 Chronic Care Staffing (CCS) is seeking a compassionate and detail-oriented
 
 Remote Care Coordinator (CMA/RMA)
 
 to join our chronic care management team. This role is ideal for certified medical assistants who thrive in a virtual care environment and enjoy supporting patients managing chronic conditions.
 
 
 As a Care Coordinator, you will serve as a consistent point of contact for patients, providing outreach, education, care coordination, and documentation ? all within a HIPAA-compliant remote setting.
 
 
 Key Responsibilities

 Conduct monthly Chronic Care Management (CCM) outreach calls and verbal enrollments
 </li>
 <li>
 Perform health risk assessments and assist with Transitional Care Management services
 </li>
 <li>
 Support Remote Patient Monitoring (RPM) programs and chronic condition tracking
 </li>
 <li>
 Communicate with patients and families regarding medications, diagnoses, and care plans
 </li>
 <li>
 Collaborate with physicians and healthcare teams to ensure coordinated care delivery
 </li>
 <li>
 Promote adherence to individualized care plans and patient education goals

 Connect patients with appropriate community resources when needed

 Maintain accurate documentation in EHR systems and meet call volume expectations

 Required Qualifications

 Must live in PST or MST time zone

 Active, unencumbered CMA or RMA certification
 </li>
 <li>
 Current BLS certification
 </li>
 <li>
 Recent experience in a clinical, care coordination, or CCM setting
 </li>

 High proficiency in EHR systems
 </li>
 <li>
 Proficiency in Google Suite
 </li>
 <li>
 Strong clinical reasoning, critical thinking, and problem-solving skills
 </li>
 <li>
 Excellent verbal and written communication skills
 </li>

 Ability to work independently in a HIPAA-compliant home office environment

Work Schedule: Monday - Friday 8:00am - 5:00pm and rotating Saturdays 8:00am-12:00pm
Job Responsibilities:<ul><li>
Perform blood collections by venipuncture and capillary techniques for all age groups</li><li>
Collect specimens for drug screens, paternity tests, alcohol tests etc.</li><li>
Perform data entry of patient information in an accurate and timely manner</li><li>
Process billing information and collect payments when required</li><li>
Prepare all collected specimens for testing and analysis</li><li>
Maintain patient and specimen information logs</li><li>
Provide superior customer service to all patients</li><li>

In lieu of work experience, a Phlebotomy certification or completed training program must come from an accredited agency</li><li>

Phlebotomist will work in either a Patient Service Center or Client office. In this role you will provide exceptional customer service, perform skilled specimen collections and be the face of the company. In addition, you will be provided opportunities for continuous growth within the organization.
Pay Range: $17.50 - $24.50 per hour

Phlebotomist&rsquo;s may be eligible for participation in the PST Incentive Plan, which pays a quarterly bonus based on performance metrics.

Job Summary:
