<p>
Hospital-wide, as a member of a Transport TEAM, provides timely individual patient transport via wheelchair or cart, primarily for designated inpatients and occasional outpatients, transporting patients for designated testing, therapy, treatment, pre or post-operative care or admission/discharge. Shift assignment of patient transporter employees is pre-set by the Patient Transporter, Supervisor, who also performs dispatching duties throughout the day-shift (peak transport times) via cell phone to each TEAM patient transporter. Responsible to coordinate patient transport with assigned charge nurse (inpatient) or technician and then safely and efficiently transport the patient, IV equipment, monitoring equipment, the medical record and oxygen equipment as required with any assigned transport. In addition, job assignments may include stock maintenance/distribution of oxygen tanks and regulators to assigned departments in the hospital.</p><br>

Essential Job Functions</b></p>

<ul><li>States own name and describes mode of transportation being used and destination to patient.</li>
<li>Retrieves patient's nurse if patient has questions regarding tests/therapy where they are being transferred to.</li>
<li>Obtains permission from patient&rsquo;s nurse to transport patient off floor for testing/procedure</li>
</ul></li><li>Provides for appropriate mode of transportation considering the patient's condition, comfort, and any safety factors.
<ul><li>Uses critical thinking skills to determine mode of transport.</li>
<li>Contacts nursing staff with questions related to transport and gets a ticket-to-ride (TTR) signed by registered nurse (RN).</li>
<li>Coordinates tasks when able.</li>
<li>Anticipates problems and follows through.</li>
</ul></li><li>Transfers patient to/from cart, wheelchair, or other transport vehicle according to procedure and is knowledgeable and effectively operates transport equipment.

<li>Alerts staff when patient's bracelet is missing.</li>
</ul></li><li>Completes tasks as assigned through capacity management.</li>
<li>Determines patient code status prior to transport.
<ul><li>Checks Capacity Management for (LH entity)</li>
<li>Reminds staff when (LH entity)</li>
</ul></li><li>Uses appropriate isolation techniques and complies with universal precautions.
<ul><li>According to isolation posting, secures and dons appropriate protective equipment before entering patient's room.</li>
<li>Removes their PPE and follows hand washing protocol before taking patient out of room.</li>
<li>Ensures patient is wearing PPE, when appropriate, before taking patient out of their room.</li>
</ul></li><li>Stores and cleans transport equipment appropriately throughout the day.
<ul><li>Reports broken equipment.</li>
<li>Returns transport equipment to department when able.</li>
</ul></li><li>Assists in coordinating patient trips.

<li>Maintains ongoing shift communication (daylight shift) with the Patient Transporter, Supervisor for patient delays, issues, problems and temporary reassignment as necessary to keep patient throughput efficient</li>
<li>Uses chain of command in the department effectively.</li>
<li>Maintains dignity and confidentiality of all patients encountered at Independence Health, directly or indirectly.</li>
<li>Ensures transport cell phone is working properly, transferring phone to patient transporter on duty or appropriate voice mail direction per Independence Health standards.</li>
<li>Responsible to check in with and assist co-workers with lift help or transport of difficult patients or volume patient load during any down-time period</li>
</ul></li><li>Safely transport of patients within the hospital for testing, procedures, surgery, treatment or admission/discharge
<ul><li>Operate wheelchairs for safe transport of patients within the hospital, following patient safety practices</li>
<li>Operate transport carts for safe transport of patients within the hospital, following patient safety practices</li>
<li>Assist patients onto transport carts or into wheelchairs for in-house transport</li>
<li>Practice infection control procedures, per policy for infectious / isolation patients</li>
<li>Provide assistance for patients who become ill and need immediate attention (vomiting, CPR, etc.)</li>
<li>Coordinate proper transport of the correct inpatient (or outpatient) for assigned testing or procedure, including the correct medical record, as coordinated and assigned by the physician, and/or assigned registered nurse</li>
<li>Maintain patient privacy, dignity and confidentiality throughout transports and hospital stay per policy</li>
<li>Provide positive communication at all times with hospital patients</li>
<li>Keep daily productivity / Capacity Management of all transports and activity.</li>
<li>Coordinate delivery of patients with receiving department, technicians, signing in at required log stations and reporting special needs of patients requiring special attention of the receiving department</li>
</ul></li><li>Safely transports assigned equipment or specimens, as necessary and time allows, with patient transport being the hospital priority.
<ul><li>Assist nursing staff with specimen or special situation equipment transport, as assigned by Patient Transporter, Supervisor</li>
<li>delivery of stock oxygen tanks or regulators to pre-assigned departments</li>
</ul></li><li>Provides in-house direction and customer service support for organizational excellence.
<ul><li>Assist hospital customers/visitors with directional information, as appropriate throughout the hospital throughout shift, as appropriate</li>
<li>Serves as greeter for visitors, patients and employees entering Independence Health. In addition to performing basic customer service and reception duties. Screens all visitors prior to entering and logs visitors in tracking program as appropriate.</li>
<li>Greet and assist patients/visitors as they enter the main entrance of the hospital.</li></ul></li><br>

