The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities.</p>
<p><b>
ESSENTIAL POSITION FUNCTIONS</b></p>
<ul>
<li>Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act.</li>
<li>Completes history and physical examinations, by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record.</li>
<li>Manages acute and chronic health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications (to the extent permitted by licensure) and or treatments, obtaining consultations or making referrals and coordinating hospice care services.</li>
<li>Reviews clinical information, coordinates and oversees patient care in consultation with the palliative care interdisciplinary team and other healthcare practitioners.</li>
<li>Assures that patient receives appropriate measures to control symptoms through collaboration with interdisciplinary team members.</li>
<li>Reviews and develops protocols for treatment and proposes options for interventions based on the current medical literature.</li>
<li>Consults the collaborating physician or organizational leadership when the patient's plan of care is outside standardized practice and protocols.</li>

<p><b>EDUCATION &amp; QUALIFICATIONS</b></p>

<li>Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.</li>

Establish the palliative plan of care, individualized for each patient, in consultation with the patient, family and other members of the Palliative Care Team.</li><li>
Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care.</li><li>
Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.</li><li>
Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. Order and interpret laboratory and other diagnostic tests within the scope of professional practice. Order treatments and durable medical equipment as indicated.</li><li>
Communicate clinical findings to the Palliative Medical Director as needed, but no less than bi-weekly. Re-assess effectiveness of care plan on a regular basis and modify as needed.</li><li>
Maintain effective working relationships with supportive care team (e.g., Social Worker, Spiritual Care Coordinator), as applicable, and involve team when patient's care plan warrants team participation to achieve desired outcome.</li><li>
Establish and maintain effective working relationships with those contacted in the course of work.</li><li>
Communicate and document clinical findings, treatment plan and care provided in the patient's medical record in a manner consistent with acceptable standards in order to support sound medical practice and reimbursement for services provided.</li><li>
Knowledgeable of Palliative Medicine Quality Measures collection, documentation and reporting.</li></ul>

Essential Functions</b></h2><p></p><ul><li><p>
Provide primary and palliative care services, focusing on symptom management, quality of life, and goal-concordant care</p></li><li><p>
Conduct comprehensive history and physical exams for patients with complex acute, chronic, and serious illnesses</p></li><li><p>
Lead and document goals-of-care discussions, including advance care planning and code status conversations</p></li><li><p>
Facilitate completion and updates of advance directives, POLST/DNR orders, and end-of-life care preferences</p></li><li><p>
Manage pain and non-pain symptoms (e.g., dyspnea, agitation, nausea, anxiety, depression) using pharmacologic and non-pharmacologic approaches</p></li><li><p>
Review abnormal vitals and laboratory findings and implement appropriate interventions or escalate to the attending physician as needed</p></li><li><p>
Evaluate and manage new admissions, ensuring alignment of treatment plans with patient goals and prognosis</p></li><li><p>
Participate in readmission reviews, identifying opportunities to improve symptom control and prevent avoidable hospitalizations</p></li><li><p>
Collaborate with the interdisciplinary team (IDT) weekly to review patient status, prognosis, and care plans</p></li><li><p>
Identify patients appropriate for palliative or hospice referral and facilitate timely transitions of care</p></li><li><p>
Support care transitions (admissions, transfers, discharges) with emphasis on continuity, comfort, and patient-centered goals</p></li><li><p>
Provide education and emotional support to patients and families regarding disease trajectory, prognosis, and end-of-life expectations</p></li><li><p>
Collaborate with multidisciplinary teams including nursing, social work, chaplaincy, and therapy services</p></li><li><p>
Monitor effectiveness of interventions and adjust care plans to maintain comfort, dignity, and physiologic stability</p></li><li><p>
Serve as a clinical resource to facility staff on palliative care principles, symptom management, and end-of-life care</p></li></ul><p></p><h2 class="jobSectionHeader"><b>
Education &amp; Experience</b></h2><p></p><ul><li><p>
Master&rsquo;s (MSN) or Doctorate (DNP) Nurse Practitioner program completion</p></li><li><p>
Experience in primary care, geriatrics, or post-acute care preferred, but not required</p></li><li><p>
Palliative care training and certification provided</p></li></ul><p></p><h2 class="jobSectionHeader"><b>
