The Outreach Worker works with the CPEP Mobile Crisis Teams (MCT) and directly reports to the Manager, Program Coordinator, and Medical Director of CPEP. As an integral member of MCT, the Outreach Community Coordinator is expected to perform ancillary functions as designated by the team&rsquo;s clinician for example, Social Worker, Nurse Practitioner, or CPEP Manager/Coordinator. The primary purpose of this position is to transport the CPEP staff to patient homes for clinical evaluation, arrange for the initial scheduling for referrals received surrounding psychiatric assessment of patients in the community, billing and insurance authorization, using EPIC, maintain statistical data on a weekly basis, which is inclusive of, but not limited to the gathering of patient demographics for performance improvement projects, completing MAVEN, performance of light clerical support, for example, faxing and copying patient data, arranging for the maintenance and upkeep of the programs vehicles, and assisting the programs clinician in emergent situations which require 911 intervention.</div>
Responsibilities:
<ul><li>Assist the CPEP clinicians in calling for police/EMS back up as needed.
</li><li>Participate with clinicians in violence de-escalation/containment in a team approach, utilizing good communication and social skills within the patient's home.
</li><li>Provides support clerical services (maintains case records) and follows up with appropriate referrals, under the direction of the designated program clinicians.</li><li>
Responsible for adequate maintenance of programs vehicles, including ensuring those vehicles are in conformity with registration and inspections.</li><li>
Completes billing in EPIC in a timely manner and works with insurance companies to obtain the necessary authorizations.</li><li>
Completes MAVEN database information for DOHMH.</li><li>
Works weekend, evenings, and holidays.</li><li>
Attends trainings including PMCS.</li><li>
Completes all hospital requirements including OHS, yearly reorientation, Corporate Compliance, Flu shot or declination form, and other requirements.</li><li>
Maintain a clean driving record.</li><li>
Neither the Hospital nor the Department will be responsible for any DMV violations that are caused by the driver, i.e. speeding, red lights, etc.</li><li>
Participate in all surveys and audits as directed by Director, Manager/Coordinator.</li><li>
Performs other related duties assigned by the Manager/Program Coordinator or other program clinicians. In an emergency, performs other duties necessary for the welfare of patients or the efficient operations of the hospital.</li></ul>
Qualifications:

GENERAL DESCRIPTION OF DUTIES
The purpose of this position is to support the Agency&rsquo;s needs by providing support based on CRT&rsquo;s Steps to Success model, the Case Manager &ndash; Certified Community Health Worker (CHW) is responsible for providing a wide-range of services; including, but not limited to, outreach, engagement, education, coaching, informal counseling, social support, advocacy, care coordination, and basic screening and assessments of any risks associated with social determinants of Health. Daily monitoring of customer contacts and activities within the Community is a primary responsibility. The Case Manager &ndash; Certified CHW conducts assessments of customer needs and coordinates referrals to Community Renewal Team programs and initiatives, Funder services, entitlement programs, as well as, to other Community resources that conduct Case Management services.

Position Summary
The Homeless Outreach program is a dynamic and unique team. Our staff work autonomously out in the community to assist clients with basic needs, case management and referrals to higher levels of care or community partners. Objectives will be to create supportive relationships with homeless individuals through street outreach and referrals to community partners to prioritize meeting clients where they are at.
This position provides outreach services to Salt Lake City&rsquo;s unsheltered individuals and/or families in order to connect clients with supportive services with the goal to decrease Salt Lake City&rsquo;s unsheltered population and connect individuals with safe and stable housing. Our city care coordination team is made up of three individuals. This team will be provided a work vehicle and will be expected to provide services to unsheltered clients across the city limit. Daily tasks may include providing clients with essential supplies, driving clients to community partner appointments, and assisting clients with obtaining vital documents that will later be essential in the housing process. Care coordinators will have the ability to build rapport with clients to assess housing motivation and add clients to HOP&rsquo;s internal case management referral list. This team is expected to respond and assess community referrals. In this position you will also respond to community concerns, assist with community education, and problem solve around homelessness issues.
Essential Duties?<ul><li>
Proactively engages and initiates contact with clients in accessing basic needs items and community-based resources, to help them overcome homelessness.</li><li>
Prioritizes client engagement in Salt Lake City, identifying priority areas, coordinate needs, and address barriers.
</li><li>Identify priority needs, (i.e. pregnant, IV/IM drug user, dual diagnosis, and other medical conditions) and refer/coordinate services as needed.</li><li>
Provide direct, Trauma Informed Care to clients experiencing unsheltered homelessness, including crisis intervention, outreach, ongoing assessments (i.e., SPDAT, Lethality Assessment , Crises Response Plans, etc.), creative problem solving, agency and community resource utilization, and network building.</li><li>
Assess medical emergencies, and provides immediate and professional response to any emergency following appropriate policies and procedures. (contacting emergency medical, utilize Naloxone kits, Etc.)</li><li>
Participate in regularly scheduled staff meetings to ensure appropriate case planning and referral of clients to the most appropriate service provider. (i.e. CTG, CE enrollment, clinical staffing, etc.)</li><li>

