<li>Schedule initial and follow-up appointments with palliative care providers, ensuring that all necessary medical records, consents, and background information are available at the time of the visit.</li>
<li>Collect, organize, and maintain accurate and up-to-date patient records related to palliative consultations and services, including histories, test results, advanced directives, and care plans. Manage sensitive information in strict compliance with privacy and confidentiality regulations (e.g., HIPAA).</li>
<li>Coordinate and participate in palliative care interdisciplinary group meetings (PC IDG), and case conferences, to relay updates, share observations, and clarify logistical details as warranted.</li>
<li>Assist with patient intake, vitals, and symptom assessments for palliative care visits (in-person or telehealth) as needed.</li>
<li>Document patient interactions accurately in the electronic medical record (EMR).</li>
<li>Support care coordination across the continuum of healthcare and with referral sources.</li>

Fairview Range Medical Center in Hibbing, MN, is seeking a Nurse Practitioner for our Palliative Care Department to join our multi-specialty practice. The Palliative Care Nurse Practitioner provides specialized medical care to patients with serious, chronic, or life-limiting illnesses in the home setting. The NP works collaboratively with physicians, nurses, social workers, and other interdisciplinary team members of our clinic and home care and hospice team to manage symptoms, improve quality of life, discuss goals of care, and support patients and families through complex medical decisions. The NP serves as a clinical resource for home care and hospice staff along with the medical director.</div><div>
Our physicians and advanced practice providers are supported by a strong multi-specialty clinic structure, collaborating with colleagues in pediatrics, internal medicine, otolaryngology, orthopedics, general surgery, ENT, urology, and sports medicine. Consulting physicians are available in neurology, cardiology, and oncology. Our partnership with the University of Minnesota provides access to additional subspecialists for consultations and outreach.</div><div>

Key Responsibilities</b></div><div></div><div><b><br>
Patient Care: </b>Provide expert medical care and emotional support to patients with serious illnesses, focusing on symptom management and improving quality of life, lead goals of care discussions, facilitate advance directive and POLST discussions and support complex decision-making with patients and families.</div><div></div><div><b><br>
Care Coordination: </b>Collaborate with an interdisciplinary team, including physicians, nurses, and social workers, to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families. Identify hospice eligibility and make referrals when appropriate.</div><div></div><div><b><br>
Assessment and Management: </b>Conduct comprehensive patient assessments in the home and clinic setting, diagnose and manage acute and chronic conditions, prescribe medications and therapies/modalities i.e. home care services, provide advanced symptom management (pain, dyspnea, nausea, anxiety, delirium, etc.) and facilitate communication between patients, families, and the healthcare team, and order and interpret diagnostic tests.</div><div></div><div><b><br>
Education: </b>Educate patients and families about disease progression, symptom management and treatment options, and advance care planning, ensuring they are informed and supported throughout their care journey. Serve as a clinical consultant to home care and hospice staff.</div><div></div><div><b><br>
Required Qualifications</b></div><br>

Progressive clinical experience.</li><li>
Master&rsquo;s or Doctoral degree in Advanced Practice Nursing or Physician Assistant Studies.</li><li>
Graduate of an accredited Nurse Practitioner or Physician Assistant program.</li><li>
Current Advanced Practice Registered Nurse (APRN) license from the Minnesota Board of Nursing or current Physician Assistant certification and licensure in Minnesota.</li><li>
DEA registration or eligibility for prescriptive authority</li><li>
