<div>Physical Therapist (PT) Responsibilities:
<ul><li> Provides patient/client care and assessment in accordance with established practice standards and facility/departmental policies.</li></ul>
<ul><li> Provides care per physician&rsquo;s orders and plan of care developed by evaluating therapist.</li></ul>
<ul><li> Takes measurements of patient/client status, within the limits of his/her licensure and ability, as appropriate in providing care and to assist in assessing the effectiveness of care.</li></ul>
<ul><li> Maintains communication and collaboration with the evaluating therapist, to promote maximum benefits from therapy. Summarizes patient performance according to established goals.</li></ul>
<ul><li> Implements education plan as outlined by the evaluating therapist, determines additional educational needs of the patient/client, family, caregiver or significant other as they arise and in collaboration with the evaluating therapist provides education to meet those needs.</li></ul>
<ul><li> Follows plan for discharge and discusses with evaluating therapist any additional discharge concerns which may arise. Assists therapist in goal reassessment and discharge planning.</li></ul>
<ul><li> Communicates/collaborates with patient, family, caregivers, significant other and other members of the healthcare team to promote maximum benefit of care.</li></ul>
<ul><li> Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient/client populations served.</li></ul>
<ul><li> Displays appropriate technical knowledge and application of available modalities and other equipment.</li></ul>

Summary: 
 
The physical therapist is responsible for providing service to the LIFE participant who has a physical impairment or delay and requires the skills of a therapist. The Physical Therapist designs and implements programs for the participant and family in accordance with agency standards and procedures. The Physical Therapist will also provide teaching to the family and caregivers as appropriate. May perform Early Intervention Program visits as approved and required. 
 
Responsibilities: 
 
EDUCATION 
<ul><li>
Licensed by N.Y. State Department of Education Required</li><li>
Graduate of B.S. or M.S. degree program in Physical Therapy Required</li></ul> 

EXPERIENCE 
<ul><li>
One year supervised experience as a Physical Therapist</li><li>
One year experience working with the frail elderly</li></ul> 

KNOWLEDGE, SKILL AND ABILITY
 
<ul><li>Must demonstrate the ability to adapt physical therapy techniques to the home/community setting</li><li>
Must be able to read, write, speak and understand the English language</li><li>
Must have knowledge of Physical Therapy procedures</li><li>
Communication skills which may be inclusive of approach, tone of voice, speed of communication, respectfulness and non verbal communication</li><li>
Physical assessment - Application of the assessment into the aging process; examples may include height/weight, skin integrity, ADL's, bowel and bladder control</li><li>
Technical skills - incorporates knowledge of age related physical and mental changes in performing technical skills; examples may include appropriate site for IM injection, range of motion and feeding skills</li></ul> 

WORKING CONDITIONS 
<ul><li>
Clinician delivers physical therapy care in home/community and/or LIFE setting working with participant, participant's family and any other available caregivers</li><li>
Maybe significant amount of time traveling, occasional exposure to unclean living environments and temperature fluctuations with seasonal changes</li></ul></div>
