Responsibilities:
<ul><li>Manages the provision of individual patient services and collaborates with clinical staff to develop rehabilitation plans. Coordinates the ongoing provision of rehabilitation services.</li><li>Formulates comprehensive, transdisciplinary rehabilitation treatment plans, clinical outcomes, goals/objectives and service delivery for referred patients in conjunction with the patient/care partner and the clinical team.</li><li>Completes CC evaluation and develops patient initial, progress and discharge reports and monitors the patient&rsquo;s status on an ongoing basis. CCs are responsible for completion of patient clinical outcome tools based on diagnosis.</li><li>Monitors patient, family, physician and payer/case manager, referral source satisfaction with the program on a regular basis.</li><li>Leads interdisciplinary case conferences, team meetings, and communicates Treatment Plans/Progress Reports on a regular basis to the physicians, external case manager/payor and/or patient/family members.</li><li>Schedules/facilitates interdisciplinary case conferences with the clinical team, payers/case managers, physicians and family members.</li><li>Communicates regularly with patient, family/ care partner, payer/case manager and physician regarding patient progress and any important issues that arise.</li><li>Facilitates acquisition of necessary equipment to enable achievement of agreed upon functional goals (i.e. HMO, insurance carrier, etc.) with funding source.</li><li>Participates in medical provider meetings to report/resolve medical issues for person served.</li></ul><div><ul><li> 
Monitors projected hours of service to be delivered to the patient and reports hours/subsequent changes to the local leadership (Executive Director/Director of Clinical Management/Program Manager).</li><li>Negotiates with funding sources for extensions of service beyond initial authorizations.</li><li>Acquires and tracks orders from physician for all provided ongoing services after the initial orders are obtained.</li><li>Adheres to standards of the Company's Code of Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.</li><li>Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy and regulations. Qualifications:
<ul><li>Minimum of Bachelor&rsquo;s degree in Allied Health (OT, PT SLP), Registered Nurse, Human Services, or</li></ul>
Rehabilitation Counseling/Psychology 
and 
<ul><li>Maintains relevant professional licensure/registration/certification in the state of practice.</li><li>Current certification as Certified Brain Injury Specialist (CBIS) preferred.</li><li>Current BLS (Basic Life Support) First Aid and CPR certifications.

<li>Monitors patient's physiological responses to therapy such as vital signs, arterial blood gases, and blood chemistry changes</li>
<li>Performs bronchopulmonary drainage and assists patient in performing breathing exercises</li>
<li>Performs pulmonary function tests to be used by Physician in diagnosis of case</li>
<li>Consults with Physician in event of adverse reactions</li>
<li>Maintains patient's chart that contains pertinent identification and therapy information</li>
<li>Inspects and tests respiratory therapy equipment to ensure equipment is functioning safely and efficiently</li>
<li>Maintains a collaborative working relationship with the nursing staff, subacute care unit, and collaborative working relationship with the unit&rsquo;s Clinical Care Coordinator, Director of Nursing and Administrator</li>
<li>Performs such other functions as are identified by the Facility&rsquo;s Clinical Care Coordinator and/or Director of Nursing, Administrator</li>
</ul>
PAY SCALE: $30 - $34 / Hour

Depending on a number of factors, including, but not limited to location, regularly scheduled work shift, knowledge, skills, experience, and expertise.