Assist in resolving conflicts utilizing de-escalation skills and trauma informed practices.</li><li>

The Outreach Specialist works with programs funded through the PATH Grant and Emergency Room Enhancement (ERE) initiatives to engage individuals who are experiencing homelessness or frequently utilizing emergency medical services due to unmet behavioral health needs. Through outreach, case management, and care coordination, this position assists individuals and families in accessing housing, behavioral health treatment, and community-based resources. Services may be delivered in community settings, homes, hospitals, shelters, or other locations where individuals need support.<div>

Key Responsibilities: 
<ul><li>Conduct community outreach to individuals experiencing homelessness or frequent emergency room utilization due to behavioral health needs</li><li>Build trusting relationships with individuals and families while engaging them in supportive services and treatment opportunities</li><li>Connect clients to housing resources, behavioral health treatment, substance use services, and other essential community supports</li><li>Conduct intake screenings and assessments to determine eligibility for services and appropriate care pathways</li><li>Provide short-term case management and crisis support while assisting individuals in stabilizing and connecting to long-term services</li><li>Assist clients with accessing community resources such as Medicaid, food assistance, emergency shelter, medications, and support groups</li><li>Collaborate with hospitals, community mental health providers, and substance use treatment agencies to coordinate care</li><li>Conduct outreach in community locations including shelters, hospitals, and street outreach environments</li><li>Maintain and manage a caseload of PATH participants and assist with preparation for enrollment in additional community support programs</li><li>Document all services provided and maintain accurate records in accordance with program and regulatory requirements</li><li>Utilize HMIS systems to enroll participants, update referrals, and track program activities</li><li>Participate in team meetings, staff development activities, and ongoing training opportunities</li><li>Respond to crisis situations and assist individuals in accessing immediate support services when necessary</li><li>Build partnerships with community organizations and participate in local meetings addressing homelessness and community support initiatives</li><li>Perform additional duties as assigned by leadership</li></ul></div><div>
Education, Experience, and/or Credential Qualifications: 
<ul><li>Bachelor&rsquo;s degree in Psychology, Sociology, Social Work, Behavioral Health, or another related human services field from an accredited institution</li><li>Knowledge of behavioral health, mental health conditions, and substance use disorders</li><li>Understanding of case management principles and community-based behavioral health services</li><li>Experience working with individuals experiencing homelessness, behavioral health challenges, or substance use disorders preferred</li></ul></div>

<div>We are looking for a dedicated candidate join our team as a Community Health Worker (CHW) to service our patients in the Mt. Pleasant area. The Community Health Worker (CHW) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality-of-service delivery. This role includes visiting patients in their homes and Assisted Living Facilities (ALFs) to facilitate visits with providers and specialty services. Additionally, the CHW is responsible for delivering

Areas of Responsibility:
<ul><li>Work closely with care team members and other staff to ensure coordinated patient care and compliance with the care plan.</li>

<li>Facilitate and coordinate visits with PT&rsquo;s, OT&rsquo;s and Providers in Assisted Living Facilities and patients' homes to enhance patient care and support.</li>
<li>Deliver medications to patients&rsquo; homes, review instructions, and perform medication reconciliation, ensuring accuracy between current medication lists and what is found in the home.</li>
<li>Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers, ensuring patients receive timely care.</li>
<li>Set up training, instruct patients on the use of Remote Patient Monitoring (RPM) equipment, 
providing ongoing support, education and troubleshooting, when necessary.</li>
<li>Deliver durable medical equipment (DME) to patients in homes and ALFs, ensuring proper 
assembly and functionality. Provide training to patients and caregivers on the correct use of 
DME.</li>
<li>Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.</li>
<li>Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.</li>
</ul>
Qualifications:
<ul><li>Certified Medical Assistant, Paramedic, or Licensed Nurse preferred. Certifications or licenses must be in good standing.</li>
<li>Phlebotomy experience preferred. If not experienced in phlebotomy, candidate must be willing to complete training and demonstrate competency in the skill.</li>
<li>Previous experience in public health or home health preferred.</li>
<li>Experience with DME delivery or logistics operations preferred.</li>
<li>Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.</li>
<li>Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.</li>
<li>Ability to read and communicate effectively. Strong written and verbal communication skills.</li>
<li>Basic computer knowledge.</li>
<li>Must be able to work independently with minimal supervision and direction.</li>
<li>Should demonstrate good interpersonal and communication skills under all conditions and circumstances.</li>
<li>Must be able to work flexible hours and travel between offices, facilities, etc.</li>
<li>Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working condition.</li>
