Patient Care Techs (PCTs) provide personal care; health-related services; and assist with activities of daily living. CNAs function under the direction; instruction; and supervision of nursing staff. PCTs have a professional obligation to participate in committee activities; surveys; improvement initiatives; and continuing education. Telemetry PCTs provide care for primarily adult patients who require cardiac monitoring. They attend to those who are being treated to manage illness and/or recovering from a procedure of surgical intervention.

The Patient Care Technician Lead supports the delivery of high-quality, patient-centered care by performing delegated basic patient care services and providing leadership to Patient Care Technicians (PCTs) within the inpatient care setting. Under the supervision of a Registered Nurse (RN), this role assists with activities of daily living, patient mobility, and vital sign monitoring, while also coordinating onboarding and training efforts for PCT staff. The Patient Care Technician Lead serves as a resource and role model for new and existing team members, contributing to the department's operational excellence and patient experience. 
 
Essential Functions<ul><li>
Provides direct patient care in accordance with established protocols and under the supervision of an RN, including assistance with hygiene, nutrition, mobility, and comfort.</li><li>
Obtains and records patient vital signs, intake and output, weight, and other key data throughout the shift, escalating abnormalities as appropriate.</li><li>
Assists with point-of-care testing and specimen collection within scope of training.</li><li>
Supports patient safety by assisting with fall precautions, monitoring at-risk patients, and participating in hourly rounding.</li><li>
Maintains a clean, organized, and safe patient care environment; stocks supplies and reports maintenance or safety issues.</li><li>
Leads the unit orientation process for new PCTs, including schedule development, progress tracking, and coaching.</li><li>
Coordinates in-service training and collaborates with vendors or subject matter experts to facilitate skill development.</li><li>
Acts as a mentor to PCT staff by modeling professional behavior, teamwork, and adherence to policies.</li><li>

<div>PURPOSE AND SCOPE: 
Supports the mission, core values, and commitment to outstanding patient care at Home Dialysis Services &amp; Dialysis Care Center, LLC (HDS/DCC). 
Ensures full compliance with all HDS/DCC policies and procedures and regulatory requirements at the state and federal level. 
 
The In Center Hemodialysis Patient Care Technician (PCT) delivers safe, high-quality hemodialysis care to patients under the supervision of the RN, helping to enhance patient outcomes and overall treatment experience. 
 
PRIMARY DUTIES / ACTIVITIES:<ul><li>
Entering prescribed treatment into dialysis machine, setting up dialysis machines, dismantling dialysis machine after treatment is completed, performing chemical disinfection of dialysis machines and recording.</li><li>
Observing dialysis machines and RO water system for correct functioning, responding to visual and audible alarms and making appropriate adjustments throughout the dialysis treatment to ensure patient safety.</li><li>
Monitoring vital signs during treatment and taking patient vitals pro and post dialysis; blood pressure, temperature, weight and documenting it on the Hemodialysis Treatment Record</li><li>
Perform vascular access evaluation pre-treatment and report unusual findings to RN immediately</li><li>
Perform cannulation of patient&rsquo;s vascular access or performing catheter care, as allowed by state law.</li><li>
Obtain homeostasis and apply appropriate dressings.</li><li>
Adheres to all facility policies, procedures, and protocols, participates in all required staff meetings.</li><li>
Collects patient data, monitor and document status of vascular accesses, and patient observations.</li><li>
Reviews lab work, collecting or obtaining blood samples and blood culture specimens and prepares package for shipping.</li><li>
Immediately reports abnormal findings or change in status to assigned RN for further evaluation.</li><li>
Communicates all emergencies, patient life-threatening complications, and related situations to the attention of the RN for further instruction.</li><li>
Maintains compliance with safety and infection preventative measures, ensuring patient safety.</li><li>
Reinforces education with patients and family members on all aspects of End Stage Renal Disease.</li><li>
Assists in the management of inventory supplies.</li><li>

PCTs must follow proper body mechanics and procedures for lifting/ repositioning patients without injury. PCTs may face hazards from exposure to chemicals and infectious/ contagious diseases. The work environment is characteristic of a dialysis facility with air temperature control and moderate noise. 
Ability to lift at least 30 pounds. Moving, lifting or transferring of patients may involve lifting of up to fifty (50) pounds.
