We are seeking a passionate, driven physical therapist assistant to deliver therapy to adults with a variety of medical conditions including orthopedic, neurological, and cardiopulmonary. In this position, you will provide and direct physical therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication and functions under physicians&rsquo; orders. The resident population at the Bethany campus numbers around 400 living in multiple levels of care including apartments, assisted living, memory care, skilled nursing, and adult day services.

Position Summary:
The primary function of your job is to return patient to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification.
Education/Qualifications:<ul><li>
