Our rehab department consists of over 150 physical therapists, occupational therapists, and speech-language pathologists, who work collaboratively with a pediatric physiatrist and physiatry nurse practitioner. Rehabilitation Services are provided at both our Minneapolis and St Paul Hospitals as well as six outpatient therapy clinics.
Children&rsquo;s Pediatric Rehabilitation Medicine program brings together outpatient support for children with complex needs and dedicated inpatient consultative care to support the acute needs of hospitalized patients. Ambulatory clinics at Children&rsquo;s MN include primary care clinics that are the longest running Health Care Home in Minnesota for children with complex chronic health care needs; spina bifida clinic; NICU follow up clinics; and post-fetal (in utero) clinical care. Hospitalization efforts support a focus on tertiary- and quaternary-care, including a large oncology program, two level 3 NICUs, a pediatric CVICU and heart transplant program, two PICUs, level 1 pediatric surgery verification care, and pediatric level 1 trauma.
Position Description:
This individual will be responsible for managing complex patient care and ensuring the delivery of rehabilitation expertise to patients, families and other health care providers. This includes the evaluation of patients, in pediatric populations, with complex rehabilitation needs through outpatient and inpatient care. Responsibilities:
<ul><li>Provide specialized Physical Medicine and Rehabilitation (PM&amp;R)/Physiatry services as a rehab consultant</li><li>
Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.</li><li>
Collaborate with PCP teams to help with the management of conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries</li><li>
Work alongside your PT, OT, and SLP teams to improve patient function and quality of life</li><li>
Follow therapy patients to track their progress in therapy</li><li>
Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and much more!</li><li>
Become an integral part of the SNF team by attending multidisciplinary meetings, and bridging the gap between therapy and internal medicine</li><li>
Engage in culture building opportunities and ongoing education including monthly Didactic lectures, virtual meetings with your Regional Team, and annual, regional dinners to meet your nearby colleagues</li></ul>
The Ideal CRC Clinician will have:
<ul><li>A passion for post-acute healthcare and serving the geriatric population</li><li>
SNF or post-acute experience (preferred)</li><li>
Confidence to function autonomously</li><li>
A desire for continuous improvement and the ability to embrace change and growth</li><li>
Strong interpersonal skills and the ability to understand the needs of our facility partners and patients</li><li>
Empathy and compassion</li><li>
A desire to invest in our company culture</li></ul>
Qualifications:
<ul><li>Nurse Practitioner/APRN/ Physician Assistant</li><li>
Rhode Island License</li><li>
Ability to work in the U.S.</li><li>
Experience in post-acute space is preferred</li><li>
A great attitude and desire to deliver the best patient care</li><li>
40 hours/week</li></ul>
Job Type: Full-time
Pay: $112,000.00 - $132,000.00 per year
Schedule:
<ul><li>8-hour shift with flexibility, though hours should hover around 9 AM &ndash; 4 PM</li><li>
5-day work week with flexible days (between Monday &ndash; Friday); specific days can be selected during onboarding</li></ul>
Supplemental pay types:
<ul><li>Bonus pay</li></ul>
Ability to commute/relocate:
<ul><li>Middletown, RI, West Kingston, RI: Reliably commute or planning to relocate before starting work (Required)</li></ul>
License/Certification:
