As the Licensed Practical Nurse (LPN), you will be responsible for providing direct resident care and supervision of residents&rsquo; needs based on their individualized service/care plan. You will monitor residents for changes in condition and signs/symptoms of illness, in accordance with federal and state laws and regulations, as well as the policies of the Community. In partnership with the Health Services Director, you will communicate and document changes or concerns regarding residents and assist in supervising the care team. 


The successful candidate for this position will have:
<ul>
<li>A passion for seniors and enhancing their quality of life</li>
<li>A valid Licensed Practical Nurse (LPN) license; a Compact license or the ability to obtain one is encouraged. An unencumbered LPN license is required.
</li><li>Minimum of at least one year of successful experience as an LPN in assisted living or long-term care</li>
<li>Experience administering medication and treatment plans per the doctor's orders and community policies</li>
<li>Knowledge of state regulations pertaining to resident care and understanding the importance of being state survey-ready every day.</li>
<li>Have experience working as a mandatory reporter and can immediately intervene, report, and investigate all suspected or alleged verbal, physical, sexual abuse, or theft from a resident.</li>
<li>Have a current driver&rsquo;s license.</li></ul>

Key responsibilities
 Provide direct nursing care within LPN scope under RN/physician direction
 Administer medications and treatments per orders and facility policy
 Perform wound care, dressing changes, catheter care, and specimen collection as assigned
 Monitor patient condition and promptly report changes to the RN/clinical supervisor
 Document care accurately and on time in the facility system
 Follow infection control, safety procedures, and HIPAA requirements
 Communicate effectively with patients, families, and interdisciplinary staff
 
Required qualifications
 Active LPN license (in good standing)
 1+ year recent LPN experience (SNF/LTC/rehab/hospital preferred)
 Current BLS/CPR (AHA preferred)
 Strong communication and documentation skills
 Reliable attendance for the full evening schedule
 
Compliance requirements (as applicable)
 Background check and drug screen
 Immunizations/health screenings per facility policy
 Ability to complete onboarding and credentialing within required timelines

The Licensed Practical Nurse (LPN) aides the medical providers with patient care through a variety of tasks. The LPN will prepare patients to be seen by the medical provider by bringing the patient to an exam room, taking vitals, and collecting the patient&rsquo;s health history. Prepares patients for the appointment by bringing them to an exam room, recording the patient&rsquo;s medical history, taking vitals, etc. Ensures all paperwork before, during, and after the patient&rsquo;s visit is correctly handled and filed to enable proper billing.</li><li>
Assists with patient care as directed by the medical provider. Accurately translates to facilitate medical provider/patient communication when appropriate.</li><li>
Administers injections, such as vaccines, as directed by a medical provider and in accordance with protocols.</li><li>
Maintains an accurate knowledge of all procedures offered and the components of the LPN&rsquo;s role in each. Participates in all staff trainings regarding new procedures and services.</li><li>
Prepares exam rooms for patient treatment per protocols. Maintains/cleans all exam rooms in compliance with protocols and procedures relative to infection control, exposure control and safety issues.</li><li>
Maintains adequate medical supplies and compiles a list of individual item shortages for inventory control and ordering purposes as assigned.</li><li>
Maintains patient records in the electronic medical records system, monitors patient flow, and assists the medical providers in assuring that all patient records and documents are properly and accurately completed and entered into the electronic medical records system.</li><li>
Receives and places necessary telephone calls consistent with professional matters, clinic business and patient care. Follows up on these calls promptly and directs calls to the appropriate personnel when necessary.</li><li>
Participates in appropriate health promotion/disease prevention activities and outreach efforts, both on-site and off-site as required.</li><li>
As directed by a supervisor, performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives.</li><li>
