General Definition and Scope of Job:
The Farmworker Health Outreach worker will be responsible for completing outreach, basic health assessments, health education, referrals, and follow-up to migrant and seasonal farm workers. This position requires flexible hours (evenings and weekends) and a valid NC driver&rsquo;s license. This position is dependent upon grant funding.

Role on the Care Team:
The Farmworker Health workers place within the care team is to provide primary clinical support services to the patient. They contribute in clinical ways (such as taking blood pressure, blood glucose levels, encouraging medical office follow-up as indicated) that the physician may not be able to, considering the locations visited outside of the office.
Duties and Responsibilities:<ul><li>
Works with team to plan and coordinate nightly visits to outreach camps, ensuring that all camps are visited in a timely manner and all necessary follow up visits are completed.</li><li>
Provides direct outreach to farmworker camps and families several evenings each week (6:00-9:30PM). Works with Farmworker and outreach teams (including interns and volunteers) to complete health assessments with farmworkers and their families at farmworker locations to ascertain services they may need.</li><li>
Works with team to disseminate information to farmworkers regarding clinic, local, and state services which respond to farmworker needs.</li><li>
Provides individual client health education, as well as group camp presentations on topics of need and interest to farmworkers. Refers farmworkers to HCCH services as appropriate. Is responsible for leading team education efforts on at least one health education topic per year with the goal of fulfilling grant requirements.</li><li>
Responsible for client case management services (i.e. assessment, referral, follow-up, and re-assessment) for farmworkers and their families in designated portions of the service area (Avery, Watauga, and Caldwell counties) in a timely manner.</li><li>
Transports and interprets for farmworker medical and social service appointments using program vehicle as available. May occasionally need to use personal vehicle and submit request for mileage reimbursement.</li><li>
Documents all farmworker encounters on appropriate forms.</li><li>
Works with team to enter all farmworker encounter data into database on a daily basis including tracking of clinical measures and all related data for UDS reporting.</li><li>
Will be staff or have program staff present with Newland evening clinic operations and interpreting.</li><li>
May supervise and recruit student interns.</li><li>
Works with team to help plan and implement projects (to be decided), including advocacy, community relationship building, and specific projects responsive to farmworker needs.</li><li>
Complies with all High Country Community Health employee requirements (including drug screening prior to hire date).</li><li>
Provides medical interpreter and document translation services to HCCH patients for staff when needed.</li><li>
Provides patient transportation to HCCH patients (and when required, family members)</li><li>
Coordinates farmworker (and their family members when necessary) transportations among outreach team members. When necessary, provides transportation to HCCH patients, farmworkers and their families.</li><li>

ROLE OVERVIEW and PURPOSE:

The Community Health Worker (CHW) is responsible for working effectively with Enhanced Care Management (ECM) members to improve their whole health through outreach and engagement activities, which are primarily field based. GENERAL DESCRIPTION OF DUTIES
The purpose of this position is to support the Agency&rsquo;s needs by providing support based on CRT&rsquo;s Steps to Success model, the Case Manager &ndash; Certified Community Health Worker (CHW) is responsible for providing a wide-range of services; including, but not limited to, outreach, engagement, education, coaching, informal counseling, social support, advocacy, care coordination, and basic screening and assessments of any risks associated with social determinants of Health. Daily monitoring of customer contacts and activities within the Community is a primary responsibility. The Case Manager &ndash; Certified CHW conducts assessments of customer needs and coordinates referrals to Community Renewal Team programs and initiatives, Funder services, entitlement programs, as well as, to other Community resources that conduct Case Management services.
SPECIFIC DUTIES AND RESPONSIBILITIES
ESSENTIAL JOB FUNCTIONS

MINIMUM TRAINING EXPERIENCE AND HEALTH CERTIFICATION

Position Summary
The Homeless Outreach program is a dynamic and unique team. Our staff work autonomously out in the community to assist clients with basic needs, case management and referrals to higher levels of care or community partners. Objectives will be to create supportive relationships with homeless individuals through street outreach and referrals to community partners to prioritize meeting clients where they are at.
This position provides outreach services to Salt Lake City&rsquo;s unsheltered individuals and/or families in order to connect clients with supportive services with the goal to decrease Salt Lake City&rsquo;s unsheltered population and connect individuals with safe and stable housing. Our city care coordination team is made up of three individuals. This team will be provided a work vehicle and will be expected to provide services to unsheltered clients across the city limit. Daily tasks may include providing clients with essential supplies, driving clients to community partner appointments, and assisting clients with obtaining vital documents that will later be essential in the housing process. Care coordinators will have the ability to build rapport with clients to assess housing motivation and add clients to HOP&rsquo;s internal case management referral list. This team is expected to respond and assess community referrals. In this position you will also respond to community concerns, assist with community education, and problem solve around homelessness issues.
Essential Duties?<ul><li>
Proactively engages and initiates contact with clients in accessing basic needs items and community-based resources, to help them overcome homelessness.</li><li>
Prioritizes client engagement in Salt Lake City, identifying priority areas, coordinate needs, and address barriers.
</li><li>Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and refer/coordinate services as needed.</li><li>
Provide direct, Trauma Informed Care to clients experiencing unsheltered homelessness, including crisis intervention, outreach, ongoing assessments (i.e., SPDAT, Lethality Assessment , Crises Response Plans, etc.), creative problem solving, agency and community resource utilization, and network building.</li><li>
Assess medical emergencies, and provides immediate and professional response to any emergency following appropriate policies and procedures. (contacting emergency medical, utilize Naloxone kits, Etc.)</li><li>
Participate in regularly scheduled staff meetings to ensure appropriate case planning and referral of clients to the most appropriate service provider. (i.e. CTG, CE enrollment, clinical staffing, etc.)</li><li>
 Ensuring that all paperwork related to client and program activities is properly completed, managed confidentially and stored in an appropriate location and submitted in a timely manner.</li><li>
Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices.</li><li>
Evaluate own performance to increase effectiveness as a Care Coordinator and maintains willingness to accept supervision and direction.</li><li>
Maintain positive, professional interactions co-workers, and all other community providers including; Homeless Resource Center staff, emergency services, fire, police and sheriff, criminal justice, staff from other community providers, neighborhood residents and businesses, client family members, agency donors, community support groups and referral sources.</li><li>
Manage purchase receipts and submit them in a timely manner.</li><li>
Assist with community education, response, and problem solving around homelessness issues.</li><li>
Works to Identify and refer clients to Case Management when appropriate. Ability to utilize Trauma Informed practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision for major situations.</li><li>
Maintain an active client caseload as assigned by supervisor, assist them in finding housing and self-sufficient resources.</li><li>