<li>Must be 18 years of age.</li>
<li>Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment.</li>

Performs a variety of duties involving the transportation of patients throughout the hospital. Assists nursing staff in the physical, lateral transfer of patients before and after patient transport. Collects and delivers items to and from areas including the Labs, Pharmacy, Radiology, Medical Records, S.P.D., Mail Room and patient care areas. Cleans, sanitizes and delivers beds and transport equipment; maintains job activity logs as required.<div></div><div>
I. Major Responsibilities:<br>
1. Escorts ambulatory patients. Transports patient by wheelchair, stretcher or bed. Prior to transport, verifies identification of all patients assigned by checking 2 identifiers and signing off on Safe Patient Transport Forms; notifies clinician if discrepancy on forms. To ensure patient comfort and safety during transport process, monitors and responds to basic patient needs before, during and after transport. Provides appropriate handoff communication when patient is delivered to receiving unit.<br>
2. Assists nursing staff and/or lift tech in moving and positioning of patients using various lift devices in preparation of patient for transport and returning patient to bed. Includes lateral transfer of patients on precaution status that requires Patient Transporter to use personal protection equipment and to follow infection control process per precaution isolation-type.<br>
3. Delivers stretchers, beds, cribs and wheelchairs as needed. Assists in deep-cleaning of all transport equipment on a biannual basis. Sanitizes transport vehicles and ancillary equipment between patient use.<br>
4. Transports patient specimens to and from the Lab, Pathology, and blood products to/from the Blood Bank in accordance with established infection control guidelines. Transports contaminated equipment to Sterile Processing within same infection control guidelines.<br>
5. Delivers other materials such as medical records, pharmaceuticals, X-ray films, sterile items, and miscellaneous reports and items as required.<br>
6. Checks E-cylinders (portable O2 tanks) for content; distributes/exchanges E-cylinders throughout the hospital.<br>
7. Interacts with a web-based dispatch system TDSS via alpha paging for job assignment; maintains individual Transporter Trip Log and other statistical records as required.<br>
8. May participate in Emergency Department triage transport assignment during disaster situations including CODE HELP, assist in transport of trauma patients and transport of deceased victims to the morgue. Guides and assists patients through decontamination unit.<br>

Essential Duties and Responsibilities include the following. Other duties may be assigned.<br>

</p><ul><li>Verifies correct patient identification by utilizing two patient identifiers and comparing the information with the assignment information. In addition to the full patient name, the patient date of birth or Medical Record Number must be verified. If both of the identifiers do not match the patient is not transported, and the discrepancy is reported to the nursing staff and the transportation dispatcher right away.</li>
<li>Ensures the safe, timely and accurate transport of patients to their destination using equipment that is safe and proper for that transport.</li>
<li>Utilizes proper body mechanics when lifting and transferring patients to and from wheelchairs, gurneys, exam tables, chairs and beds.</li>
<li>Demonstrates competency in transporting all specific age groups and cultures.</li>
<li>Verifies and utilizes proper mode of transportation.</li>
<li>Follows standard and isolation precaution guidelines.</li>
<li>Ensures proper oxygen levels in tanks before transporting patients.</li>
<li>Notifies nursing immediately if patient condition changes.</li>
<li>Properly secures IV, drainage tubes, etc. and secures the patient prior to transporting.</li>
<li>Notifies nursing personnel, clinical staff, technicians or receptionists upon arrival of a patient.</li>
<li>Ensure the comfort of the dignity of patients by being attentive; offers blanket, covers patient properly. Only engages in appropriate conversation with patients. Does not transport a patient who is experiencing moderate to severe pain.</li>
<li>Assists nursing in transferring patients to and from bed or chair to wheelchair or gurney. Consults with nursing and obtains assistance when required.</li>
<li>Consults with nursing and obtains assistance.</li>
<li>Notifies dispatcher of any delays or cancellations or changes in status of the transport.</li>
<li>Removes and properly bags soiled linen after each transport is completed</li>
<li>Sanitizes all transport equipment after each use.</li>
<li>Never leaves patient alone in a treatment area or corridor. Always ensures proper hand off to receiving department personnel.</li>
<li>Reports any equipment needing repair.</li>
<li>Maintains confidentiality of patients in accordance with hospital policy.</li>
<li>Collects and returns transportation equipment to proper storage place.</li>
<li>Responds to emergency situations in according to hospital policy.</li>
<li>Transports small equipment or tools between departments.</li>
<li>Other duties as assigned.</li>

Transportation staff will arrive at the unit with the transport vehicle (i.e. stretcher or wheelchair) specified at the time of request, stop at the Nursing station to retrieve chart and/or card, report to patient room, greet the patient by name, identify the patient by armband, introduce themselves to the parents, patient and nursing staff as appropriate. Assist the patient and/or nursing with the physical transfer of the patient onto the vehicle and transport patient to the appointment.
