Responsibilities<div> 
The Collaborating Physician works with an APN (or more than one) who is performing in home and/or telehealth wellness assessments on patients&rsquo; designated by HarmonyCares Medical Group. APNs working with the Collaborating Physician will not have prescriptive authority, nor will they be treating patients. As a Collaborating Physician you will:</div><ul><li>
Be available via telecommunication or in-person for consultation with APN to address patient assessment questions</li><li>
Regularly performs annual chart reviews</li><li>
Consults with APN ensure adherence to policies, procedures, state laws and quality</li><li>
