Responsibilities:

The principal role of the Advanced Practice Provider within AlignMed Partners is to provide primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. The AlignMed Partners APP works collaboratively with Center Leadership to achieve the center&rsquo;s mission of improving the lives we touch through the delivery of high quality health care and everyday compassion.</div><div></div><div><br>
Distinctively, the Palliative Care APP provides palliative care services to nursing center residents with serious illness in collaboration with the primary care team. Emphasis is on the care of the individual patient and family with a focus on pain and symptom management, psychosocial support, and advance care planning as an individual navigates the spectrum of a life threatening illness. The Palliative Care APP serves as a specialized consultant within the interprofessional team who provides recommendations for the plan of care while incorporating patient goals and preferences. Additionally, the Palliative Care APP provides guidance and education to the patient, family, primary care team, nursing, social work and rehabilitation therapies regarding risks and benefits of relevant interventions in order to facilitate the shared-decision making process.</div><div></div><div><br>
1. Utilizes evidence based practice and standards of care for managing patient care. This process includes:<br>
Assessment of health status, Diagnosis, Interpreting labs and diagnostics, Development and implementation of treatment plan in accordance and collaboration with interdisciplinary care planning process, Actively develops and implements advanced care planning on all residents<br>
Follow-up and evaluation of the patient status as necessary, Completion of all necessary documentation regarding health care of customer in accordance with established policies and procedures and relevant State and Federal regulatory requirements Follows evidence-based practice guidelines</div><div>
2. Provides health education to patients and families<br>
3. Demonstrates continued professional competence. Recognizes the importance of continued learning through: maintenance of current, knowledge by attending continuing education programs, maintenance of certification in compliance with current state law, applying standardized care guidelines in clinical practice<br>
4. Supports the development of nursing staff, in collaboration with the Center Nursing Executive programs. Partners with Center Executive Director and Staff Development Coordinator/ Nurse Practice Educator to identify and meet clinical education priorities. Serves as a clinical expert for nursing staff and provides targeted clinical education when needed<br>
5. Participates in center-based quality initiatives. Actively participates in clinical initiatives including avoiding hospital transfer, ACO initiatives and other quality improvement initiatives 6. Demonstrates quality care and service<br>
Ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals&rsquo; needs and rights<br>
7. Must be able to adhere to Coding and Billing Compliance Program, including timely submission of billable charges<br>

Qualifications:

The Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master&rsquo;s Degree in Nursing and have a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). Will consider other concentrations/specialty tracks based on experience and business need. The Physician Assistant must be a graduate from a program accredited by the ARC-PA and take the PA National</div><div></div><div><br>
Certifying Exam (PANCE) to use the PAC designation. PA state licensure required. NCCPA certification required.</div><div></div><div><br>
A minimum of two years experience working with the adult or geriatric population is preferred.</div><div></div><div><br>

<b>Levine Cancer Institute, </b>a part of Atrium Health, is currently seeking a <i>full-time Oncology Palliative Medicine Advanced Practice Provider</i> to join their existing groundbreaking Department of Supportive Oncology outpatient care team in the Center for Supportive Oncology in Charlotte, NC. This position is occupied by nurse practitioners (NP) and/or physician assistants (PA) who care for patients on a follow-up basis in the clinic as they receive continuous palliative care throughout the course of their cancer treatment and after treatment ends.</p><p></p><p><b>
About the Position:</b></p><ul><li>
Part of a care-team model with each APP collaborating with two or more physicians</li><li>
Established multidisciplinary clinic team including Physicians, APPs, RNs, CMAs, SWs, chaplain and PharmD</li><li>
Full-time outpatient clinic position seeing an average of 8-10 patients per day for follow-up appointments</li><li>
Four 10-hour shifts</li><li>
Teaching opportunities with APP fellows and research opportunities</li><li>
Weekly educational (Ground Rounds, Research) and IDT meetings</li><li>
Collaborates with multidisciplinary team members in Palliative Medicine and seven other departmental clinical sections, including Cancer Rehabilitation, Integrative Oncology, and Psycho-Oncology</li></ul><p></p><p><b>
Position Qualifications:</b></p><ul><li>
Graduation from an accredited NP or PA program. Current license to practice as a NP or PA in North Carolina. Current prescriptive privileges and DEA license required. BLS for healthcare provider from AHA required.<ul><li>
Nurse practitioners are required to have a master&rsquo;s degree or doctorate in nursing practice. Certification as a nurse practitioner from a nationally recognized certifying body required.</li><li>
Physician Assistants with certification from the National Commission on Certification of Physician Assistants preferred. Current or pending CAQ certification preferred.</li></ul></li><li>
1+ years of experience preferred. Either oncology or palliative medicine experience required; oncology palliative medicine experience preferred.</li><li>
Demonstrates appropriate critical thinking and interpersonal skills when working in a fast paced, high acuity work environment. Able to perform thorough history and physical assessment. Able to work with multidisciplinary team.</li><li>
