of discharge.</p>
<p>
7. Recommends changes in patients' work or living environments that is consistent with their needs and capabilities.</p>
<p>
8. Provide documentation of medical necessity for approval of DME/adaptive equipment and ongoing treatment.</p>
<p>
9. 1-2 continuing education courses per year</p>
<p>

Qualifications</b><ul>
<li>Bachelor&rsquo;s or Master&rsquo;s degree in Occupational Therapy
</li><li>Graduate of accredited Occupational Therapy Program</li>
<li>New York State Occupational Therapy License</li>
