Under the general supervision of the Oncology practice manager. This is a hybrid position that wil work mostly in the clinic with some remote hours to develop, implement, and organize the navigation of services for Oncology patients. Coordinates diagnostic testing, prescription refills and authorizations, referrals to specialists and community services, and follow up care with medical oncologists. Provide ongoing education and assessment, refer patients to ancillary services as needed, provide supportive listening and engage in end-of-life conversation and preparations, and advocate for patients&rsquo; informed decision making. Develops, implements, and organizations the Navigator position with Oncology Nursing Manager, Oncologists, and referring providers.</div>
Responsibilities:

 Working with the Komen Patient Care Center&rsquo;s team of highly trained professionals, the Patient Navigator provides support to a diverse population in a telehealth patient navigation model. The Patient Care Center powers Komen&rsquo;s mission to save lives by addressing the most critical needs of our communities right now. Through personalized patient support, navigators identify and address barriers to care, ensuring people have access to the care they need to achieve health equity, and helping those with aggressive and deadly breast cancers get the unique support they need.

 The navigator will work directly with people concerned about breast cancer, including patients, caregivers and family members. The navigator will identify and address barriers to facilitate timely access to care throughout the breast cancer continuum and provide emotional support, education on a variety of breast health topics, guidance to navigate the health care system, and information on resources via phone calls/emails/text/chat. The navigator will provide support on a broad range of issues including financial needs, emotional wellbeing, and medical decision-making.

The Patient Navigator manages a variety of front office functions and is key to smooth operation of a dynamic outpatient wound care center as well as performs general office duties to assist the staff of the WCC.<br>

Essential Functions/Responsibilities:</b></h2><ul><li>
Coordinates with Center leadership to maximize daily patient census.</li><li>
Actively participates in staff meetings to support key functions within the Center.

Maintains all patient communication needs including scheduling, rescheduling, and appointment reminders. Also works and schedules for provider, according to the care continuum model.</li><li>
Obtains and verifies patient insurance information, to include pre-certifications and pre-authorizations for services and enters data in appropriate databases.</li><li>
Collects and enters patient charges in databases.
</li><li>Verifies and reconciles charges as directed, prepares monthly patient survey data and appropriate documentation, then transmits to providers.

Coordinates/schedules ancillary testing with other hospital departments.</li><li>
Arranges for patient transportation as needed.
