The Outpatient Coordinator is responsible for coordinating non-clinical tasks to support the delivery of case to members who meet case management criteria. Responsibilities include but are not limited to assisting in scheduling medical appointments, durable medical equipment, home health, eligibility/benefit verification, requesting medical records, data entry, and review of medical management reports. The Coordinator, Medical is accountable for outreaching members telephonically to introduce and invite them to Case Management. The Coordinator, Outpatient will maintain high quality and reliable information related to reports, data, cases, and be able to identify and report any discrepancies. Creating and developing a team play approach by being a good communicator and being flexible to the needs of the department.

Essential Duties and Responsibilities include the following:

<li>Assists Outpatient Case Manager(s) with arranging services for members, such has home health, DME, physician appointments, specialist appointments, and transportation.</li>
<li>Follows up with patients to ensure arranged services are received.</li>
<li>Maintains turnaround time compliance in all aspects of the case management Standards of Operation.</li>
<li>Keeps patient and/or family members or other customers informed of care coordination.</li>
<li>Assists other staff and supports the team approach.</li>
<li>Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner.</li>
<li>Demonstrates honesty and integrity in everyday activities.</li>
<li>Protects privacy for both patients and employees; ensuring all personal health information is kept confidential i.e., (sensitive papers, charts, and reports are not in view of the public).</li>
<li>Recognizes when an error has been made and immediately reports to appropriate manager.</li>
<li>Participates in &ldquo;service recovery&rdquo; through follow-up with an upset member, gathering information, and demonstrating empathy.</li>
<li>Treats members and their families with respect and dignity.</li>
<li>Functions as liaison between administration, members, physicians, and other healthcare providers.</li>
<li>Interacts professionally with other healthcare service providers.</li>
<li>Communicates appropriately and clearly to management, co-workers, and physicians.</li>

The pay range for this position at commencement of employment is expected to be between $23 - $25 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

SUMMARY

A Patient Coordinator is trained to act as the first point of contact for our patients where they set the tone for the patient&rsquo;s visit through excellent patient care. This employee will also perform the necessary administrative responsibilities needed to create a smooth check-in/out experience for patients. 

ESSENTIAL DUTIES AND RESPONSIBILITIES
<ul>
<li>Provide exceptional customer service during every patient encounter (in person or via phone). Display a professional attitude, greet patients promptly with a smile, and thank them when they leave</li>
<li>Answer phones (both external and internal); assure prompt, courteous service at all times</li>
<li>Practice urgency at all times with patients&rsquo; time, as well as Doctor&rsquo;s time and schedule</li>
<li>Manage patient flow in the office</li>
<li>Knowledge of common fees charged for common visits and collect correct payments</li>
<li>Complete daily reconciliations / close day / countdown cash drawer</li>
<li>General office duties and cleaning to be assigned by manager</li> 
</ul>
QUALIFICATIONS
<ul>
<li>Ability to interact with all levels of employees in a courteous, professional manner at all times</li>
<li>Desire to gain industry knowledge and training</li>
<li>Demonstrates initiative in accomplishing practice goals</li>

Strong fine motor skills and full use of hands are essential, as the role demands constant grasping, writing/typing, and use of technology. Visual and auditory acuity&mdash;including color, depth, peripheral vision, and the ability to adjust focus&mdash;is required 100% of the time.
