The Job</b></p><p>
The Advanced Practice Clinician will provide and coordinate primary care to SCAN members to achieve the Company&rsquo;s mission to keep seniors healthy and independent.</p><p></p><p><b>
You Will</b></p><p>
Provide care to an assigned panel of members (patients) in their place of residence, most commonly in senior living communities or skilled nursing facilities. This includes conducting physical/wellness exams and assessments; occasional urgent medical visits and telephonic triage to ensure timely, appropriate care and hospital avoidance if possible; administering vaccines; ordering and interpreting diagnostic tests; initiating goals of care conversations with patients and their loved ones; and chronic disease management.</p><p>
Identify patient health care needs and develop a comprehensive plan of care for the patient, their family, and living-community staff.</p><p>
Collaborate with our case management team to ensure members have timely access to necessary and appropriate services.</p><p>
Independently manage visitation schedule in line with established practice protocols and adjust schedule based on members&rsquo; needs and acuity.</p><p>
Engage with Primary Care Physicians and other health care professionals in a collaborative relationship aimed at disease prevention, health promotion, and chronic disease management. Partner closely with Primary Care Physicians for clinical oversight and guidance.</p><p>
Connect with specialists, hospitals, ancillary care providers, and community-based organizations to ensure continuity of care and promote shared decision-making while advocating for members.</p><p>
Actively monitor and quickly address clinical quality gaps in care in partnership with other care providers.</p><p>
Document member visits and interactions in a timely manner according to established practice protocols.</p><p>
Serve as an ongoing direct resource for members and their families, as well as for their communities and support staff.</p><p>
Participate with Integrated Care Sales team in events to build member awareness and educate communities.</p><p>
Initiate and complete virtual health visits with members as needed to promote accurate triaging and reduce drive time.</p><p>
Participate in on-call program available to our members after hours and on weekends.</p><p>
Maintain professional and technical knowledge by attending educational workshops and reviewing professional publications.</p><p>
Contribute to team efforts to meet quality healthcare goals by closing care gaps, attending high risk group discussions, and optimizing clinical workflows and protocols.</p><p>

Your Qualifications</b></p><ul><li><p>
Master&rsquo;s degree in Nurse Practitioner Certificate OR Master&rsquo;s from an accredited PA program, Education in Adult/Geriatric medical care preferred.</p></li><li><p>

Monitors hospitalized patients, communicating and ensuring the team follows up as necessary.</li><li>
Completes requested schedules ensuring all orders and services (e.g. lab draw, supervisory visits, add-on visits or disciplines etc.) are included as required.</li><li>
Supports Clinical Manager and works collaboratively to ensure timely communication with patients, clinicians, referral sources, BD team and other office personnel.</li><li>
Runs all applicable reports and responds to workflow taking appropriate actions.</li><li>
Assists with internal or external transfer of patients between care centers and/or Hospice services.</li><li>
If a licensed clinician and meets all criteria to be a field clinician, may provide direct patient care as necessary and only in times of an emergency.</li><li>
May be responsible for maintaining supply closet with routine supplies, ensuring supplies are within expiration dates and packaged appropriately, and serve as liaison with the field for patient supply needs.</li><li>
In the absence of an Intake Coordinator, may be responsible for the referral and intake process within the care center.</li><li>
If a licensed clinician, may obtain verbal orders for patients and documents as applicable.</li><li>

Qualifications:
<div>Required-</div><ul><li>
Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to that state the employee is assigned to work by the company.</li><li>
High School diploma or GED equivalent.</li><li>
One (1+) year of administrative experience in a health care environment.</li></ul><div>

<div>The Clinician Support Specialist is responsible for enhancing the performance of the clinical care team through supporting and facilitating clinical services with their assigned providers. The Clinician Support Specialist is required to work in the field with the assigned provider as directed by Lead Clinician Support Specialist or Manager/Supervisor. You may be asked to travel to facilities up to 60 miles from your home.</div>

<ul><li>Ensure that the clinical provider in the assigned building is able to focus on taking care of their patients for the day</li>
<li>Ensure that each patient is seen as medically necessary per clinical protocol.</li>
<li>Serve as a &ldquo;buffer&rdquo; between provider and families/staff to facilitate proper patient management.</li>
<li>Document appropriate encounter and time for CCM and BHI services</li>
<li>If scheduling is assigned: Scheduling rosters should be updated daily to ensure accuracy and schedules sent timely.</li>
<li>Assist with any day-of changes to the scheduled visits due to acute needs, hospitalizations, admissions, or discharges</li>
<li>Prepare clinical documentation for assigned providers each day</li>
<li>Complete basic opening of encounter note as per discipline protocol</li>
<li>Perform medication reconciliation on all scheduled patients each encounter</li>
<li>Enter appropriate facility chart data on all scheduled patients each day into patient encounter notes and assign to provider</li>
<li>Enter lab and other diagnostic study results into patient encounter and assign to provider as per guidelines</li>
<li>Assist with collection and sorting of all clinical paperwork needed for clinician review and ensure completion each day</li>
<li>Host telehealth sessions for providers as assigned by manager/supervisor, both routine and acute visits</li>
</ul>
<div></div><div><b>Facility Support</b></div>
<ul><li>When scheduling is an expectation for your role:
<ul><li>Share patient schedules with key facility staff as needed.</li>
</ul></li><li>Assist with any triage needs as directed by assigned provider (whether they are in or out of the building that day)</li>
<li>Assist with triage documentation as needed.</li>
<li>Ensure that all needed paperwork is routed for the facility to Eventus providers, office, or families.
</li><li>Assist with consents for any services needed.</li>
<li>Attend stand up meetings to report any needed information from the facility to Eventus providers and vice versa if allowed by facility.</li>
<li>Assist with discharge planning of clinical services as needed.</li>
<li>Ensuring check in and out each day with facility staff regarding patients seen, status, etc.</li>
</ul>
<div></div><div><b>Patient Support</b></div>
<ul><li>Maintain patient confidentiality at all times.</li>
<li>Document appropriate encounter and time for CCM and BHI services</li>
<li>Check all forms of communication several times daily to ensure patient needs are met.</li>
<li>Inform patient and family of recent visits, new orders and defer to provider when needed</li>
<li>Communicate transitions of care to CCM team to enhance patient care and education.</li>
</ul><div>Additional Duties and Responsibilities</div>
<div><ul><li>Clinician Support Specialists are expected to be knowledgeable and adhere to Eventus WholeHealth standards, policies and procedures and outlined in employee handbook including HIPAA rules and regulations.</li>
<li>Clinician Support Specialists should adhere to the Eventus WholeHealth dress code policy i.e. clean, wrinkle-free, matching scrubs with appropriate, closed-toed footwear and name badge.</li>
<li>When duties are completed ahead of scheduled workday, report to your Lead Clinician Support Specialist for any additional assignments.</li>
<li>Clinician Support Specialists should seek further training if uncertain how to proceed with any assignment.</li>
